A comparison of ear reattachment methods: a review of 25 years since Pennington.

BACKGROUND: A failed ear reattachment tends to complicate subsequent reconstruction with autogenous rib cartilage. The various reattachment methods differ in technical demand, in the secondary damage of periauricular tissue, and in aesthetic outcome. In the past 25 years, the microsurgical method has proven to be especially successful. The aim of this work was to assess case reports of auricular trauma since the first microsurgical ear replantation by Pennington. METHODS: An extensive review of the literature of acute ear trauma published between 1980 and 2004 was performed. For a better comp

Nonmicrosurgical reconstruction of the auricle after traumatic amputation due to human bite.

BACKGROUND: Traumatic auricular amputation due to human bite is not a common event. Nonetheless, it constitutes a difficult challenge for the reconstructive surgeon. Microsurgery can be performed in some cases, but most microsurgical techniques are complex and their use can only be advocated in specialized centers. Replantation of a severed ear without microsurgery can be a safe alternative as long as a proper technique is selected. METHODS: We present two cases, one of a partial and one of a total traumatic auricular amputation, both caused by human bites, that were successfully managed in o

Three cases of successful microvascular ear replantation after bite avulsion injury.

We present three cases of sub-total amputation of the external ear caused by bite avulsion injury. The ears were all successfully replanted despite us being unable to perform a venous anastomosis in one case. These outcomes support attempted microsurgical replantation for total or sub-total amputations of the ear, as successful replantation is the most effective surgical option.

Successful auricle replantation via microvascular anastomosis 10 h after complete avulsion.

OBJECTIVE: To report a case of successful total auricle replantation 10 h after complete amputation. MATERIAL AND METHODS: Replantation was achieved by means of microvascular anastomosis and other therapies, and assisted by careful nursing. The patient's auricle was completely cut off with a sharp blade by an assailant, who retained it for 5 h. Having been retrieved, it was then preserved in ice for 5 h. Using microscopy, an artery and vein were found at the confluences of the upper and middle and lower and middle parts of the amputated auricle and head wound, respectively An end-to-end anast

Enhancement of auricular composite graft survival with hyperbaric oxygen therapy.

OBJECTIVE: To continue investigation regarding the efficacy of hyperbaric oxygen (HBO) therapy in improving survival of reattached auricular composite grafts. DESIGN: A prospective, randomized, double-blind study using 20 New Zealand albino rabbits randomized to a treatment or control group. The treatment group received 30 HBO treatments over 19 days following amputation and reattachment of composite auricular grafts. The control group received standard care. Ears were examined grossly and microscopically on postoperative day 21 to determine the percentage of graft survival. RESULTS: The mea

Supramicrosurgical ear replantation: case report.

Successful microvascular ear replantation is rarely reported, and most of the time involves the use of leeches for venous congestion. A case of microvascular replantation of the ear is reported in which two arterial inflow anastomoses and two outflow anastomoses (venovenous and arteriovenous) were performed. Postoperative, prostaglandin E1 (PGE1) was given. The replant was 100 percent successful, and no venous congestion developed.

[Development of surgical antibioprophylaxis kits: evaluation of the impact on prescribing habits].

In our hospital, surgical antibioprophylaxis (ATBP) was too often administered too late, thus raising the infectious risk. Antibiotic stocks of the anaesthesia department were also systematically used, instead of nominal prescriptions of these drugs. The pharmacy could neither charge antibiotics to each surgical department nor quantify and differentiate ATBP from curative antibiotic therapy. The pharmacy and anaesthesia departments therefore set out to standardize surgical ATBP, in order to adapt this treatment to each surgical indication, and particularly in the case of allergy to beta-lactam

Applications of hyperbaric oxygen in otolaryngology head and neck surgery: facial cutaneous flaps.

Hyperbaric oxygen therapy is of significant benefit in the setting of an ischemic flap of the head and neck. Mechanistically, hyperbaric oxygen decreases local tissue edema and improves oxygen delivery to compromised tissues. Both capillary and fibroblast in-growth occur at a greater rate with hyperbaric oxygen therapy, and there is an increase in the tensile strength of the wound. Additional indications in the head and neck include traumatic composite amputations, necrotizing soft-tissue infections, and osteoradionecrosis of the facial skeleton.

[Localized loss of facial tissue. Criteria of use of extra-oral implants and clinical cases].

Osseointegrated maxillofacial epitheses are an interesting therapeutic response for rehabilitation after loss of facial tissue when reconstructive plastic surgery cannot be proposed. We analyzed the criteria used for deciding on this type of treatment for the loss of local areas of auricular tissue, the nasal pyramid and the orbitopalpebral region, presenting individual clinical examples.

[External ear injuries--classification and therapeutic concept].

The treatment of traumatic defects of the auricle concerning the exact reconstruction of details and aesthetic aspects is exacting for the surgeon. To face the expectations towards best trauma care, the different locations and extent of auricular injuries should be included in the different therapeutic strategies. The insufficient treatment results in mostly difficult secondary conditions. We present a classification for ear traumas, which considers size and location of defects. Depending on this, different techniques and methods of reconstruction are recommended. Advantages and disadvantages

Reconstruction of a subtotally amputated auricle: a case report.

A 42-year-old woman presented with a subtotal amputation of the left auricle except for the helical skin. The treatment was by primary suture with debridement of some margins, and the auricle healed well. The fact that the treatment was successful without microvascular anastomoses is important, considering the anatomical features of the auricular vascular networks.

Microsurgical replantation of a totally amputated ear without venous repair.

The authors present a case of successful replantation of a totally avulsed ear in a 37-year-old patient, using a termino-terminal arterial anastomosis on the superficial temporal artery. As no suitable vein could be identified in the amputated part, they relied on leech therapy and systemic anticoagulation for venous drainage. Despite an 18-hr ischemia, the ear survived completely.

Successful partial ear replantation after prolonged ischaemia time.

We present the case of a 34-year-old male patient who had successful replantation of upper pole of pinna 33 h after amputation. As no vein was anastomosed, systemic heparinisation and subcutaneous injection of heparin to the replanted ear were used to encourage outflow. Complications included arterial spasm and bleeding. Management of similar cases as planned urgent cases rather than emergency cases is discussed.

Salvage of ear replantation using the temporoparietal fascia flap.

The authors report a case involving a 46-year-old man who sustained a traumatic amputation of approximately 60% of his ear from a human bite. The ear was replanted microsurgically without the benefit of venous anastomosis. Blood transfusion was not required despite the use of leech therapy and systemic anticoagulation. The replantation appeared to be a success at the time of his discharge from the hospital, but during the late postoperative period the replanted ear became progressively necrotic. The failing ear replantation was rescued successfully with the use of temporoparietal fascia flap r

[Subcutaneous preservation of an amputated auricle. Morphological changes].

BACKGROUND: Microsurgical replantation of an avulsed auricle remains a challenge in reconstructive surgery. Secondary reconstruction of a traumatic lost auricle is usually performed using a costal cartilage framework according to well documented techniques or with a prosthesis. In order to minimize donor-site morbidity, various efforts can be undertaken to preserve the amputated auricle by implanting the de-epithelialized cartilage framework in a subcutaneous pocket on the surface of the mastoid. Where preservation is successful, this original cartilage could be used for reconstructive treatme

Reconstruction of the upper portion of the ear by using an ascending helix free flap from the opposite ear.

Reconstruction of partial, marginal defects of the ear has been a challenge. The ascending helix free flap based on superficial temporal vessels has been described and used solely to repair nose defects. We used reversed pedicle helical free flap for the repair of a major loss of the upper one-third of the opposite auricle. The method permits the transfer of tissue of the same quality with satisfactory cosmetic result. The equalization of the ears in dimension was accomplished with minimal donor-site deformity.

Single-vessel arteriovenous revascularization of the amputated ear.

Successful single-vessel arteriovenous replantation of a completely amputated human ear is described. This result was followed by an experimental study using a rabbit model, to confirm that an amputated ear could survive replantation with only a single arteriovenous anastomosis. Fifteen animals were placed in one of two experimental groups: Group 1-arteriovenous replantation, no treatment (n = 6); and Group 2-arteriovenous replantation with medicinal leeching (n = 9). All ears demonstrated initial reperfusion of the replantation immediately following microanastomosis. Laser Doppler flow readin

Use of a platysma myocutaneous flap for the reimplantation of a severed ear: experience with five cases.

CONTEXT: The traumatic loss of an ear greatly affects the patient because of the severe aesthetic deformity it entails. The characteristic format of the ear, with a fine skin covering a thin and elastic cartilage, is not found anywhere else in the human body. Thus, to reconstruct an ear, the surgeon may try to imitate it by sculpting cartilage and covering it with skin. OBJECTIVE: To use a platysma myocutaneous flap for the reimplantation of a severed ear in humans. DESIGN: Case report. SETTING: Emergency unit of the university hospital, Faculty of Medicine, Ribeirão Preto - USP. CASE REPO
