Regional differences in amplitude and spatial homogeneity of muscle activity in the biceps femoris long head.

PURPOSE: Hamstring injuries, particularly in the proximal Biceps femoris long head (BFlh), remain frequent in sports involving sprints and accelerations despite extensive research. Non-uniform muscle activity may contribute to these injuries by causing uneven load distribution. This study examines spatial homogeneity of muscle activity and amplitude in the proximal and distal BFlh at different knee flexion torque levels and muscle-tendon unit (MTU) lengths under controlled isometric conditions. METHODS: Fifteen male recreational athletes performed unilateral isometric knee flexion contraction

Augmented primary repair of chronic partial complex triceps brachii tear in a kabaddi player - a case report.

UNLABELLED: Injury to the triceps brachii tendon is relatively rare (<1 % of all tendon ruptures). Partial tears of triceps are seen in indirect injury mechanisms usually involving the superficial part of the muscle with intact deep medial head, which may require operative intervention in high-demand individuals. CASE: A 36-year-old right-handed athlete with chronic complex triceps tear with a history of anabolic steroid use managed operatively with augmented primary repair using fibre-wire with excellent outcome at 2-year follow-up. We describe a novel technique of using a transosseous fibre

Dermal Allograft and Biceps Superior Capsule Reconstruction for Massive Irreparable Rotator Cuff Tears.

Since the use of autologous fascia lata for superior capsule reconstruction of massive irreparable rotator cuff tears (MIRCTs), the technique has evolved into various modifications, including dermal allografts, long head of the biceps tendon (LHBT), and combinations of both, which will be discussed in this article. After making sure the remnant cuff cannot be restored to the anatomical footprint of the supraspinatus, a double-loaded or triple-loaded, suture-based anchor is inserted 5-8 mm posterior to the bicipital groove to secure the long head of the biceps (LHBT) initially. A bone trough is

[Focus on the shoulder : Precise diagnostics for complex pathological alterations].

The shoulder joint combines three main joints and two auxiliary joints, which enable a unique freedom of movement. Stability is primarily ensured by muscles and ligaments, making injuries, overuse or degenerative processes common causes of complaints. A systematic examination includes the anamnesis, inspection, palpation, range of motion assessment and functional tests. During the inspection the pattern of movement, muscle atrophy and malalignments are assessed. Bone and soft tissue structures are tested by palpation. Functional tests help to differentiate pathological alterations. A structure

The choice of bearings in revision total hip arthroplasty : rationale, algorithm, and outcome.

AIMS: The aim of this study was to determine the success of an algorithm designed to guide the choice of bearing based on gluteus medius deficiency to be used in revision total hip arthroplasty (THA). METHODS: Dislocation following revision THA remains a leading cause of failure, and while bearings which offer enhanced stability are available, the indications for their use remain unclear. The integrity of the abductor muscles is a major contributor to stability. We describe the use of an algorithm based on gluteus medius deficiency to determine the choice of bearing in revision THA. The defau

Improved clinical outcomes after arthroscopic partial rotator cuff repair with rotator interval shift for the treatment of massive irreparable anterior L-shaped rotator cuff tears.

PURPOSE: The treatment of massive anterior L-shaped rotator cuff tears (RCTs) remains challenging. This study focused on restoration of humeral head coverage by superiorly repositioning the rotator interval tissue. The study aimed (1) to present the short-term clinical and radiological outcomes of arthroscopic partial repair combined with rotator interval shift and (2) to identify risk factors associated with failure in restoring superior humeral head coverage. METHOD: A retrospective review was conducted on arthroscopic rotator cuff repairs performed by a single surgeon between January 2018

Effect of humeral rotation on rotator cuff strain, loading and kinematics: an in vitro study.

BACKGROUND: Despite its main function as abductor, the role of the supraspinatus as stabilizer and rotator cannot be neglected. A supraspinatus tear may not only influence humeral head rotation during abduction but also the strength and loading of the acting (intact) rotator cuff muscles. The purpose of this study was to investigate the effect of constrained humeral rotation and elevation on rotator cuff loading, strain and kinematics with intact and torn cuff conditions. METHODS: Active humeral elevation until 30° was simulated in twelve fresh-frozen cadaver shoulders with free humeral rotat

Fully Arthroscopic Anterior Latissimus Dorsi Transfer for Irreparable Subscapularis Tears.

Latissimus dorsi (LD) transfer can be used for the treatment of irreparable subscapularis tears. This article describes a fully arthroscopic LD transfer procedure to manage irreparable subscapularis tears. The LD tendon and muscle are dissected and released arthroscopically, and care must be taken to avoid injury to the axillary and radial nerves. With the use of a portal superior to the upper margin of the pectoralis major (PM) and a portal inferior to the lower margin of the PM, detachment of the proximal PM tendon is not necessary for access to the insertion of the LD tendon. The posterior

Arthroscopic Superior Capsule Reconstruction for Irreparable Posterosuperior Rotator Cuff Tears with Autologous Fascia Lata Graft.

This study explores the efficacy of superior capsule reconstruction (SCR) using fascia lata autograft for treating irreparable rotator cuff tears. Based on our experience and existing literature, SCR has demonstrated promising outcomes, offering improved shoulder stability, reduced pain, and prevention of humeral head migration. Patients undergoing SCR with fascia lata autograft achieved near-normal shoulder range of motion postoperatively, with maintained acromio-humeral distance observed in radiographic follow-ups. Various graft modifications, including the use of dermal grafts and the long

Ipsilateral Supernumerary Heads of the Sternocleidomastoid and Flat Fibers of the Omohyoid Muscle: A Cadaveric Case Report.

This case report presents an ipsilateral anatomical variation in the right neck of an 80-year-old male cadaver. The Sternocleidomastoid muscle exhibited four heads, one sternal and three clavicular heads. All the heads are merging into a common belly and inserting normally into the mastoid process. The right omohyoid muscle appeared as flat fibers, lacking an intermediate tendon and having its superior belly fused with the sternohyoid muscle. Abnormal development of the embryonic branchial arch mesoderm and occipital myotome leads to these variations. The Sternocleidomastoid and omohyoid are c

In vivo characterization of Achilles subtendon function and morphology within the tendon cross section and along the free tendon.

The Achilles tendon is composed of three distinct fascicle bundles, or "subtendons," each originating from the head of one of the three triceps surae muscles. In a healthy tendon, these subtendons slide relative to each other during muscle contractions. This subtendon sliding is reduced in older adults and younger adults who suffer an Achilles tendon injury. However, subtendon sliding is challenging to quantify in low-load scenarios that are critical for monitoring subtendon biomechanics in patients with mechanically compromised tendons, like following an Achilles tendon rupture and repair sur

Musculoskeletal architecture of the shoulder: A comparative anatomy study in bats and mice informing human rotator cuff function.

Overhead motion in humans often leads to shoulder injuries, a consequence of the evolutionary trade-off in glenohumeral joint anatomy that balances stability with mobility. Bats consistently engage in overhead motion during flight, subjecting their shoulders to substantial loading throughout their relatively long lifespan. Remarkably, despite the demands placed on a bat's shoulder, instability and rotator cuff tears, which could be fatal to bats in short order, are not observed in nature. We were thus inspired to study functional adaptations in the shoulders of bats that enable this overhead m

A novel bilateral distal variation of the biceps brachii.

The biceps brachii muscle is known for its anatomical variability, particularly regarding the presence of additional heads, most commonly arising proximally as third, fourth, or even fifth heads. Distal variations, however, are rarely described in the literature. In this case report, we describe a rare bilateral finding of an accessory muscle belly originating from the short head of the biceps brachii. This accessory belly gives rise to a distinct tendon and inserts distally to the main biceps tendon. Recognition of this unusual anatomical arrangement in the cubital fossa is crucial to avoid m

Morphologic and morphometric study of the long head of the biceps femoris in male cadavers: tendon and MTJ insights for injury diagnosis.

The long head of the biceps femoris (BFlh) is among the most frequently injured components of the hamstring complex, particularly at the myotendinous junctions (MTJs). Despite its clinical relevance, the gross morphology of the BFlh-including its tendon structure and anatomic variability-remains insufficiently characterized. This study aimed to provide a detailed anatomic and morphometric analysis of the BFlh, focusing on the proximal and distal tendons and their integration with surrounding muscle fibers. Thirty-five formalin-fixed male cadavers were dissected via a posterior longitudinal app
