How I do it? Microsurgical treatment of traumatic anterior cerebral artery pseudoaneurysm with cerebrospinal fluid rhinorrhea.

BACKGROUND: Despite the increasing reports of successful treatments for traumatic intracranial aneurysms, their characterization and the impact of concurrent head injuries on interventions aren't well explored. METHODS: A traumatic anterior cerebral artery pseudoaneurysm with CSF leakage was successfully treated. The aneurysm was treated microsurgically, while the CSF leakage was managed through dural repair. CONCLUSION: Traumatic intracranial aneurysms may have strong aneurysm walls in the subacute stage, making microsurgical repair feasible. Concomitant skull base fractures and CSF rhinorr

Manipulation of the Lower Cranial Nerves and Superior Jugular Bulb During Subtotal Temporal Bone Resection for Advanced Temporal Bone Malignancies: Anatomical Considerations and Surgical Strategies.

INTRODUCTION: En bloc subtotal temporal bone resection (STBR) is challenging. En bloc resection aiming for negative margins is considered the most reliable treatment strategy for advanced squamous cell carcinoma of the external auditory canal. However, the literature indicates that STBR carries a high perioperative risk. This report retrospectively examines 5 cases of en bloc STBR, considering the manipulating of the lower cranial nerves and the superior jugular bulb (SJB) from a microsurgical anatomical perspective. METHODS: Retrospective observational study. This study includes 5 cases of S

Prevalence of Venous Sinus Stenosis in Spontaneous Cerebrospinal Fluid Leak and the Role for Venous Sinus Stenting: A Systematic Review and Meta-Analysis.

BACKGROUND: Given the association between spontaneous cerebrospinal fluid (sCSF) leak and idiopathic intracranial hypertension (IIH), and the association of IIH and venous sinus stenosis (VSS), we sought to determine the prevalence of VSS in sCSF leak and the role of venous sinus stenting for sCSF leak. METHODS: A protocol was registered in PROSPERO [CRD42024568270]. A search was conducted in four electronic databases: Medline (Ovid), Embase, CINAHL, Web of Science, and Cochrane CENTRAL. Two reviewers independently screened citations and extracted data. Methodological quality was assessed usi

A Case of a Huge Left Common Carotid Artery Paraganglioma Resected With the Left Thyroid Gland.

BACKGROUND: Paraganglioma (PGL) is a rare neuroendocrine tumor. In the head and neck region, surgical resection of PGL is extremely difficult due to its proximity to many vital blood vessels, nerves, and organs. There is still some controversy about whether preoperative embolization is salutary. METHODS: We present a case of a giant left common carotid artery PGL with nodular goiter, which needs surgical resection combined with interventional and endovascular treatments. RESULTS: The patient recovered well after the operation without vascular and nerve injury complications. His chest tightne

Overcoming the Challenges of Uncinate Process Oncologic Management: The Left-Side Approach in Minimally Invasive Pancreatoduodenectomy: Step-by-Step Technique and Video.

BACKGROUND: The uncinate process (UP) represents one of the greatest challenges in laparoscopic pancreatoduodenectomy (LPD) due to its deep anatomic location and proximity to major vascular structures. Ensuring complete resection of the UP and mesopancreas is crucial for achieving negative surgical margins and adequate lymphadenectomy with tumors of this region. The standard approach from the right-side of the superior mesenteric artery (SMA) often requires significant tension on mesenteric vessels, increasing the risk of vascular injury. This video describes a step-by-step, left-side approach

Bullet fragment embolisation to posterior cerebral artery from a gunshot wound to the left shoulder.

A male patient in his late 40s presented after a single high-velocity gunshot injury to the left shoulder. He was haemodynamically unstable on arrival with no evidence of neurological deficit. CT of the thorax and abdomen showed multiple bullet fragments throughout the posterior left thorax with evidence of an acute aortic injury. Following resuscitation, the patient stabilised, and his aortic injury was managed conservatively. Two days postinjury, postextubation, the patient reported a homonymous hemianopia. CT brain and angiography showed a subacute left posterior cerebral artery (PCA) terri

Styloidectomy for Jugular Decompression in Venous Outflow Disorders.

BACKGROUND: Jugular venous stenosis at the level of C1 and the styloid process is increasingly implicated in cerebral venous outflow disorders, often presenting with headache, pulsatile tinnitus, and cognitive complaints. Although small studies suggest benefit from open decompression via styloidectomy, larger series are needed to clarify outcomes and complication profiles. METHODS: A single-center, retrospective review was performed evaluating patients who underwent styloidectomy with or without partial C1 resection for symptomatic jugular stenosis between 2022 and 2024. Inclusion criteria in

Pseudoaneurysm from an aberrant internal carotid artery injury during tympanoplasty repaired with a pipeline embolization device: A rare case in a pediatric patient.

BACKGROUND: An aberrant internal carotid artery (ICA) is a rare anomaly that can be subject to iatrogenic injury during middle ear surgery. The management of this potentially fatal complication is not well-described in the pediatric otolaryngology literature. Herein, we describe our unique management of an extremely rare case of an aberrant ICA injury during a tympanoplasty procedure that was further complicated with the formation of a pseudoaneurysm. CASE PRESENTATION: A 13-year-old female patient with a longstanding history of recurrent ear infections and conductive hearing loss underwent t

Fish bone migration: complications, diagnostic challenges, and treatment strategies.

BACKGROUND: Fish bone impaction in the pharynx is a common otolaryngological emergency. However, if the fish bone perforates the pharyngeal wall or the gastrointestinal wall and migrates to the neck tissues or organs, entering the lungs, mediastinum, heart, liver, biliary tract, spleen, pancreas, or other structures, or damages major blood vessels in the thoracic or abdominal cavities, it can lead to severe complications. This condition is rare and dangerous, potentially resulting in a series of serious complications, including neck abscess, thyroid abscess, thrombosis or air embolism of the c
