Salvage of the severely injured ear.

We present three cases of severe external ear injuries and their management. The first patient's ear, amputated in an automobile accident, was revascularized microsurgically. Replantation, when possible, offers the ultimate result. However, disadvantages include the length of the operation and prolonged hospitalization. The second patient sustained a human bite ear amputation. Salvage consisted of immediate coverage with a vascularized temporoparietal fascial flap and split-thickness skin graft. Revision of the external auditory meatus and postauricular sulcus was required. This technique is r

Immediate reconstruction of the auricle using the amputated cartilage and the temporoparietal fascia.

Successful restoration of the auricle following amputation using microsurgical techniques for reimplantation remains the best reconstructive alternative. In many cases of partial amputation, however, this option may not be technically feasible. Deepithelialization of the amputated part followed by coverage of the ear cartilage using an ipsilateral temporoparietal fascia turn-down flap may allow a single-stage salvage procedure that provides an acceptable final result.

Medial arm flap revisited.

Twenty fresh cadaver dissections examining the medial arm flap were completed. Blood supply to the medial arm skin was from the branches off the superior ulnar collateral artery (65%), direct cutaneous branches from the brachial artery (20%), or both (15%). Injection studies were completed demonstrating the extent of the medial arm flap. Four clinical cases were completed. The results in this article are different from some previous studies because they show that the superior ulnar collateral system is always present, the superior ulnar collateral vessels never directly supply the medial arm s

Microsurgical reattachment of totally amputated ears.

At a time when microsurgical reattachment of amputated limbs is commonplace, successful reattachment of total ear amputations is rare. Avulsed vessels, small diameters (0.3 to 0.7 mm), and technical difficulties provide primary obstacles. Three clinical cases of ear reattachments are presented which encompass the clinical spectrum of an uncomplicated success, a salvage of an impending failure, and a late failure due to venous thrombosis. In all cases there was difficulty with identification of vessels, with differentiation of arteries from veins, and in the performance of venous anastomoses. V

Successful replantation of amputated nose and auricle.

Amputations of the nose and of the auricle present difficult management problems. Application of simple reattachment techniques, followed by aggressive medical therapy which consists of cooling, anticoagulation, antibiotic coverage, and multiple stab incisions in the amputated tissues, have resulted in the successful replantation of major portions of an amputated nose and an amputated auricle with satisfactory cosmetic results.

Ear replantation: a case report.

This paper presents the fifth reported case of replantation of a traumatically amputated ear and its management. The ear was amputated in an automobile accident and was revascularized microsurgically. Replantation, when possible, offers the ultimate result; however, shortcomings include the length of operation, prolonged hospitalization, and postoperative management of venous congestion.

Ear replantation.

The authors' tactical and technical contribution to this type of microsurgical replantation can be summarized in three main points: (1) direct sutures to the superficial temporal vessels, when these are undamaged, which simplifies the operation and makes it safer; (2) use of Kunlin's technique to make up for the great difference in diameter between the arterial vessels; and (3) deepithelialization of the posterior auricular skin to allow for creation of venous drainage channels during the first postoperative days and the enlargement of the area of contact with the recipient bed, which also hel

[Amputation of the point of the nose in children due to a dog bite. Repair with a Washio flap. Long-term results].

Washio's temporo-retro-auricular flap was used to repair point of nose in three children and one ala nasi amputated by dog bites. Details of the technique are presented and long-term results discussed. Despite two inconveniences--dyschromia and relative stenosis of nares--this method compares favorably with other surgical procedures and over the time has proved to be one worth recommending.

Ear replantation combined with local flaps.

Various techniques of reattachment of the amputated ear have been described. Some of these reattachments may be successful, and some may survive but without good aesthetic results owing to changes in the cartilaginous framework or its skin cover, while many others may result in complete failure. To improve the blood supply to the reattached ear, especially the venous return, local flaps have been devised to cover part of the ear. Eight patients with partial or complete amputation of their ears have been treated with this technique. They have had good postoperative results and at least 6 months

Replantation of a totally amputated ear by means of a platysma musculocutaneous "sandwich" flap.

A total amputation of an ear from an automobile accident was salvaged by suturing the fragments of the skeletonized auricular cartilage and reattaching it to its original position "sandwiched" in a platysma musculocutaneous flap. The anterior portion of this flap was subsequently removed to apply a skin graft to the thin platysma muscle left over the convolutions of the auricular cartilage. This technique represents the application of the principle of bringing blood supply to avascular tissue in order to salvage it.

Use of resurfaced temporoparietalis flap in total ear reconstruction with less-than-favorable skin coverage.

Use of a vascular pedicled temporoparietalis muscle flap resurfaced with a full-thickness skin graft from the opposite ear, instead of less-than-favorable skin coverage, is advocated to achieve more satisfactory aesthetic results in total ear reconstruction handicapped by the presence of scarred local skin. The technical feasibility of the procedure and the authors' experiences in the past 9 years are described.

Contralateral local anaesthesia was employed in patients with chronic (n = 42) and acute (n = 8) pain syndromes (see Table 1). I. Phantom limb and stump pain: n = 10; II. Pain of face and ear: n = 10; III. Pain of neck and trunk: n = 14; IV. Pain of the hip joint: n = 6; V. Pain of the extremities: n = 10: Sa. = 50 patients. In 27 patients CLA displayed a clear effect upon the chronic pa

Traumatic amputation of the external ear: the War of Jenkins' Ear and George Washington.

Given the right circumstances, a traumatically severed ear can now successfully be reimplanted. The career of George Washington was influenced by the involvement of his half brother Lawrence Washington in a war (1739 to 1742) fought between England and Spain on account of a severed ear. It was called The War of Jenkins' Ear.

Behavior of the vestibular nerve following labyrinthectomy.

Transtympanic labyrinthectomy was performed on 24 cats, and after survival times of one month to three years, the temporal bones were prepared for light microscopic study. The operated ears showed mean neuronal losses of 12% in six months, 24% in 1 year, 35% in 2 years, and 53% in 3 years. These was no evidence of regeneration of vestibular nerve fibers nor of formation of traumatic neuromata. The temporal bones of two human subjects who had undergone transtympanic labyrinthectomy are also presented; one shows atrophy of the vestibular nerves while the other exhibits proliferation of nerve fib
