Severe auricular traumas with extensive involvement of the surrounding structures present with a serious defect necessitating free tissue transfers for reconstruction. In this case report, we present a case of whole left auricle reconstruction with a radial forearm flap prelaminated with porous polyethylene (Medpor®) implant in a 17-year-old female patient. First, a subdermal pouch was fashioned on the volar aspect of the left forearm along the projection of the radial artery and the Medpor implant was placed in this pouch. Four weeks later, the prelaminated radial forearm flap containing the

Amputation of an ear is a rare injury. Subsequent replantation is a challenging and very difficult task for an experienced micro-surgeon. Every well described successful replantation of an ear is a valuable lesson for a micro-surgeon mainly because securing the venous drainage of the replanted ear is very difficult. In the worldwide medical literature, only a minimal amount of cases have described a successful replantation of an ear after total amputation on a child. The authors describe the whole treatment process for a seven-year old boy whose ear was bitten off by a dog. They also describe

Microvascular ear replantation is a significant challenge because of the small size of the vessels and the fact that traumatic amputations are frequently avulsed. The zone of trauma is therefore extended and the primary repair of the injured vessel is rendered unlikely. The purpose of this study is to review the literature of ear replantation. A review of the relevant literature that has been published since 1980 revealed 47 cases reported in 37 publications. We present 5 cases from our own experience and analyze a total 52 cases of microvascular ear replantation. The patient's age, sex, degre

We present a case of successful total auricular reconstruction after total amputation of the auricle. Owing to the severe damage to the auricular cartilage, replantation was not attempted; instead, for the next step of the reconstruction, severed superficial temporal artery and vein were anastomosed. At the second stage, total auricular reconstruction was planned. A huge temporoparietal fascial flap was used (based on STA/STV), which was used to cover the costal cartilage ear framework. This case suggests that, even for the total loss of the auricle, autogenous reconstruction can still be succ

BACKGROUND: Reconstruction following traumatic amputation of the external ear remains a unique challenge to the plastic surgeon. The authors report a series of ear reconstructions with autologous costal cartilage in patients following traumatic partial amputation of the ear. Technical points regarding the carving of the cartilage framework and methods of skin coverage are discussed.
METHODS: Fifty partial ear reconstructions with autologous costal cartilage were performed over a 4-year period. All patients had suffered previous traumatic amputation of part of the external auricle due to bite

The retroauricular flap has many applications, providing coverage for various portions of the face as a pedicled flap or a free flap. The retroauricular skin, having similar attributes to the facial skin, is an ideal donor site for facial reconstruction. Also, the donor site can be primarily closed without morbidity, which is also an advantage. In this article, a large helical defect resulting from a traumatic amputation was reconstructed using the contralateral conchal cartilage graft and a retroauricular perforator-based island flap. Using the "perforator pedicled propeller flap" concept, no

Traumatic auricular amputation due to human bite is an uncommon event. Nonetheless, it poses a difficult challenge for the reconstructive surgeon. Although microsurgery must be performed in some cases, most microsurgical techniques are too complex and must be performed in specialized centers only. Replantation of a severed ear without microsurgery can be a safe alternative as long as a proper technique is selected. Here, 2 successful ear replants performed without microsurgical technique are described. The ear cartilage was used with the perichondrium after degloving of the skin from the carti

Traumatic auricular amputation due to human bite is not a common event, but it constitutes a difficult challenge for the reconstructive surgeon. Microsurgery can be performed in some cases, but replantation of a severed ear without microsurgery can be a safe alternative. We present a case of a 44-year-old male individual who was involved in a fight and sustained a human bite, resulting in almost a complete amputation of his right ear. The Baudet technique has been used successfully. It is a simple technique and very reliable because it allows a great surface of contact between the graft and th

Prosthetic rehabilitation of auricular defects can be a demanding procedure due to a broad variety of clinical presentations and a wide array of treatment options. The retention of auricular prostheses is a major factor influencing the successful outcome of rehabilitative treatment following cancer surgery or trauma. Auricular prostheses have been retained by methods including implants, adhesives and spectacle frames but the choice of repair ultimately depends on patient factors, amount of soft tissue loss, and location of the auricular defect. This article describes a newer technique of using

BACKGROUND: Ear reconstruction with autologous tissues and creating a natural-appearing ear is a difficult, multistage surgical procedure. Problems, such as a thick skin or an inadequate cartilage-flap relationship, may cause obliteration of the framework details. A nonsurgical technique to improve auricular definition is described.
MATERIALS AND METHODS: Twenty-six patients with reconstructed ears due to inadequate auricular definition were enrolled in the study. To achieve a better auricular shape, triamcinolone acetonide injections up to 4 sessions were applied. Results of injections were

PURPOSE: The purpose of this study is to present our experience with patients who underwent traumatic ear amputation.
METHODS: Between January 1988 and April 2002, 10 patients sustained ear amputations. Of these, six patients underwent microvascular replantation (arterial anastomosis only and arterial and venous anastomosis in three patients each), and replantation was attempted in one patient. However, no suitable vessel could be found for the anastomosis, and the amputated ear was treated as a composite graft and buried in a retroauricular pocket. Staged costal cartilage reconstruction was

Replantation of an avulsed ear was completely successful with only arterial repair (to the superficial temporal artery) and the use of medicinal leeches.

Microsurgical ear replantation is a significant challenge because vessel sizes are diminutive. Furthermore, as ear vessels are larger in the medial portion than in the lateral portion, microsurgical replantation of a replant including only the helix is extremely difficult. The authors replanted a partial helix of the ear using a single arterial anastomosis. As no suitable veins could be identified, medicinal leech therapy and systemic heparinization were used to achieve venous drainage. The replanted helix survived completely and the cosmetic result was excellent. Microsurgical replantation sh

In the microsurgical area, replantation of ear amputation by microvascular anastomosis is considered to be the best method in terms of texture, color, and shape. Only a few cases of successful ear replantation with microvascular anastomosis have been reported because the size of the vessels is very small, and identifying appropriate vessels for anastomosis is difficult. Furthermore, most cases were total or subtotal (upper two thirds of the ear) amputations, and they were successfully reconstructed by replantation. To the best of our knowledge, this is the first case of a patient with ear lobe

Total ear amputation is common, and management can necessitate different procedures, especially microsurgical anastomosis. Partial ear amputations supplied by narrow pedicles, however, have been reported rarely. In a subtotally amputated auricle, the chance of survival depends on the vascularization within the pedicle. In our case, the right ear of a 36-year-old male patient was subtotally amputated following a traffic accident, leaving only a 6-mm skin pedicle on the cranial side. The subtotally amputated segment was bleeding from the wound margins. The ear was reattached with primary suture

The reconstruction of facial parts after traumatic amputation is of special interest because of the psychological and functional implications such accidents have on the patient. Most amputations result from dog bites or knife wounds and are often sustained by children and young adults. The amputated part may be an anatomic structure of functional importance (e.g. , the lips) or a static structure of primarily aesthetic importance (e.g. , the ear or nose). Here, the Authors present results in 6 cases of facial amputation reconstructed using microsurgical replantation for upper lip amputation, w

Survival of the ear based on a single vascular pedicle has been previously documented. The anatomic basis for this survival remains incompletely described. In the 3 clinical cases of inferior-to-superior near-total ear avulsion presented herein, the authors have observed that the ear can survive based on a narrow pedicle along the helical root. In an anatomic study to further investigate the relevant vascular anatomy, the common carotid artery system of 6 fresh human cadaver specimens was injected with latex. The superficial temporal artery and its branches located at the helical root were exa
