Injectate distribution patterns in posterior infrazygomatic and transoral approaches to the pterygopalatine fossa.

BACKGROUND: Injectate distribution patterns in the pterygopalatine fossa may differ based on the drug administration approach used. This study primarily aimed to assess and compare injectate distribution following the posterior infrazygomatic and transoral approaches. The secondary aim was to evaluate the safety of both approaches. METHODS: Injectate distribution patterns were evaluated in 13 cadaveric head specimens. The vessels were perfused with a gelatin-based solution containing an iodinated contrast agent. The ultrasound-guided posterior infrazygomatic approach and transoral approach we

Postmortem diagnosis of fatal venous air embolism due to sigmoid sinus rupture associated with a basilar skull fracture.

BACKGROUND: Venous air embolism (VAE) secondary to basilar skull fractures is rare but potentially fatal. While VAE is a known complication in central venous catheter management, traumatic VAE-particularly in forensic contexts-remains underreported. In forensic medicine, few case reports have detailed the use of postmortem computed tomography (PMCT) to diagnose VAE. CASE PRESENTATION: A man in his 30 s sustained a head injury at a construction site when his head was caught between a 500-kg steel plate and the ground. Despite resuscitation attempts, spontaneous circulation was not restored, an

Development and validation of machine learning models to predict vertebral artery injury by C2 pedicle screws.

PURPOSE: Cervical 2 pedicle screw (C2PS) fixation is widely used in posterior cervical surgery but carries risks of vertebral artery injury (VAI), a rare yet severe complication. This study aimed to identify risk factors for VAI during C2PS placement and develop a machine learning (ML)-based predictive model to enhance preoperative risk assessment. METHODS: Clinical and radiological data from 280 patients undergoing head and neck CT angiography were retrospectively analyzed. Three-dimensional reconstructed images simulated C2PS placement, classifying patients into injury (n = 98) and non-inju

Cannulated Compression Screw Placement for Swine Femoral Neck Fracture to Protect the Main Intraosseous Arteries.

Femoral head necrosis is a common and serious complication of femoral neck fractures, primarily attributed to disrupted blood supply. This study introduces a novel method of placing cannulated compression screws in swine femoral neck fractures, designed to protect the blood supply to the femoral head while ensuring mechanical stability. Using microscopic perfusion techniques and 3D technology (via Materialise 3-Matic11.0 and Materialise Magics 21.0 software), we created perfusion and corrosion casting specimens of the intraosseous arteries from 21 fresh swine proximal femurs. The spatial distr

Targeted transarterial embolization of the femoral head: development of a minimally invasive approach to model Legg-Calvé-Perthes disease in piglets.

Clinical management of children with Legg-Calvé-Perthes Disease (LCPD) is hampered by incomplete understanding of how the extent of ischemic injury and the duration and quality of subsequent repair determine patient outcome. The traditional piglet model of LCPD is limited to capturing global femoral head ischemia; thus, a new model is needed in which the extent of ischemia can be varied to replicate the spectrum of disease seen in children. In this exploratory study, we used an iterative approach to test and refine methods to bilaterally occlude vessels supplying the femoral heads in n = 8 you

Lateral Proximal Femoral Epiphyseal Vessel Avulsion in Slipped Capital Femoral Epiphysis: A Newly Identified Mechanism of Avascularity: A Case Report.

CASE: We report 2 cases of Loder-unstable slipped capital femoral epiphysis (SCFE). Both patients had initial pinning followed by surgical hip dislocation where complete avulsion of the lateral epiphyseal vessels from the nutrient foramina was identified. Both patients underwent epiphysiodesis to revascularize the femoral head but developed osteonecrosis with collapse 9 months after their injury. CONCLUSION: Avulsion of the lateral epiphyseal vessels currently represents a nonreversible mechanism of avascularity in SCFE. Although Loder previously theorized the existence of this mechanism, it

Intraoperative patterns and causes of superior retinacular vessel damage in safe surgical hip dislocation for hip preservation.

BACKGROUND: Safe surgical dislocation has revolutionised hip preservation surgery by allowing complete access to the proximal femur while minimising the risk of avascular necrosis. However, despite meticulous technique, intraoperative injury to the superior retinacular vessels (SRV), the primary blood supply to the femoral head, can still occur, potentially compromising outcomes. PURPOSE: To identify, classify, and analyse intraoperative patterns and mechanisms of SRV injury during surgical hip dislocation procedures performed for complex hip deformities. METHODS: A prospective analysis of a

Enhancing foundational validity of forensic findings in nonlethal medico-legal strangulation examinations.

BACKGROUND: Nonlethal strangulation occurs when the brain is deprived of oxygen because of external compression leading to the occlusion of the neck's blood vessels and/or airway. The current state of strangulation science confines expert testimony to merely describing injuries attributed to strangulation 'based on the expert's experience and training.' Expert testimony that can quantify the likelihood that observed injuries are attributable to strangulation would strengthen the scientific validity of such testimony. PURPOSE: This study aims to identify those documented features, or clusters

Protecting the Nerve Coaptation: Connector-Assisted Nerve Repair in Complex Injuries.

PURPOSE: This study evaluated differences in outcomes of peripheral nerve repair using connector-assisted repair (CAR) or direct repair (DR) in an injured soft tissue bed. METHODS: The sciatic nerve of the right leg in 20 male Lewis rats was exposed and transected. We simulated a traumatized wound bed by cauterizing the underlying muscle bed with a bipolar coagulator. Nerves were repaired with either DR or CAR using porcine small intestine submucosa conduits. At 6 weeks, adhesions were assessed semiquantitiatively, and the gastrocnemius wet muscle weight of each hind limb was recorded to eval

Application of Special Cross-midline Blood Supply Internal Mammary Artery Perforator Flap for Facial Reconstruction.

Severe burn injuries may limit head and neck mobility while distorting facial features; they also place significant psychological strain on patients. The anterior thoracic area, similar in color and texture to the adjacent face-neck regions, often uses preexpanded flaps for reconstruction. Preexpanded internal mammary artery perforator (IMAP) flaps can further minimize donor area damage, provide a relatively thin flap for repairing large skin defects, and do not require an additional graft flap to close the donor site. In this article, we presented a case involving prolonged tissue expansion l

Repairment of an interrupted oculomotor nerve in the cavernous sinus.

Oculomotor nerve palsy following head trauma can cause eye movement disorders and severe visual problems, but it was considered to have a poor prognosis with a wide range of interventions in the past. However, we performed neuroanastomosis in a patient with isolated oculomotor nerve injury and obtained a satisfactory long-term recovery, although the short-term effect was not obvious. The clinical presentation, surgical management, and functional rehabilitation of this patient are discussed.

Dual-arterial occlusion of the celiac trunk and superior mesenteric artery for laparoscopic enucleation of deep-seated or broad-based pancreatic head tumors: a prospective, single-arm pilot study.

BACKGROUND: Enucleation has the advantages of preserving function and avoiding pancreaticoduodenectomy for benign and low-grade malignant neoplasms in the pancreatic head. However, laparoscopic enucleation (LEn) of pancreatic head tumors remains challenging in terms of bleeding control and duct integrity preservation because of the complicated blood supply to the pancreatic head and the adjacent relationships of lesions with the main pancreatic duct (MPD), especially for deep-seated or broad-based lesions. Here, we developed a novel dual-arterial occlusion technique to facilitate LEn of pancre
