Neuromuscular signatures differentiating functional and impaired cases in full-thickness rotator cuff tears.

BACKGROUND: The relationship between structural damage and functional impairment in patients with full-thickness rotator cuff tears (FT-RCTs) remains inconsistent. While some patients maintain high function despite large tears, others experience profound disability, suggesting that compensatory neuromuscular mechanisms may be more closely associated with functional status than with anatomical integrity. This study aims to identify the clinical, structural, and neuromuscular factors that differentiate high-functioning from poor-functioning individuals with FT-RCTs. METHODS: Fifty-three patient

Isolated avulsion of the short head of the biceps tendon from the coracoid insertion: A case report and narrative review of the literature.

Isolated injuries to the short head of the biceps (SHB) are uncommon, and the existing literature mainly includes case reports documenting tears at the muscle belly or myotendinous junction. This case report presents the first reported SHB tendon injury at the coracoid insertion in a 34-year-old healthy male patient after trauma while playing with his daughter. The injury occurred with the arm forced into hyperextension, abduction, and external rotation, with the elbow fully extended. Diagnosis was confirmed by MRI, showing an isolated insertional tear of the SHB tendon with intact coracobrach

Combined anterior latissimus dorsi and teres major transfer and superior capsular reconstruction using autogenous biceps tendon for anterosuperior irreparable rotator cuff tears: technical note.

This technical note describes the surgical technique of combining anterior latissimus dorsi and teres major (LDTM) tendon transfer with biceps superior capsular reconstruction (SCR) for the treatment of anterosuperior irreparable rotator cuff tears (AIRCTs). Anterosuperior irreparable rotator cuff tears, characterized by irreparable tears in the subscapularis and supraspinatus tendons, can lead to significant shoulder dysfunction. The described procedure aims to restore glenohumeral stability, prevent humeral head migration, and optimize shoulder mechanics. The LDTM tendon transfer replicates

Comparison of donkey and stallion semen preservation: effects of egg yolk plasma, seminal plasma and extenders on sperm motility, MALDI profiles and fertility.

ABSTRACT: The conservation of donkey breeds at risk of extinction requires the development and optimisation of semen preservation and artificial insemination strategies aimed at improving reproductive efficiency, increasing genetic diversity and supporting sustainable population management. The effects of commercial extenders (INRA96, BotuSemen Gold, Beyond), seminal plasma (SP) removal and egg yolk plasma (EYP; 0-10%) supplementation on jack semen stored at 4°C for 24-48 h were evaluated, using stallion semen as a reference. Sperm motility was assessed by computer-assisted sperm analysis (CAS

Patient Valuation of Visit Types, Speed of Care, and Continuity With Primary Care Physicians: A Discrete-Choice Survey.

INTRODUCTION: Since the COVID-19 pandemic, there has been rapid uptake of portal messaging by patients to address acute and chronic health conditions. Demand for in-person appointments has remained high. It is not known how US patients value trade-offs between rapid portal care, a shorter wait for a visit with any available physician, or a longer wait to see their primary care physician. We used a discrete-choice experiment to elicit patient preferences for different types of care delivery and clinician type, varying time to appointment. METHODS: In 2023 our academic family medicine departmen

Posterior sentinel sign of the biceps tendon as an arthroscopic indicator of supraspinatus tears.

INTRODUCTION: Magnetic resonance imaging (MRI) is highly accurate for diagnosing full-thickness rotator cuff tears but inaccurate in detecting small anterosuperior cuff tears near the rotator interval. Expanding on the concepts of long head of biceps tendon (LHBT) instability and the anterior sentinel sign, the present study evaluated the posterior sentinel sign, a novel arthroscopic indicator of supraspinatus tears. METHODS: This monocentric prospective cohort study included patients undergoing arthroscopic repair of rotator cuff tears at a single institution. The integrity and stability of

A novel arthroscopic suturing technique for rotator cuff tears: A retrospective clinical study.

To overcome the drawbacks of these conventional suturing methods, we have developed a novel suture technique. This new technique combines the advantages of the traditional Mason-Allen technique and the suture bridge technique, which enhances the tendon-to-bone contact area and prevents suture cutout, among other benefits. This study is to investigate the method, technique, and clinical efficacy of this novel suture technique in the treatment of rotator cuff injuries. Between January and December 2023, 21 patients (9 male, 12 female; age range: 46-71 years, mean: 51) underwent rotator cuff repai

Design of a rigid subacromial implant to treat massive irreparable rotator cuff tears.

BACKGROUND: Massive rotator cuff tears often lead to superior migration of the humeral head and abnormal shoulder kinematics and function. In an attempt to restore normal joint kinematics, a modular subacromial implant was developed and evaluated in this biomechanical cadaveric study. METHODS: Eight cadaveric specimens were assessed using a muscle-loading shoulder simulator that applied loads at 0, 30, and 60 degrees of glenohumeral abduction. Testing states included the intact rotator cuff (native), the simulation of a massive rotator cuff tear, and multiple implant states. Two implant thick

Compensatory Load Sharing by Residual Rotator Cuff Subregions Preserves Glenohumeral Mechanics in Partial and Massive Tears.

BACKGROUND: Rotator cuff (RC) tears are common shoulder injuries that cause pain, dysfunction, and abnormal humeral head translation. Balanced force couples in the transverse and coronal planes help to maintain normal glenohumeral mechanics. Although clinical and biomechanical studies have suggested that compensatory activation of residual RC muscles preserves function, the contribution of individual RC subregions to glenohumeral contact mechanics and humeral head translations across progressively increasing tear sizes remains unclear. METHODS: Eight fresh-frozen male cadaveric shoulders (mea

Knotless Independent Double-Row Repair and Biceps Augmentation for Anterosuperior Rotator Cuff Tears.

Rotator cuff tears are frequent, mainly involving the anterosuperior shoulder tissue, such as the supraspinatus, subscapularis, and the long head of the biceps tendon (LHBT). The double-row repair was designed to provide better footprint coverage and a lower retear rate than a single-row repair for supraspinatus tears. Traditionally, a double-row repair needs two medial-row suture anchors and two lateral-row suture anchors to provide a wide compression area. However, the medical expenses associated with this technique are quite high. We developed a modified knotless independent double-row repa

Complete Proximal Rectus Femoris Avulsion Repair in a Division I Collegiate Track Athlete: A Case Report.

CASE: A 20-year-old track athlete experienced a pop in her left hip with immediate pain and weakness during a race. She had a palpable defect distal to the anterior inferior iliac spine (AIIS). Magnetic resonance imaging demonstrated a complete tear of the rectus femoris direct head tendon from the AIIS, with 3 cm of retraction. Given her age and activity level, she underwent successful proximal rectus femoris avulsion repair and experienced a full return to sport. CONCLUSION: For athletes with an isolated avulsion of the direct head of the rectus femoris and significant tendon retraction, su

Injuries to accessory head of rectus femoris in professional football players: anatomical description, imaging findings, and clinical implications.

BACKGROUND: The quadriceps femoris muscle compartment comprises four muscles, including the bi-articular rectus femoris, which is particularly prone to strain in kicking athletes. Anatomical variations such as accessory heads occur frequently but are seldom identified clinically. This retrospective case series describes injuries involving a previously undescribed accessory rectus femoris head in professional footballers. MATERIALS AND METHODS: We retrospectively reviewed a series of cases in professional footballers from 2022-2024 with acute anterior thigh injuries. Inclusion criteria require

Knee and hip joint dynamics differ between sprinting and Nordic hamstring exercises.

Sprinting and the Nordic hamstring exercise (NHE) are common training modalities used to reduce hamstring injury risk, but the differences in the biomechanical demands of sprinting and the NHE are unclear. We conducted an exploratory analysis comparing knee and hip joint kinematics and kinetics, and biceps femoris long head (BFLH) muscle-tendon unit length and velocity during the flight phase of sprinting and the NHE. We collected motion capture and force data from fourteen athletic participants (8 males, 6 females) running (4-8 m/s) and performing the NHE. We used this experimental data and m

Adaptations in biceps femoris long-head muscle-tendon mechanics during the Nordic hamstring exercise in response to 9 weeks of training.

BACKGROUND: Prolonged Nordic hamstring exercise (NHE) training reduces hamstring injury risk, but the underlying mechanisms remain unclear. This study examined how NHE training alters biceps femoris long-head (BFlh) muscle-tendon unit (MTU) mechanics during the exercise, focusing on how muscle adaptations influence fascicle and sarcomere lengths during the NHE after training. METHODS: Twelve recreationally active participants completed 9 weeks of supervised NHE training and 3 weeks of detraining. Knee flexor torque was measured using ankle-mounted load cells and estimated from the knee joint

Modified Long Head of Biceps Tendon Rerouting and Fixation as Partial Capsular Reconstruction for Massive Irreparable Rotator Cuff Tears.

Massive irreparable rotator cuff tears (MIRCTs) present a particularly complex and difficult challenge for the orthopedic surgeon. Fatty infiltration of muscles, chronic excessive retraction of tendons, and overall tissue degeneration predispose these cases to high failure rates following surgical treatment. Several approaches have been introduced for transferring the long head of the biceps tendon (LHBT) to help stabilize the humeral head and reestablish shoulder muscle strength. This article proposes a technical modification of capsular reconstruction using arthroscopic LHBT rerouting and fi

Deltoid Osteomuscular Transfer Suppresses Humeral Head Translation Caused by Massive Irreparable Rotator Cuff Tears: A Cadaveric Biomechanical Study.

BACKGROUND: The optimal treatment for massive irreparable rotator cuff tears (MRCTs) remains unclear. The deltoid flap transfer has been described in the literature as an option for MRCT; however, it has a high retear rate and therefore its effectiveness is limited. PURPOSE: To describe a modification of the deltoid flap transfer, termed the deltoid osteomuscular transfer (DOT), and assess its biomechanical effectiveness in the management of irreparable MRCT in a cadaveric model. STUDY DESIGN: Controlled laboratory study. METHODS: Eight fresh-frozen cadaveric shoulders were tested. Segmenta
