Pre-existing vertebrojugular fistula unmasked following an uncomplicated internal jugular vein cannulation and its management.

A vertebrojugular fistula (VJF) is a vascular malformation characterised by abnormal communication between the vertebral artery (VA) and the internal jugular vein (IJV). It can occur following neck trauma, iatrogenic injury or spontaneously in connective tissue disorders, and rarely, it can be congenital. A young adult man in his early 20s with aplastic anaemia was planned for allogeneic haematopoietic stem cell transplantation (Allo-HSCT) for which right IJV cannulation was planned. After a single attempt at straightforward IJV cannulation, high flow was observed through all lumens; an anomal

Cardiac Catheterization for Assessment of Pulmonary Hypertension in Preterm Lambs.

Bronchopulmonary dysplasia with pulmonary hypertension (BPD-PH) is a devastating complication of preterm birth that is associated with increased morbidity and mortality. Preterm birth disrupts normal lung and vascular development, and postnatal exposures such as mechanical ventilation and oxygen therapy can further impair lung and pulmonary vascular growth, contributing to the development of BPD-PH. In the preterm lamb model of evolving BPD, lambs are delivered preterm and mechanically ventilated for up to 21 days, incorporating interrupted lung development along with injury from mechanical ve

Intraoperative Ultrasound for Post-lesion Confirmation in Cervical DREZ Surgery: Technical Feasibility and Clinical Outcomes.

BACKGROUND: Microsurgical dorsal root entry zone (DREZ) lesioning is an established treatment for refractory pain following brachial plexus avulsion. However, lesion depth and trajectory are critical technical variables that may influence safety and outcomes. We report our single-center experience using intraoperative ultrasound for systematic post-lesion confirmation during cervical DREZ lesioning. METHODS: Between January 2016 and March 2025, 23 consecutive patients with complete or partial brachial plexus avulsion and severe neuropathic pain (VAS ≥6) refractory to optimized pharmacological

Injury patterns and epidemiology of orthopedic trauma in polytrauma ICU patients: A 10-year retrospective analysis at major trauma hospital.

BACKGROUND: Polytrauma requiring intensive care remains a leading cause of morbidity and mortality, particularly among patients with orthopedic injuries. Despite advances in trauma systems and surgical management, outcomes for this cohort are influenced by complex interactions between injury burden, physiological status, and pre-existing comorbidities. There remains limited evidence focused specifically on predictors of outcome within orthopedic trauma patients admitted to the intensive care unit (ICU). METHODS: This retrospective cohort study analyzed all adults (≥18 years) admitted with ort

Catheter malposition leading to superior vena cava perforation: A rare but serious complication of hemodialysis access.

BACKGROUND: Central venous catheterization through the internal jugular vein is a standard procedure for hemodialysis access in end-stage renal disease (ESRD). Although generally safe, serious and life-threatening complications can occur. CASE PRESENTATION: We present a rare case of superior vena cava (SVC) perforation during tunneled dialysis catheter (PermCath) insertion in a patient with chronic renal failure. The injury led to mediastinal hematoma, bilateral hemothoraces, and hemorrhagic shock, requiring emergency thoracotomy and surgical repair. DISCUSSION: Perforation of the SVC is an

Optimum Sonoanatomical Approach for Selective Cervical Nerve Root Block: Evaluation of Three Different Needle Pathways in Cadavers and Volunteers.

OBJECTIVE: To identify the safest approach for selective cervical spinal nerve block (SCSNB) using sonoanatomic evaluation of spinal nerves (SNs) and adjacent vascular structures. DESIGN: This study included 50 healthy volunteers and 7 cadavers. In volunteers, C5-C7 SNs were evaluated using ultrasound. Three virtual needle trajectories posterior-to-anterior (P-A), anterior-to-posterior (A-P), and lateral-to-medial (L-M) were assessed within a region of interest. Cadaveric dissections examined morphometric relationships between the vertebral artery (VA), ascending cervical artery (ACA), deep c

Predicting cervical muscle forces in fighter pilots: a comparison of shear wave elastography and PID controllers using finite element analysis.

PURPOSE: Helmet loads have been identified as a source of neck pain in fighter pilots. To minimize risk for neck pain, the effect of helmets on cervical spine biomechanics must be understood. In-vivo methods are difficult and invasive, so finite element modeling (FEM) techniques have been utilized recently to understand neck pathology development. Accurate muscle forces are crucial for physiologically relevant results. Therefore, this study evaluated the relationship between helmet loads and cervical muscle forces using two FEM muscle activation strategies. METHODS: A validated cervical spine

Multimodal Imaging of a Post-Traumatic Cervical Arteriovenous Fistula with Concomitant Pseudoaneurysm After a Gunshot Wound Treated with a Covered Stent-Graft.

Background: Arteriovenous fistula is an abnormal connection between an artery and a vein. Post-traumatic fistulas are uncommon and usually occur after penetrating injuries, including gunshot wounds. Cervical arteriovenous fistulas are particularly rare and may coexist with a pseudoaneurysm, creating diagnostic and therapeutic challenges because of the proximity of major vascular, neurological, and aerodigestive structures. In hemodynamically stable patients, the limited external appearance of the wound may underestimate the extent of internal vascular injury; therefore, imaging is central to d

Trust, but verify: the importance of additional imaging after BCVI screening.

BACKGROUND: Clinical practice guidelines recommend screening for blunt cerebrovascular injury (BCVI) based on the Denver Criteria. BCVI is typically treated with antithrombotic therapy, which may be high-risk in patients with concomitant brain or spine injuries. At our center, we often perform additional imaging in high-risk patients after positive initial screening for BCVI. Our study investigates how frequently follow-up imaging changes diagnosis and management of patients screening positive for BCVI. METHODS: A cross-sectional study of all trauma patients admitted to a level 1 trauma cente

Cervical nerve root contribution to diaphragm motor innervation revisited via intraoperative clinical and neurophysiological observations.

OBJECTIVE: Classical descriptions state that diaphragmatic motor innervation arises predominantly from the fourth cervical nerve root (C4), with additional contributions from C3 and C5. However, clinical evidence supporting the functional involvement of C3 and C5 remains limited. Study aimed to reassess cervical root contribution to diaphragmatic innervation through intraoperative observations. METHODS: Eighteen consecutive patients undergoing dorsal root entry zone (DREZ) lesioning for refractory pain in upper-limb mostly due to brachial plexus injury were included. The procedure allowed int

PICC migration and perforation of internal jugular vein after neck dissection.

Peripherally inserted central catheters (PICCs) were introduced as an alternative to centrally inserted central catheters (CICCs) inserted into the jugular vein to provide central venous access. In head and neck reconstructive surgery, PICCs are preferred over CICCs because they do not obstruct surgical access, such as during neck dissection. Additionally, PICCs offer a safer option than peripheral access for the infusion of vesicants, irritants, hyperosmolar solutions, antibiotics, prolonged parenteral nutrition, and chemotherapy agents. We present a case of perforation of the internal jugula

A predictable cascade of junctional failure following progressive long-segment fusion: an 11-year case analysis.

Instrumented spinal fusion is a common treatment for degenerative spinal disease but may lead to progressive mechanical and biological complications, especially in long multilevel constructs. We report the case of a 62-year-old woman who developed serial adjacent segment degeneration, pseudarthrosis, implant failure, vertebral fractures, and progressive deformity over 11 years following posterior lumbar fusion. Multiple revision surgeries with stepwise cranial extension to the upper thoracic spine were required. Despite advanced fixation techniques-including multi-rod constructs, iliac fixatio

[Trochanteric fixation nail advanced for trochanteric femoral fractures : Central cut-out as a new problem?].

INTRODUCTION: Trochanteric femoral fractures (TF) are mostly treated with intramedullary nail osteosynthesis. An inherent but rare complication of the system is the central cut-out of the femoral neck helical blade, i.e., the dislocation through the femoral head towards the acetabulum. After switching from proximal femoral nail antirotation (PFNA) to the trochanteric fixation nail advanced (TFNA) by the manufacturer, this rare type of complication has repeatedly been observed. MATERIAL AND METHOD: The retrospective single center case-control study was based on a database which includes all TF

What is the risk of dislocation after hip arthroplasty for patients with neuromuscular disease: a systematic review and meta-analysis.

PURPOSE: Recent research has demonstrated excellent functional outcomes for patients with neuromuscular (NM) disease undergoing hip arthroplasty (HA). However, significant concerns persist regarding a potentially elevated risk of postoperative instability and dislocation in this specific cohort. Therefore, our objective was to synthesize the existing evidence through a systematic review and meta-analysis to evaluate the risks of dislocation in NM patients after HA. The purpose of this study was to answer the following question using a systematic review and meta-analysis: for patients undergoin

Towards Automated Spine Fracture Detection on Whole-Body CT of Polytraumatized Patients.

Treatment of severely injured patients is challenging, and timely reading of whole-body computed tomography (WBCT) images therefore crucial. Artificial intelligence is increasingly used to prioritize and detect acute injuries in this context. Algorithms focusing on the cervical spine and compression fractures have been deployed successfully. However, tools for whole spine assessment and the entirety of fracture morphologies are lacking. We aimed to investigate the capabilities of an algorithm to detect spine fractures on WBCTs and factors contributing to the difficulties in its development. A
