Aberrant peripheral nerves and microneuromas in otherwise normal medullas.

The occurrence of aberrant peripheral nerve (PN) fibers in otherwise normal medullas was studied in 1,016 consecutively autopsied patients. Fine, myelinated PN fibers occurred as small parallel bundles or microneuromas about arteries and arterioles exclusively in the dorsal half of the medulla. They were more common in male than female patients, but were not related to any specific systemic disease process. The PN lesions were not found in patients under 20 years of age, but were observed in 1.5, 9.6, and 16.4% of those patients 20-49, 50-69, and 70-89 years of age, respectively. We regard the

The broken needle: report of case.

After any patient who is to receive a local anesthetic has given informed consent about the possibility of broken instruments, the practitioner should carefully prepare the patient and instrumentation for a successful anesthetic. If a needle does break and cannot be immediately removed, the practitioner should not try to retrieve it. The patient should be referred to a knowledgeable specialist familiar with the detailed anatomy of the head and neck and the surgical removal of foreign bodies from the same.

Forum. Femoral neck surgery using a local anaesthetic technique.

Thirty cases of femoral sub-capital fractures, Garden grades 1-4, were reduced and internally fixed with crossed Garden screws using femoral nerve block. In addition, sedation and analgesia was provided by low dose ketamine and diazepam. No deaths or other complications occurred in these patients. In a similar group of patients who received spinal analgesia in the same unit under similar conditions there was one death and two cerebrovascular accidents.

Injuries to the neck and upper extremities of dancers.

In general, injuries to dancers involving the cervical spine and upper extremities are quite mild and cause only limited disability. When such injuries do not involve specific elements of the musculoskeletal system, their origin may be in the more subtle neurovascular structures of the neck. The importance of a thorough examination is emphasized.

Experiences with phrenic nerve pacing in children.

We report a series of four spinal cord-injured children with ventilator dependency who were implanted with radiofrequency phrenic nerve pacers. At injury, their ages ranged from 6 to 9 years. Implantation occurred from 15 to 47 months following injury. The longest term of pacing has been 59 months. One patient, who was being paced full-time, died suddenly at 32 months of a viral myocarditis. Two patients underwent bipolar phrenic implantation in the neck, while two other patients underwent bilateral thoracotomy with monopolar electrode implantation in the mediastinum. There have been no compli

Catheter embolization following median sternotomy.

A case of intraoperative internal jugular vein catheter embolization in a patient undergoing median sternotomy for open-heart surgery is described. This complication suggests that right sided cannulation of the internal jugular vein is to be preferred in all patients having a median sternotomy in order to avoid the risk of intersection by median sternotomy.

The effects of embolectomy-thrombectomy catheters on vascular architecture.

The purpose of this report is to describe the evolution of an embolectomy-thrombectomy catheter (ETC) injury over a six week period. Carotid arteries and jugular veins in six adult dogs were subjected to ETC withdrawals at a standard velocity and balloon size. Vascular segments were excised as early as one hour and as late as six weeks. The specimens were prepared for light, scanning electron and transmission electron microscopic examination. In early specimens, arteries and veins showed endothelial denudation followed by regeneration. In later specimens, the arteries showed progressive disrup

Relationship between intracranial hypertension and ultrasonic patterns of the common carotid artery and the internal jugular vein.

The Doppler technique has only been used in neurological practice to evidence local vessel pathology such as occlusion or stenosis. Both common carotid artery and internal jugular vein flows can change not only because of pathological processes of the vessels but also because of impedance of their distribution territory. In this report we analyze the relationship between flow velocity, one of the parameters of blood flow, and intracranial impedance variations which occur in cerebral concussion, tumors and acute vascular cerebral pathology. During our observations we noticed that the diastolic

[Seldinger's technic of puncture of the internal jugular vein in a patient with a coagulation disorder. Report of a fatal complication].

It is reported that the catheterization of the internal jugular vein in patients with severe coagulopathies had fatal consequences. The catheterization was done with a Sheldon-catheter for dialysis. The technique of Seldinger was performed, which has been described for morbid patients in most literatures.

Experience using central venous access for long-term hemodialysis. A new concept.

Central venous access for acute renal failure has been used for a number of years. The femoral vein and, more recently, the subclavian vein have been the routes of access. This technique has many advantages, however, it also has some significant limitations. We have recently been using a catheter for long- as well as short-term hemodialysis. The catheter is placed by means of a short incision through an opening in the internal jugular vein, and maneuvered so that the tip lies in the superior portion of the right atrium. After dialysis, the catheter is filled with heparin. The catheter then req

Treatment of traumatic carotid-cavernous fistula using detachable balloon catheters.

The goal of therapy in patients with traumatic carotid-cavernous fistulas is to occlude the fistula, preferably while maintaining the carotid blood flow. Surgical techniques that treat the fistula remote from the cavernous sinus often cannot maintain carotid patency. Various interventional techniques using detachable balloons have been developed. The most common technique uses the endarterial route, introducing the balloon catheter in the neck or the groin. If the balloon is detached in the cavernous sinus, the carotid blood flow will be preserved. A second approach uses the venous retrograde

Fatal retropleural hematoma complicating internal jugular vein catheterization. A case report.

This report describes a case of fatal retropleural hematoma complicating percutaneous insertion of a central vein catheter. The patient was a 55-year-old female undergoing surgery for severe rheumatic mitral valve disease. Factors responsible for this catastrophic incident are the anticoagulation of the patient, the edematous state of the soft tissues, and the multiple punctures of the wall of the jugular vein.

Tracheal puncture. A complication of percutaneous internal jugular vein cannulation.

We report two cases in which tracheal puncture (diagnosed by puncture of the orotracheal tube cuff) followed internal jugular vein cannulation by the posterior approach. This complication is only likely to occur when using posterior approach and it may be followed by potentially dangerous subcutaneous emphysema, pneumomediastinum or air trapping between the chest wall and the pleura. This has led us to believe that the posterior approach should be substituted by the alternative central or anterior approach whenever it is possible.

Hydrothorax as a late complication of central venous indwelling catheters.

Two cases of delayed onset hydrothorax following central venous catheterization are reported. Both involved left internal jugular vein percutaneous insertion sites, and chest roentgenograms of both demonstrated that the catheter tips were juxtaposed against the superior vena cava wall. Although both catheters functioned well for several days, we believe that this left neck site allowed for catheter tip motion, resulting in delayed perforation of the vessel. To avoid this complication we recommend that central venous catheters be checked routinely for position, with the catheter tip being paral

Mechanism and frequency of brachial plexus injury in open-heart surgery: a prospective analysis.

A computer-assisted prospective analysis of 531 patients undergoing open-heart operations revealed that 26 patients (5%) sustained brachial plexus injury. In 22 of the 26 patients (85%), the lesion involved the lower trunk or C8-T1 nerve roots. Electromyograms confirmed the clinical impression in 13 patients. In 19 of the 26 patients (73%), the side on which the plexus lesion was found correlated with the side of internal jugular vein cannulation. Because of the anatomical proximity of the lower trunk to the internal jugular vein and the preponderance of lower trunk lesions, we postulate that

Arteriovenous fistulas following central venous catheterization.

We report three patients in whom arteriovenous fistulas probably occurred following placement of central venous catheters. Two fistulas apparently followed internal jugular vein catheterization (or attempts), and one was demonstrated angiographically following subclavian vein cannulation. One fistula was repaired operatively, and in this case two separate fistulas in the same anatomical region were found. One patient refused surgery but remained asymptomatic after 6 months of follow-up. In the third patient the fistula probably spontaneously disappeared within one week of its appearance. We di
