Primary blast injury: update on diagnosis and treatment.

BACKGROUND: : Injuries from combat and terrorist explosions are increasing worldwide. As such, physicians can expect to treat more patients with complex and unique patterns of injury produced not only by fragments and blunt trauma, but also by high-pressure air expanding from the detonation center. DISCUSSION: : Tissue damage from the blast wave or primary blast injury can be an important cause of occult trauma to the ocular, aural, pulmonary, cardiovascular, musculoskeletal, and neurologic systems. Awareness of the extensive corporal effects of the blast wave is an essential prerequisite to

Successful replantation of the ear as a venous flap.

Successful microvascular ear replantation is well described, and often involves the use of arterial inflow anastomoses with leeches and systemic anticoagulation to augment outflow. Failed replantation attempts have also been published, with most of the failures attributed to venous drainage problems. A case of successful replantation of an ear is presented using a single 0.5-mm arteriovenous anastomosis, as no suitable artery could be identified in the amputated part. Postoperatively, dextran and leech therapies were employed to promote adequate venous drainage. We report a 100% successful ear

Totally avulsed ear: new technique of immediate ear reconstruction.

INTRODUCTION: Reconstruction of the avulsed auricle remains a challenge to the plastic surgeon. Many surgeons suggest reattachment of the total or subtotal amputated ear as a composite graft, but this solution is unreliable. Some surgeons advocate the use of the retro auricular pocket to enhance survival of the reattached ear. However, this burying method entails multistage procedures with somewhat poor results due to the associated fibrosis. Microsurgical replantation can be performed but its applicability is limited due to difficulty with venous drainage. In this paper we advocate the use of

[Replantation of the ear without microvascular anastomosis: a case report].

A 42-year-old man presented with a left-sided supralobular total ear amputation resulting from an occupational accident. Initial dissection of the amputated ear revealed no suitable vessel for anastomosis. The fenestrated cartilage with its posterior skin excised was attached as a composite graft. Four days postoperatively, it was observed that 90% of the graft was viable. Three weeks later, subtotal separation of the ear (80%) was performed combined with skin grafting for surfacing the postauricular defect. At one-year follow-up, there was partial cartilage resorption with pigmentation and mi

[Surgical management of traumatic ear amputations: literature review].

Traumatic ear amputation (TEA) is a complete avulsion of a part or of the total auricular tissue. TEA are rare (only 74 cases have been described in the literature) and their handling is complex. The surgeon's objective is to obtain the best cosmetic result without demolishing the auricular area in order to allow future ear reconstruction in case of replantation failure. Many techniques of ear replantation have been described in the literature during the last 30 years: microsurgical replantation, pocket techniques and reattachment techniques. Microsurgical replantation should be achieved every

Immediate or delayed nasal reconstruction in infant after subtotal amputation? Nasal reconstruction with forehead flap in a 2-year-old child.

Nasal reconstruction using the forehead flap is one of the oldest recorded reconstructive procedures. The forehead flap is considered a standard for reconstruction of large defect. Most of reports concerning the forehead flap in nasal reconstructions deal with adults. Nasal reconstruction in infants raises some controversies, and very few articles report nasal reconstruction in the infants. A 15-month-old boy was presented after subtotal nasal amputation, after a dog bite. Microsurgical replantation was performed, but proved unsuccessful. The wound healed with debridement and local care. At ag

[Optimal result after ear reconstruction following a dog bite injury].

A case of a young woman is reported who lost a big part of the right auricle by a dog bite. An aesthetically satisfying immediate reconstruction after refixation of the deep-ithelialised amputated helical rim could be achieved with the aid of a temporal fascial island flap and full thickness skin grafts.

Successful microsurgical replantation of a child's completely amputated ear.

We present a case of a 9-year old boy who underwent successful replantation of a completely amputated ear. Few reports have been published on replantation of children's ears that are completely avulsed. Venous drainage is a well-known factor contributing to the failure of completely amputated ear replantation. In this case, we used a technique of transillumination to examine the amputated ear, and selected veins for exploration. Finally, a suitable vein was found and anastomosed to the one on the recipient side. The ear seemed to be completely viable once the problems of arterial blood supply

Microvascular ear reconstruction using a free radial forearm flap after dog bite.

A 68-year old woman suffered from a subtotal amputation of the left ear due to a dog bite. The recovered ear was implanted and reconstructed using a radial forearm flap. In our opinion, the forearm is an excellent choice for implantation of recovered ear cartilage as preparation for one-stage reconstruction.

Replantation of an avulsed ear, using a single arterial anastamosis.

Avulsion of the ear is relatively uncommon and replantation a technical challenge. A case in which an avulsed ear was successfully replanted using a single arterial anastamosis is described. The surgical difficulties encountered, the pharmaceutical approach to postoperative care and the problems which resulted from the lack of venous drainage are discussed.

Delayed, bilateral, non-microvascular ear replantation after violent amputation.

Amputation of any body part is undoubtedly a traumatic experience leaving a terrible deformity, especially when the part or parts involved are visible and constitute an essential component of someone's facial whole. Bilateral ear amputation and successful subsequent replantation has been reported historically, but not in the modern surgical literature. We report the case of a 27-year-old female who was abducted and suffered a bilateral ear amputation at the hands of one of her captors to speed delivery of ransom money; the severed parts were sent to the parents approximately 2 hours after the

[Near-total ear amputation: anatomical bases and management].

BACKGROUND: In cases of near-total ear avulsions, replantation is often successful without microsurgery. The purpose of our study was to investigate the relevant vascular anatomy associated for ear survival. PATIENTS AND METHODS: Four cases of successful surgical intervention in near-total ear avulsions are presented. Injection studies using latex were performed to identify the blood supply to the auricle on 13 cadaveric ears. RESULTS: A small superior branch of the superficial temporal artery above the tragus was identified extending along the upper border of the auricle and connecting with

Temporal flap variations for craniofacial reconstruction.

LEARNING OBJECTIVES: After studying this article, the participant should be able to: 1. Describe the anatomy of the temporal region. 2. Identify the types and possible indications of the temple group flaps to be selected depending on the defects, and then raise the flap safely for reconstruction. 3. Predict and manage successfully the potential complications of surgery. BACKGROUND: Good harmony of color and texture with surrounding tissues, thinness and adequate pliability, good alignment, obliteration of the cavities, and minimal donor-site morbidity are the main features of an ideal flap to

A new method in the treatment of ear amputation: experimental and clinical study.

There have been plenty of reconstruction methods for ear amputation, and replantation preserves its importance. In situations where replantation is not feasible, various methods were proposed. We indicate an alternative technique for the ear amputation without replantation indication. The method of replacing of a vascular structure into the tunnel formed on the posterior side of the amputated ear was used instead of replacing the ear cartilage into a vascular area that was described in the literature of ear prefabrication. The dorsal fascial flaps which were prepared from the back of 10 New Ze

[Replantation of traumatic amputated ears by Mladick procedure: 6 cases].

UNLABELLED: Traumatic ear amputations are relatively rare. Whenever possible, ear reimplantation should be attempted, however the choice of the surgical procedure must be judicious. PATIENTS AND METHODS: We present 6 cases of partial ear amputation treated by the pocket technique described by Mladick. The series comprises 5 partial amputations severing less than half of the auricule and 1 subtotal amputation preserving the ear lobule. RESULTS: Necrosis of the avulsed fragment occurred in the only case of subtotal amputation. In the 5 other cases the revascularisation of the severed part was

Versatility of the bilobed cervical skin flap for total ear reconstruction in the unfavorable temporoauriculomastoid region.

BACKGROUND: The unfavorable temporoauriculomastoid region is a serious clinical problem that makes total ear reconstruction a real challenge. In patients who have it, all local flap solutions to provide skin coverage to the cartilage frame are precluded because of loss and/or severe scarring of the regional skin in association with loss of the axial vascularity of the region. In the current literature, all available techniques for these patients are lengthy and risky free flap procedures that can provide a poor quality of skin coverage with no aesthetic harmony at best. In this article, the au
