The Safe and Appropriate Use of a High-Speed Drill.

A high-speed drill is an essential tool for any skull base surgery. However, if not appropriately used, it may cause serious inadvertent complications. Here, the authors present one case of surgery for a paraclinoid aneurysm of the internal carotid artery, in which tissue grabbing occurred during the drilling of the anterior clinoid process. This resulted in venous injury to the vessels of the nearby Sylvian fissure. Fortunately, the injury was manageable and caused no significant problems. The authors discuss the basic and other important points for the safe use of a high-speed drill. Discuss

Middle Meningeal Artery Embolization in Chronic Subdural Hematoma in Post Shunted Normal Pressure Hydrocephalus.

BACKGROUND: Shunted hydrocephalus patients are at an inherent risk of developing chronic subdural hematomas (cSDHs) from overdrainage. The preliminary results of three much-anticipated clinical trials (STEM, EMBOLISE, MAGIC-MT) have recently been presented, showing efficacy of middle meningeal artery embolization (MMA-E) in reducing observational failure and the need for reoperation in non-shunted patients, but the effect of this procedure in shunted patients remains to be further investigated. In this series, we present our institution's experience with MMA-E in shunted normal pressure hydroc

The Application of the Posterior Atlanto-Occipital Membrane Tension Release Technique for Surgical Exposure of the Horizontal Part of the Vertebral Artery's Third Segment: An Anatomical and Clinical Investigation.

OBJECTIVE: This study aims to elucidate the anatomical principles governing the surrounding venous structures (VS) of the horizontal part of the third segment of the vertebral artery (V3h) and develop a safe and bloodless surgical technique for exposing V3h. METHODS: This study used 10 formalin-infused cadaveric head specimens. The dissections were performed stepwise to simulate the far lateral approach process, exposing the V3h with a novel technique. Additionally, we applied this technique to 10 patients undergoing far or extreme lateral approaches. RESULTS: The VS surrounding V3h is divid

Displaced cortical vein sign on CT in infants: a reliable predictor to distinguish low-attenuation subdural collections from benign enlargement of subarachnoid spaces.

Differentiating benign enlargement of subarachnoid spaces (BESS) from low-attenuation subdural collections on CT imaging of infants can be challenging. This distinction is crucial in infants, as subdural collections may raise the concern for abusive head trauma (AHT). To evaluate the utilization of the displaced cortical vein sign on CT as a predictor of pathological subdural collections confirmed by MRI and to assess the reproducibility of this finding among radiologists with different levels of clinical experience. A total of 104 imaging exams were independently reviewed by junior- and senio

Venous sinus stenting for management of spontaneous skull-base CSF leaks: A systematic review and meta-analysis.

BackgroundIdiopathic intracranial hypertension (IIH) is strongly associated with spontaneous skull-base cerebrospinal fluid (CSF) leaks. Venous sinus stenting (VSS) has proven effective for the treatment of IIH. Hence, its role in spontaneous skull-base CSF leaks is being explored actively.MethodsWe performed a systematic literature search across EMBASE, MEDLINE, Scopus, The Cochrane Library, and Google Scholar to identify studies reporting the use of VSS for spontaneous skull-base CSF leaks. Studies with pediatric patients, non-English articles, and nonspontaneous leaks were excluded. Failure

Topographic and anatomical variability of the facial artery: Structure and physiology.

A large number of complications associated with incorrect visualization of the facial artery or ignorance of the anatomy of the major facial vessel make this problem quite relevant. An in-depth understanding of anatomical variations, projections and topography, taking into account the depth of the facial artery, can help reduce the risk of adverse outcomes during cosmetic procedures. In addition, it assists plastic and maxillofacial surgeons in managing facial trauma and performing surgical procedures, including advanced endoscopic facelift techniques and endoscopic blepharoplasty, where the r

Vascular Response, Downstream Effect, and Pharmacokinetics After Sirolimus- and Paclitaxel-Coated Balloons in Porcine Coronary Arteries.

BACKGROUND: Paclitaxel (PCB) and sirolimus-coated balloons (SCB) are major therapeutic options for coronary artery disease, but there is no direct head-to-head histological comparison of their effects during the percutaneous coronary intervention (PCI). AIMS: We aimed to investigate the vascular and downstream effects and drug pharmacokinetics in a porcine coronary model treated with MagicTouch-SCB (MT-SCB), Selution-SRL-SCB (SEL-SCB), Agent PCB, and plain old balloon angioplasty (POBA). METHODS: Twenty-eight coronary arteries from 10 pigs were treated with one of three drug-coated balloons

Repeated loading and damage progression in cerebral arteries.

Traumatic brain injury poses a major public health challenge with significant immediate and long-term effects. Repetitive head trauma is an ongoing area of research, and little is known about the response of cerebral blood vessels to such loading. This study investigated the mechanical response of cerebral arteries to repetitive overstretch, hypothesizing that repeated overstretch leads to cumulative damage. To test this hypothesis, middle cerebral artery segments from twelve piglets were subjected to sub-yield, high-rate overstretch of varying severities, with up to 10 repetitions. The stress

Age matters: A Secondary Analysis of Endothelial Biomarkers in the Prehospital Tranexamic Acid for Traumatic Brain Injury Trial.

BACKGROUND: Injured older adults account for nearly 25% of trauma admissions nationwide with increased morbidity and mortality compared with younger adults. Endothelial dysfunction has been associated with poor outcomes in trauma patients. We hypothesized that posttraumatic endothelial changes in older versus younger adult trauma patients will be different with worse outcomes in older adults. METHODS: This is a retrospective secondary analysis of the "Tranexamic Acid (TXA) in Traumatic Brain Injury" prehospital database (2015-2017). We studied patients with admission endothelial biomarkers: i

Use of Spinosum Roentgen Index (S.R.I.) to determine candidacy for middle meningeal artery embolization.

INTRODUCTION: Middle meningeal artery embolization (MMAe) is a minimally-invasive approach for the treatment of subdural hematomas (SDH). Small vessel caliber and ectopic origin of the middle meningeal artery (MMA) may lead to unsuccessful embolization and/or serious morbidity. Thus, use of an objective scale to pre-procedurally assess candidacy for MMA embolization is an essential component of operative planning. METHODS: A retrospective analysis of all patients having undergone MMAe across 4 years at a single institution was performed. RESULTS: 169 MMAe procedures were performed for SDH, o

Palliative middle meningeal artery embolisation for symptomatic chronic subdural haematoma in an individual with resistant thrombocytopaenia due to acute myeloid leukaemia.

An older male presented with intermittent headaches, reduced mobility and transient facial weakness. CT head scan revealed acute-on-chronic subdural haematoma (cSDH), causing mass effect and midline shift. A background of progressive acute myeloid leukaemia was reported, characterised by severe thrombocytopaenia resistant to haematological therapies. The risk of neurosurgical intervention was deemed to be excessive.Right middle meningeal artery embolisation (MMAE) was pursued to arrest repeated subdural haemorrhage and prevent symptomatic neurological deterioration. The patient was discharged

The vascular neck restraint of mixed martial arts: understanding the pathophysiology, hemodynamics, and long-term consequences.

OBJECTIVES: B Mixed martial arts (MMA) brings various forms of martial arts disciplines together as a unified sport. Sportive 'choking,' also known as vascular neck compression, vascular neck restraint (VNR), or carotid restraints are utilized in MMA to apply jugular and carotid compression and subdue one's opponent. Few studies have explored the long-term consequences of VNRs, as well as the underlying pathophysiology and hemodynamics of the restraint. A narrative review was conducted in order to explore the long-term consequences of VNRs, as well as the underlying pathophysiology and hemodyn

Neutrophil extracellular traps induced by neoadjuvant chemotherapy of breast cancer promotes vascular endothelial damage.

BACKGROUND: The mechanisms underpinning neoadjuvant chemotherapy-induced vascular endothelial injury in breast cancer remain elusive. Our study aims to demonstrate that Neutrophil Extracellular Traps (NETs) play a pivotal role in neoadjuvant chemotherapy-induced vascular endothelial injury in breast cancer, elucidating that chemotherapy-induced upregulation of Solute Carrier 11a1 (Slc11a1) modulates Reactive Oxygen Species (ROS) generation, which may be critical for NETs formation. METHODS: We investigated the impact of neoadjuvant chemotherapy for breast cancer on NETs formation and vascular

Cellular and molecular mechanisms of pathological tau phosphorylation in traumatic brain injury: implications for chronic traumatic encephalopathy.

Tau protein plays a critical role in the physiological functioning of the central nervous system by providing structural integrity to the cytoskeletal architecture of neurons and glia through microtubule assembly and stabilization. Under certain pathological conditions, tau is aberrantly phosphorylated and aggregates into neurotoxic fibrillary tangles. The aggregation and cell-to-cell propagation of pathological tau leads to the progressive deterioration of the nervous system. The clinical entity of traumatic brain injury (TBI) ranges from mild to severe and can promote tau aggregation by indu

Adjunct middle meningeal artery embolization versus surgery alone for chronic subdural hematoma: A meta-analysis and trial sequential analysis of randomized controlled trials.

INTRODUCTION: Middle meningeal artery embolization (MMAE) has been studied as an adjunct to surgical evacuation for chronic subdural hematoma (cSDH). Previous meta-analyses comparing MMAE with conventional treatment for cSDH included both observational and randomized studies. To provide a more robust assessment, we performed a meta-analysis of randomized controlled trials to evaluate the outcomes of adjunct MMAE compared to surgical treatment alone. METHODS: We searched PubMed, EMBASE, and Cochrane CENTRAL databases for studies comparing adjunct middle meningeal artery embolization to surgica

Middle meningeal artery embolisation for chronic subdural haematoma - a UK single-centre experience.

INTRODUCTION: Chronic subdural haemorrhage (cSDH) is characterised by a pathological chronic collection of blood in the subdural space. Surgical evacuation is the conventional treatment of larger symptomatic cSDHs. Recurrence after conventional surgery remains an issue in up to 20%, and re-operation after surgical drainage is reported in 12%. Middle meningeal artery (MMA) embolisation selectively targets the distal MMA branches that give rise to the capillary feeders to the cSDH. This study aims to report the clinical and radiological outcomes of this procedure at a tertiary neuroscience centr
