Effective management of traumatic venous sinus injuries in a resource-limited health system: a retrospective cohort study.

OBJECTIVE: Traumatic venous sinus injury (VSI) is a rare but serious complication with high rates of morbidity and mortality. In resource-limited health systems, restricted access to advanced imaging and high trauma volumes further complicate care. The objectives of this study were to quantify the effectiveness of a resource-constrained imaging protocol, evaluate its impact on outcomes, and identify predictors of VSI. METHODS: The authors conducted a retrospective cohort study of adults with skull fractures overlying dural venous sinuses at Tygerberg Academic Hospital, a resource-limited publ

Doppler High-Frequency Ultrasound in Vascular Complications.

The increase in the number of injectable filler procedures has led to a rise in associated complications, with vascular compromise being the most feared. As a result, blood flow can be interrupted, leading to ischemia, necrosis or ulcerations, pulmonary embolism, blindness, and cerebral vascular injuries. In this context, Doppler high-frequency ultrasound (DHFUS) is a radiological tool that can be used to avoid, to diagnose, and to treat vascular complications. This study aims to review facial vessels anatomy and major variations, describe the examination technique, probe positions and teach h

Penetrating dural venous sinus injury: a scoping review of clinical presentation, operative management, and prognosis.

Penetrating injury to the dural venous sinuses (DVS) commonly occurs at the superior sagittal sinus (SSS), transverse sinus (TS), and torcular confluence. While rare, DVS penetrating injuries carry high risk of intracranial hemorrhage, air embolism, and venous infarction. The current literature pertaining to penetrating DVS injury consists primarily of isolated case reports and small series. This scoping review consolidates published cases to characterize mechanisms, management strategies, and clinical outcomes pertaining to penetrating DVS injury. A scoping literature search of PubMed, PubMed

Direct Anterior Approach to Hip Resurfacing Arthroplasty.

BACKGROUND: Hip resurfacing arthroplasty remains an appealing option for young and active patients1, but its use has declined because of technical challenges, risks of femoral neck notching, and adverse reactions associated with metal-on-metal bearings, raising concerns regarding long-term implant survival2. Accurate preparation of the femoral head and acetabulum is critical for successful outcomes3. In the present video article, we describe a reproducible technique utilizing preoperative 3D computed tomography (CT)-based planning and patient-specific 3D-printed jigs to optimize femoral head p

Morphometric analysis of the C2 transverse foramen and vertebral artery using computed tomography angiography.

BACKGROUND: The anatomical relationship between the C2 transverse foramen (TF) and the vertebral artery (VA), particularly at the superior and inferior openings of the C2 TF, is clinically important in upper cervical spine surgery because of the potential risk of VA injury. This study aimed to quantify the morphology of the C2 TF and VA using computed tomography angiography (CTA), compare the superior and inferior openings, and evaluate the correlation between TF area and VA area. MATERIALS AND METHODS: Head and neck CTA images from 80 subjects were retrospectively analyzed. Curved multiplana

Vessel Sealing Systems in Veterinary Oral and Maxillofacial Surgery: Current Evidence and Clinical Applications.

Vessel sealing systems (VSSs) are medical devices that use bipolar electrothermal energy to seal blood vessels by denaturing collagen and elastin in the vessel wall, successfully occluding the vessel and reducing blood loss during surgical procedures. These devices eliminate the need for conventional ligatures or surgical clips in many situations and have become widely adopted in both human and veterinary surgery. Reported advantages include reduced operative time, improved hemostasis, decreased blood loss, and the ability to perform dissection and vessel sealing with a single instrument. The

Mechanical stiffening and delayed cell death of endothelial cells exposed to impulsive pressure.

Head impact induces rapid fluctuations in intracranial pressure, which are implicated in vascular damage even in the absence of skull fracture. Endothelial cells lining the inner lumen of blood vessels sense mechanical stimuli and transduce them into biological responses. We hypothesized that impact-induced pressure fluctuations alter endothelial cell mechanics and contribute to delayed endothelial injury. In this study, impulsive pressures with amplitudes and frequencies comparable to those generated during head impact were applied to cultured endothelial cells. Cellular stiffness was evaluat

Avoiding Iatrogenic Axillary Artery Injury in Reverse Total Shoulder for Fracture: The Use of a Separate Axillary Incision: A Case Report.

CASE: A 71-year-old woman sustained a left proximal humerus fracture dislocation. Imaging demonstrated displacement of the humeral head medial to the axillary artery without initial injury. To avoid iatrogenic injury, a separate medial axillary incision was successfully used to remove the humeral head and then reverse total shoulder arthroplasty was performed through a deltopectoral approach. At 1 year, the patient had excellent range of motion and PROMIS Physical Function CAT (v2.0) score was 46.2. CONCLUSIONS: A separate medial axillary approach can be used to safely remove anterior/inferio

Temporary Cement Prosthesis as a Bridge After Control of Axillary Artery Hemorrhage in Proximal Humeral Fracture-Dislocation: A Case Report.

CASE: A 64-year-old woman underwent open reduction and internal fixation for four-part proximal humeral fracture-dislocation with palpable distal pulses. Upon dislocated head removal, life-threatening hemorrhage occurred from posterior circumflex humeral artery tear. Bleeding was controlled through packing, resuscitation, and open vascular repair. Owing to unavailable shoulder prosthesis and prolonged operative time, a temporary cement prosthesis with Steinmann pins was used. At the 3-year follow-up, partial function was restored with no loosening. CONCLUSION: This case highlights axillary br

A minimum of 16° of rotation of transcervical femoral neck fractures leads to arterial collapse: an anatomical cadaveric study.

PURPOSE: Avascular necrosis (AVN) of the femoral head is a significant complication following femoral neck fractures. Rotational deformity has been proposed as a mechanism of vascular compromise, yet the degree of rotation required to interrupt intracapsular blood flow remains undefined. This cadaveric study quantified the degree of external rotation required to produce angiographic occlusion of the femoral circumflex arteries. METHODS: Twelve cadaveric hips underwent catheterization of the deep femoral artery with radiocontrast injection under fluoroscopy to visualize the medial and lateral f

Forecasting the rise of middle meningeal artery embolization for chronic subdural hematoma patients: United States projections from 2022 to 2050.

OBJECTIVE: Chronic subdural hematomas (cSDHs) are increasingly common in aging populations, driven by the rising use of anticoagulant and antiplatelet therapies. Middle meningeal artery embolization (MMAE) is a promising minimally invasive treatment. This study projects the future US incidence of cSDH through 2050 and estimates MMAE demand based on hematoma size criteria. METHODS: The authors utilized data from the National Inpatient Sample from 2016 to 2019 to calculate age-specific incidence rates of cSDH. Institutional CT scans of the head from a level I comprehensive trauma center were an

Cell therapy for stroke and spinal cord injury in clinical trials.

Brain injury describes a variety of injuries to the tissues and blood vessels in the head. It can be external such as in road accident or physical assaults or internal such as a stroke. Regardless, they are largely incurable with a long route to recovery with symptoms-relieving medications and rehabilitation. As such, many therapies were explored including cell therapy. However, not all were evidence based and in many instances banking of patients' desperation, many treatments were done without any evidence from clinical trials. Here, we reviewed clinical trials on clinicaltrials.gov for strok

Venous sinus stenting: where does it fit in the treatment paradigm for cerebrospinal fluid leaks?

OBJECTIVE: Spontaneous CSF leaks are traditionally managed with surgical repair. However, a growing body of evidence suggests that some of these leaks may result from idiopathic intracranial hypertension (IIH) and/or elevated intracranial pressure (ICP) due to underlying venous sinus stenosis-an association that remains underrecognized. Venous sinus stenting (VSS) may address CSF leaks by restoring normal venous outflow and reducing ICP. However, few series have evaluated VSS as a primary or adjunctive treatment for CSF leaks. Herein, the authors present a case series highlighting the role of

A Systematic Review With Meta-analysis of the Mastoid Emissary Foramen: Implications for Posterior Cranial Fossa Surgery.

INTRODUCTION: The mastoid emissary foramen (MEF), transmitting the mastoid emissary vein, is a key anatomical structure encountered during posterior cranial fossa and cerebellopontine angle approaches. Injury to this variable venous pathway may result in significant hemorrhage. However, published data regarding the prevalence and morphology of the MEF remain inconsistent. METHODS: A systematic review with meta-analysis was conducted according to PRISMA 2020 and Evidence-based Anatomy guidelines. Four databases were searched for osteological and imaging studies reporting MEF prevalence or morp

Quantitative analysis of macular and peripapillary microvasculature in non-arteritic anterior ischaemic optic neuropathy.

CLINICAL RELEVANCE: Non-arteritic anterior ischaemic optic neuropathy is a major cause of optic nerve dysfunction in older adults. Understanding its structural and microvascular consequences is essential for identifying disease susceptibility and guiding clinical monitoring. BACKGROUND: Non-arteritic anterior ischaemic optic neuropathy results from impaired perfusion of the optic nerve head, leading to permanent structural and vascular injury. Although clinically affected eyes show marked damage, fellow eyes may also demonstrate anatomical crowding and reduced vascular supply. METHODS: This

Impact of middle meningeal artery embolization on neurological complications and discharge outcomes in chronic subdural hematoma: A machine learning-based comparative study.

INTRODUCTION: Chronic subdural hematoma (cSDH) predominantly affects older adults, often those with prior head trauma, anticoagulation therapy, or chronic comorbidities. Traditional management involves surgical evacuation; however, middle meningeal artery (MMA) embolization has emerged as a less invasive alternative with the potential for fewer complications. This study compares outcomes of surgical evacuation, MMA embolization, and combination therapy using machine learning to identify predictors of neurological complications and discharge disposition. METHODS: Patients with cSDH from the 20

Transverse Facial Artery Morphometry Based on Head Dimensions and Palpable Anatomical Landmarks.

BACKGROUND: The transverse facial artery (TFA) is a key vascular structure supplying the lateral region of the face. Because of its central location, the artery is vulnerable to transection during lateral facial rejuvenation and other invasive facial procedures. OBJECTIVES: The authors of this study aim to determine the location of the TFA origin relative to 2 palpable bony landmarks: the head of the mandible and the upper border of the zygomatic arch. METHODS: Thirty hemifaces from 15 embalmed cadaveric heads were examined. Two palpable landmarks-the upper border of the zygomatic arch and t

Cerebral air embolism from oesophageal-pulmonary vein fistula induced by fish bone ingestion.

Fish bone ingestion is common, but cerebral air embolism as a secondary complication is exceedingly rare. A man in his 80s presented to our tertiary emergency centre after sudden collapse following oesophageal discomfort from fish bone ingestion the previous day. Head CT revealed punctate hypodense foci in the right frontal white matter, consistent with cerebral air, while CT perfusion showed decreased cerebral blood flow with delayed transit time. Chest CT demonstrated an impacted fish bone in the distal oesophagus and air bubbles in the pulmonary vein. Urgent endoscopy confirmed the fish bon
