BACKGROUND: Hamstring strain injuries (HSIs) have a high rate of recurrence, highlighting the need for effective tertiary prevention strategies. Flywheel resistance training appears effective in targeting muscular risk factors for HSI.
HYPOTHESIS: Flywheel leg curls will result in greater improvements in eccentric knee flexor strength and biceps femoris long head (BFLH) fascicle length compared with conventional leg curls.
STUDY DESIGN: Randomized controlled trial.
LEVEL OF EVIDENCE: Level 2.
METHODS: Twenty-six athletes with a history of HSI within the past 18 months participated in an 8-

BACKGROUND: Lower trapezius tendon (LT) transfer has shown promise in managing irreparable posterosuperior massive rotator cuff tears (PSMRCTs) with weakness of active external rotation (ER). However, the influence of teres minor (TM) trophicity on restoring ER strength has not been thoroughly investigated. This cadaveric biomechanical study evaluated the effect of atrophic, normotrophic, and hypertrophic TM on humeral head stability and ER recovery following LT transfer in PSMRCTs.
METHODS: Eight fresh-frozen cadaveric shoulders were tested under 5 conditions: (1) Intact rotator cuff, (2) PS

The long head of the biceps has traditionally been ignored, tenodesed, or tenotomized during shoulder arthroscopy. However, more recently, it has been recognized as robust autograft tissue that may be mobilized or harvested to aid in a range of augmentation and reconstructive procedures. Several applications are included in this infographic. An intertendinous augmentation (biologic superior capsular reconstruction) technique is indicated in massive tears that are either partially or completely reparable. The intact biceps tendon is mobilized from the groove, transposed and secured to the great

Hamstring injuries are a significant concern in high-speed running and kicking sports, contributing to a high incidence and recurrence rate among athletes. Anatomical and biomechanical properties of the hamstrings, especially the biceps femoris long head, make them susceptible to strain, contributing to the high injury rate observed in athletes. Key risk factors, including prior injury history, neuromuscular deficiencies, excessive load, and muscle-tendon architecture, have been identified as contributors to injury prevalence. Eccentric strengthening exercises, particularly the Nordic hamstrin

PURPOSE: Shoulder deformities and impaired function in individuals with birth brachial plexus injury (BBPI) are often caused by internal rotator (IR) contractures and abductor weakness, which may progress to glenohumeral dysplasia. Although subscapularis-complex release and tendon transfer is a time-tested procedure, there are controversies regarding the appropriate management of shoulder contractures, especially in preschool children. Therefore, this study examines the efficiency of arthroscopically done progressive controlled release of IR contracture combined with tendon transfer.

The Achilles tendon is composed of three distinct fascicle bundles, or subtendons, each originating from the head of one of the three triceps surae muscles. In a healthy tendon, these subtendons slide relative to each other during muscle contractions. This subtendon sliding is reduced in older adults and individuals who suffer from an Achilles tendon injury. However, subtendon sliding is challenging to quantify in low-load scenarios that are critical for monitoring subtendon biomechanics in patients with mechanically compromised tendons, such as following an Achilles tendon rupture and repair.

INTRODUCTION: Deficits in lower-limb muscle strength and altered gait mechanics are common after anterior cruciate ligament reconstruction (ACL). While isokinetic strength testing is widely accepted in return-to-sport assessment, the role of plantar pressure analysis in detecting compensatory gait strategies remains underexplored.
METHODS: This study included 10 male patients (30.27 ± 5.59 years; 178.37 ± 6.30 cm; 84.85 ± 10.74 kg) who underwent ACL reconstruction using bone-patellar tendon-bone autografts. Assessments were performed preoperatively and at 3 and 6 months postoperatively, evalu

INTRODUCTION: Osteochondroma is a common benign bone tumor, but its occurrence in the spine is rare, particularly in the lumbar region. Among spinal osteochondromas, involvement of the pedicle is uncommon. This case highlights a rare presentation of a pedunculated exostosis arising from the right pedicle of the first lumbar vertebra (L1) with a fracture at its base, leading to persistent lumbar pain and swelling. Given its potential for neurological compromise and malignant transformation, timely recognition and surgical intervention are crucial.

BACKGROUND: Arthroscopy-assisted lower trapezius tendon transfer (aLTT) has emerged as a reasonable treatment option for posterosuperior irreparable rotator cuff tears (PSIRCTs) due to its biomechanical advantages and favorable clinical outcomes. Although there are various surgical techniques for aLTT, the optimal positioning of the graft on the humeral head footprint is unknown in aLTT. This study aimed to evaluate clinical and radiological outcomes based on the interpositional graft position of aLTT graft in PSIRCTs.
METHODS: Patients who underwent aLTT for PSIRCTs from 2017 to 2022 were re

BACKGROUND: Subcoracoid impingement co-occurs with subacromial impingement in 19% of the patients with rotator cuff tears. The influence of subcoracoid impingement in patients with full-thickness rotator cuff tear (FT-RCT) remains unknown. Therefore, it is worthwhile to investigate potential differences between FT-RCT patients with and without subcoracoid impingement.
METHODS: Twenty-two participants were categorized as having an FT-RCT combined with subcoracoid impingement, while 25 others were diagnosed with an FT-RCT without subcoracoid impingement. Characteristics and tear sizes were reco

In high-velocity sports, hamstring strain injuries are common causes of missed play and have high rates of reinjury. Evaluating the severity and location of a hamstring strain injury, currently graded by a clinician using a semiqualitative muscle injury classification score (e.g. as one method, British Athletics Muscle Injury Classification - BAMIC) to describe edema presence and location, aids in guiding athlete recovery. In this study, automated artificial intelligence (AI) models were developed and deployed to automatically segment edema, hamstring muscle and tendon structures using T2-weig

BACKGROUND: Suprascapular nerve lesions caused by rotator cuff tears have been widely reported. However, the accuracy of electromyography as a definitive diagnostic method for nerve lesions is controversial. This study aims to evaluate the diagnostic effectiveness of electromyography for nerve lesions following varying degrees of rotator cuff tears in rat models.
METHODS: Twenty rats underwent bilateral rotator cuff tenotomy and were divided into four groups to simulate clinically relevant rotator cuff tears of different sizes. The supraspinatus tendon free ends in three groups were sutured a

BACKGROUND AND OBJECTIVES: Recovery of upper extremity function is considered the top priority in the tetraplegic population. Midcervical injury often results in paralysis of the hand and loss of elbow extension. Elbow extension is essential for extending one's workspace and facilitating mobility (ie, self-transfer, wheelchair propulsion). Tendon transfers have traditionally been used to restore elbow extension, but the axillary to radial nerve transfer (ARNT) has emerged as a promising alternative. Our objective was to review a case series of single-surgeon outcomes for recovery of elbow exte

Rotator cuff tears stand as the most prevalent shoulder condition prompting medical intervention. Failure rates as high as 60% have been reported after surgery, indicating the need for improved treatments. The aim of this study was to confirm whether an implant composed of freeze-dried chitosan and thrombin rehydrated in autologous platelet-rich plasma (CS-FIIa-PRP) is effective in improving supraspinatus tendon regeneration in a New Zealand white rabbit model. Complete tears of the supraspinatus tendon were created bilaterally, then repaired using transosseous sutures. On the treated shoulder

INTRODUCTION/OBJECTIVES: Biceps tenodesis is a widely accepted treatment for patients with long-head biceps tendinopathy, particularly in younger, high-demand individuals. Despite its efficacy, the biomechanically optimal location for biceps tenodesis remains a subject of ongoing debate. This biomechanical study aims to compare the ultimate failure load and tendon displacement across four distinct biceps tenodesis locations in fresh-frozen human humeri to determine the strongest and most stable fixation site.
METHODS: Fresh-frozen porcine tendons were fixed using interference screws at four b

The hamstrings are critical for athletic performance; however, no study has examined differences in hamstring muscle-tendon geometry (cross-sectional area/volume) and muscle typology (proportion of Type I/II fibers) between elite sprinters/jumpers and recreationally active individuals. This study aimed to compare hamstring geometry and typology between these groups and examine how these characteristics relate to sprint and strength performance. Elite sprint and jump athletes (n = 15, 3 female, 21.7 ± 2.2 y, 180.6 ± 9.9 cm, 72.2 ± 9.6 kg) and recreationally active individuals (n = 15, 4 female,

Rotator cuff tears decrease glenohumeral joint stability; however, surrounding uninjured muscles could compensate for the reduced function of injured muscles to maintain joint stability. This study set out to analyze compensatory mechanisms that could maintain glenohumeral joint stability following rotator cuff tears of varying severity during normal daily life, using a musculoskeletal model simulation of rotator cuff tears. Fifteen healthy males performed 6 activities including shoulder flexion and abduction, contralateral shoulder reaching, head reaching, and lifting a 1 kg object placed on
