Velocity-Dependent Stress Changes in the Hamstring Muscle-Tendon Complex: A Study Using Human Cadavers.

Hamstring strain injuries frequently occur during high-speed running, when muscle tension is markedly increased. Given the viscoelastic properties of muscle, stress during elongation is expected to be velocity-dependent. Therefore, this study investigated the effect of elongation velocity on the stress in individual hamstring muscles. Seven Thiel-embalmed cadaveric lower limbs were used to isolate the biceps femoris long head, semimembranosus, and semitendinosus muscles. Specimens were fixed to a testing apparatus and passively elongated to 10% strain at five elongation velocities (20-300 mm/m

Bilateral Fusion of the Semitendinosus and Long Head of the Biceps Femoris Muscles in a Male Cadaver.

Bilateral variation of the semitendinosus (ST) and the long head of the biceps femoris (BFlh) muscles is an anatomically rare occurrence. In this case report, we describe a male cadaver exhibiting bilateral fusion of the ST and BFlh muscles at their proximal origin on the ischial tuberosity, with the muscles separating distally into their respective muscle bellies. As far as we are aware, this bilateral fusion has not been previously documented in the literature; the only known report of fusion between the ST and BFlh muscles describes a unilateral fusion in a female cadaver, which was associa

Combined subscapularis and supraspinatus tears markedly increase the risk of long head of the biceps tendon pathology.

OBJECTIVE: This study aimed to examine the frequency of long head of the biceps tendon (LHBT) pathologies in anterosuperior rotator cuff tears (RCT) involving the LHBT pulley system and investigate the relationship between RCT types and LHBT pathology subtypes. METHODS: A retrospective review was conducted of 555 shoulder arthroscopies performed for rotator cuff tears between 2017 and 2023. Patients were classified into 3 groups: isolated subscapularis (SSC) tears (n=74), isolated supraspinatus (SSP) tears (n=177), and combined SSC+SSP tears (n=304). The severity of the subscapularis tear was

Active shoulder external rotation recovery in upper brachial plexus birth injury following nerve surgery: is spinal accessory nerve-suprascapular nerve transfer superior to C5-suprascapular nerve grafting?

OBJECTIVE: A key residual deficit in brachial plexus birth injury (BPBI) is the lack of active glenohumeral external rotation (ER). Surgical options for reinnervating the suprascapular nerve (SSN) include nerve grafting from a proximal stump and nerve transfer using the spinal accessory nerve (XIN) as a donor. The optimal surgical strategy is unclear from the current literature. This study aimed to compare the outcomes of nerve grafting and nerve transfer for SSN reinnervation in upper trunk BPBI. METHODS: The authors conducted a retrospective study (1990-2020) on prospectively collected data

Comparison of subacromial volume and clinical parameters of patients who underwent biceps tenotomy or tenodesis due to long head of biceps tendon pathologies during rotator cuff repair surgery.

The aim of this study was to compare the results of patients who underwent biceps tenotomy or tenodesis alongside rotator cuff rupture repair (RCRR), as well as to assess its relationship with alterations in subacromial volume. The hypothesis was that patients undergoing biceps tenodesis would experience greater improvements in outcome scores 2 years after the surgery and larger subacromial volumes as the long head of the biceps tendon maintains its tension.  Methods: This is a retrospective study including patients who underwent an RCRR concomitant with a biceps procedure (88 tenotomy and 63

Platelet-Rich Plasma Versus Polydeoxyribonucleotide for Myofascial Pain Syndrome and Musculoskeletal Disorders: A Narrative Review of Mechanisms, Clinical Evidence, and Translational Considerations.

Myofascial pain syndrome (MPS) of the cervicoscapular girdle-most typically involving the upper trapezius and the rhomboid muscles-is among the most frequent causes of chronic regional musculoskeletal pain, yet the mainstays of interventional treatment (dry needling, local anesthetic trigger point injection, corticosteroid, botulinum toxin) act principally on nociceptive transmission or on the contractile taut band rather than on the disordered tissue biology that sustains it. Two injectable biologics with fundamentally different design logic have been proposed to fill this gap. Platelet-rich

CXCL13-neutralizing Antibody Alleviate Chronic Skeletal Muscle Degeneration in a Mouse Model.

INTRODUCTION: Skeletal muscle degeneration is a common effect of chronic muscle injuries, including fibrosis and fatty infiltration, which is the replacement of preexisting parenchymal tissue by extracellular matrix proteins and abnormal invasive growth of fibroblasts and adipocytes. METHOD: This remodeling limits muscle function and strength, eventually leading to reduced quality of life for those affected. Chemokines play a major role in the regulation of immunocyte migration, inflammation, and tissue remodeling and are implicated in various fibrotic and degenerative diseases. In this study

Editorial Commentary: Partial Rotator Cuff Repair and Deltoid Retraining Shows Positive Outcomes for Large to Massive Rotator Cuff Tear.

In terms of rotator cuff repair, there is a goal for complete repair and healing, as rotator cuff integrity correlates with clinical and functional results. Retear has been shown to have a significant influence on progression toward osteoarthritis, and patients with an intact supraspinatus show superior abduction and flexion strength. However, in cases where complete repair may not be possible and/or cost limitations may prohibit augmentation, partial repair can provide a respectable outcome. Furthermore, regardless of healing status, partial repair may provide satisfactory relief for patients

Effects of adipose allograft matrix on viability of humeral head cartilage and rotator cuff tendon.

BACKGROUND: Rotator cuff repairs may fail because of compromised blood supply, suture anchor pullout, or poor fixation to bone. To augment the repairs and promote healing of the tears, orthobiologics, such a platelet-rich plasma (PRP), and biologic scaffolds have been applied with mixed results. Adipose allograft matrix (AAM), which recruits native cells to damaged tissues, may also be a potential treatment for rotator cuff tears. METHODS: To assess the potential use of AAM on rotator cuff tears, humeral head cartilage and subscapularis tendon were collected from patients undergoing reverse s

Associations between injury occurrence and environmental temperatures in the Australian and German professional football leagues.

A cross-sectional analysis was performed to investigate associations between environmental temperatures and injury occurrence in two professional male football (soccer) leagues. Data from seven seasons of the German Bundesliga (2142 matches) and four seasons of the Australian A-League (470 matches) were included. Injuries were collated via media reports for the Bundesliga and via team staff reports in the A-League and comprised injury incidence, mechanisms (contact, noncontact), locations (e.g., ankle, knee, and thigh), and types (e.g., muscle and tendon, joint and ligament). Weather data incl

Editorial Commentary: Outcomes of Shoulder Biceps Tenotomy Versus Tenodesis Are Difficult to Determine From Nonrandomized Studies Due to Selection Bias: Tenodesis Is More Commonly Performed on Younger Males.

The long head of the biceps tendon is a common source of anterior shoulder pain, frequently addressed during rotator cuff repairs. Surgical management typically involves either tenotomy or tenodesis, each offering distinct advantages and disadvantages. Tenotomy, a straightforward procedure with a short rehabilitation period, is well suited for low-demand patients but carries a higher risk of Popeye deformity and cramping pain. In contrast, tenodesis, which reattaches the long head of the biceps tendon, minimizes these risks and is preferred for younger, active patients. Randomized controlled t

Relationship between humeral head translation, spur formations, and the locations of tendon tears in massive rotator cuff tears.

OBJECTIVE: This study aimed to clarify the relationship between the directions of humeral head translation, the presence of acromial or coracoid spurs, and the locations of tendon tears in massive rotator cuff tears. Thirty shoulders from thirty patients with massive rotator cuff tears who underwent reverse shoulder arthroplasty were included. Preoperative 3DCT classified humeral head translation into three groups: minimal type, posterosuperior type, and anterosuperior type. The presence of acromial or coracoid spurs was also assessed. Preoperative MRI and intraoperative findings determined th

A combined anterior and posterior cuff transfer: a novel technique for massive irreparable rotator cuff tears.

INTRODUCTION: Irreparable rotator cuff tears (IRCTs) are large tears that can't be surgically repaired due to poor tissue quality, degeneration, or severe tendon retraction. These tears often involve multiple tendons and lead to fatty infiltration, humeral head migration, and tendon retraction. Patients with IRCTs typically present with pseudoparalysis, muscle atrophy, or anterosuperior escape. While various surgical options exist, outcomes are often inconsistent. This case series presents a novel technique for managing massive IRCTs, showing excellent, consistent results and offering a promis

Which is more important for achieving superior stability in a massive rotator cuff tear? The spacer effect (static) vs. concavity compression (dynamic): a biomechanical cadaveric study.

BACKGROUND: This study aimed to investigate the biomechanical effect of subacromial spacer (physically depressing the humeral head) and magnets (concavity compression restoration) in massive rotator cuff tear (MRCT). We assessed the influence on superior stability by employing the subacromial spacer (the spacer effect) and magnets (concavity compression effect) separately or in combination in MRCT. METHODS: A customized shoulder testing system tested 7 fresh-frozen cadaveric shoulders. The prosthesis implantation was performed prior to all experimental conditions. Each specimen underwent the
