A GLP-1 receptor agonist semaglutide attenuates cardiac microvascular injury in HFD/STZ-induced diabetic mice.

BACKGROUND: Cardiac microvascular injury from hyperlipidaemia and hyperglycaemia is associated with increased major adverse cardiovascular events (MACE). Semaglutide, a long-acting GLP-1 receptor agonist, reduces diabetic cardiovascular complications beyond its glycaemic and weight-lowering effects. However, the impact of semaglutide on diabetes-induced coronary microvascular injury and the integrated mechanisms involved remain unclear. METHODS: A combined streptozotocin (STZ) and high-fat diet (HFD) induced diabetes model was established in ApoE-/- mice, followed by 8 weeks of semaglutide tr

TBI-induced vessel softening increases brain susceptibility to injury with repeated head trauma.

Repeated traumatic brain injury (TBI) is a significant concern among military personnel, athletes, and abuse victims. However, little is known about the mechanisms that drive the brain's apparent increase in injury susceptibility with repeated loading. One critical factor may be the softening of cerebral blood vessels, which are significantly stiffer than brain tissue and influence its mechanical response during trauma. In this study, we employed a finite element model of a Göttingen minipig head to investigate how progressive vascular softening influences strain changes in brain tissue during

Surgery for Locally Advanced Pancreatic Cancer: Oncological Landmarks for Venous and Arterial Reconstruction.

BACKGROUND: According to the National Comprehensive Cancer Network (NCCN) guidelines, pancreatic cancer is classified as borderline resectable (BR-PC) on the basis of three criteria: (1) anatomical-the involvement of major vessels such as the celiac axis, hepatic artery (HA), superior mesenteric artery (SMA), or the mesenterico-portal venous axis; (2) biological-an elevated CA19-9 level exceeding 500 kU/L; and (3) conditional-patient unfitness for surgery due to general health status. The extensive involvement of major vessels is classified as locally advanced pancreatic cancer (LAPC). In such

Identifying Potential Carotid Blowout on Post-Treatment Neck Surveillance Imaging.

Carotid blowout is the rupture of a carotid artery or branch vessel, a dangerous complication in post-treatment head and neck cancer patients. The purpose of this retrospective case review is to identify imaging features suggestive of eminent carotid blowout on routine surveillance post-treatment neck imaging, so that a prompt neuroendovascular consult is obtained and the patient is preemptively treated, thus improving patient outcomes. It is important to carefully interrogate the carotid vessels with appropriate window/leveling for irregularity, adjacent tissue ulceration, exposed vessel to e

Vascular injuries of the head and neck in martial Arts: A scoping review.

BACKGROUND: Blunt cerebrovascular injury (BCVI) is one of the most feared complications of martial arts practice. Both striking and grappling disciplines place the cervical vasculature at risk either by way of direct blows or application of chokeholds, respectively. Research on BCVI in martial arts remains limited, primarily consisting of case reports. This study aimed to provide a scoping review of BCVI in martial arts. METHODS: Following the PRISMA guidelines, a systematic literature review was performed using PubMed and Ovid Embase databases from date of database inception to October 2024.

Arytenoid dislocation in a patient with Marfan syndrome after undergoing aortic arch surgery.

Postoperative vocal cord dysfunction can be caused by direct laryngeal trauma, nerve injury or can occur spontaneously. Patients with connective tissue disorders such as Marfan syndrome may be at higher risk for arytenoid dislocation due to tissue laxity. A woman in the 50s with Marfan syndrome underwent emergent total arch replacement for Type A aortic dissection. On endotracheal extubation, she was noted to have stridor. Fibreoptic examination was notable for an immobile right arytenoid. This case highlights the risk of arytenoid dislocation following endotracheal intubation in patients with

Fenofibrate attenuates the adverse effects of radiation on endothelial cells through modulation of ROS-NO signalling and inflammation.

Exposure to high doses of ionizing radiation has been consistently associated with the development of cardiovascular disease (CVD), particularly in patients undergoing thoracic radiotherapy. Radiation-induced CVD is characterized by alterations in metabolic and inflammatory homeostasis in the heart and vascular system. These changes promote myocardial and vascular pathological remodelling. Over time, such changes contribute to the development of atherosclerosis and heart failure. A key driver of these alterations is inactivation of the transcription factor peroxisome proliferator-activated rec

Are isolated linear fractures over major dural venous sinuses a risk factor for sinus thrombosis in mild TBI?

Cerebral venous sinus thrombosis (CVST) can increase intracranial pressure and cause secondary brain injury. There is uncertainty in the literature regarding the risk of traumatic CVST (tCVST) following fractures over a major sinus, the need and timing for venous imaging, and treatment. The current study aims to review our experience with tCVST when treating patients with isolated linear fractures over the major sinuses. Methods. This is a retrospective study of 95 adult patients, treated between January 2017 and March 2023, with mild, blunt, isolated traumatic head injury (GCS 13-15) and cran

Hospital setting of endovascular repair influences procedural outcomes in blunt traumatic aortic injury.

BACKGROUND: Thoracic endovascular aortic repair (TEVAR) has become the mainstay of treatment for blunt thoracic aortic injuries (BTAI) over open repair. Since the arrival of TEVAR, hybrid operating rooms have emerged as highly specialized environments equipped to streamline endovascular and open cases. Procedure characteristics and outcomes may vary when TEVAR is performed in the setting of a standard operating room with a portable C-Arm vs in a hybrid operating room or interventional radiology (IR) suite with a fixed imaging system. The purpose of this study was to compare clinical characteri

The Interplay of Angiogenesis and Osteogenesis in Non-Stabilized Incomplete Tibial Fractures: A Temporal Study in Rats.

Prior research on angiogenesis and osteogenesis during bone fracture healing has primarily focused on stabilized models, often within controlled environments. However, the dynamic interplay of these processes in the context of long bone fractures without scaffolds or external factors remains poorly understood. This study investigated the temporal dynamics of angiogenesis and osteogenesis in a non-stabilized incomplete transverse tibia bone fracture model. Multi-modal observation approach was carried out using micro-CT analysis of bone mineral density, histological assessment of fracture healin

The clinical application and anatomical analysis of proximal peroneal artery perforator in free fibula flap based on CTA.

BACKGROUND: The vascularized free fibular flap is increasingly recognized as the standard technique for the repair of complex soft tissue and hard tissue defects in oral and maxillofacial surgery. Conventionally, the vascular supply to the skin island is derived from the distal perforators of the peroneal artery. However, complications may arise if these distal perforators are either absent or damaged during surgical procedures, highlighting the necessity to employ the proximal peroneal perforators as an alternative. This study aims to investigate the anatomical characteristics-including the p

Catastrophic iatrogenic abdominal aorta haemorrhage during anterior resection.

A woman in her 70s with hypertension, breast cancer and diverticulosis underwent laparoscopic anterior resection for a tubule-villous adenoma, converted to open Hartmann's with aorto-bi-iliac bypass due to a vascular injury. Intraoperative complications included haem-o-lok penetration of the calcified aorta, necessitating vascular team intervention. Postoperative issues included bilateral popliteal artery emboli requiring embolectomy and fasciotomy, and a parastomal abscess. Despite significant blood loss and complications, the patient recovered well and remains cancer-free. This case undersco

Endovascular treatment of a traumatic aortic transection via implantation of self-expanding nitinol stent with open-cell design: a case report.

BACKGROUND: Traumatic aortic transection is a life-threatening injury with an acute mortality rate approaching 80%, typically resulting from high-energy blunt trauma, such as motor vehicle collisions. Without intervention, the overall mortality rate increases to 90-94%, primarily due to the rupture of pseudoaneurysms or expanding hematoma leading to exsanguination. Although delayed surgical repair may decrease immediate mortality, it is associated with significant perioperative risks. Endovascular techniques are becoming increasingly prevalent and have demonstrated lower morbidity and mortalit

The clinical effects and skin histological changes induced by a novel insulated radiofrequency microneedle: a pilot study.

Radiofrequency microneedle (RFMN) could mechanically penetrates the epidermis and emits radiofrequency energy to the target skin layer. This innovative system offers the capability to deliver precisely controlled radiofrequency energy at varying depths within the skin in a single insertion. We hypothesized that the new RFMN could improve both pore size and skin laxity simultaneously by single insertion and multiple discharges, thus reducing the number of treatment passes and improving the treatment efficiency. Therefore, we carried out this pilot study to confirm the clinical effects and corre
