Ipsilateral femoral neck and shaft fractures occur in 2.5% to 6% of femur fractures. The injury results from high energy trauma. Victims are usually young, with multiple associated injuries. The diagnosis of the neck fracture is delayed in 19% to 31% of patients. The neck fracture line is almost vertical and nondisplaced, or minimally displaced in 26% to 59% of cases. A 35-year-old man sustained multiple injuries including a fractured pelvis, a ruptured urethra, and a femur fractured at the neck, the shaft, and the supracondylar region. A series of fourteen difficult fractures and nonunions of the humerus and elbow have been treated over a period of twenty-eight years with a modified Kuntscher Nail. Closed, reamed, antegrade nailing remains the standard of care for femoral shaft fractures. Between 1974 and 1989 39 patients with multi-fragmentary fractures of the head of the radius or radial neck fractures were treated by resection of the radial head at the Department of Trauma and Reconstructive Surgery (UKE-University Hospital Hamburg). From 1986 to 1991 we treated 63 patients with subtrochanteric fracture of the femur. The etiology in 53 patients with 40 fractures mostly pedestrians, hit by a car; or motor vehicle accidents, in other words high velocity injuries that resulted in a significant number of multiply injured patients of whom 38 were polytraumatized, by our definition a multiply injured person with a life threatening injury of at least one of the following body parts: head, neck, thorax, abdomen, spine, extremities with neurovascular compromise and pelvic girdle.

The crash of Avianca Airlines flight no. 052 en route to JFK Airport on January 25, 1990, in Cove Neck, New York, resulted in the death of 72 passengers. Eighty-nine victims were admitted to 13 regional hospitals. Despite difficult access to the wooded crash site, early warning and prompt response by 37 volunteer fire and rescue units resulted in organized EMS triage and rapid hospital transport. This report reviews the specific injuries incurred, highlights the team management approach to a major aviation accident in a suburban area, and studies the likelihood of accidents of this magnitude.

The purpose of this study was to determine the effectiveness and associated problems of emergency intubation in 605 injured infants and children admitted to the Children's Hospital of Pittsburgh in 1987. We identified 63 patients (10.4%) undergoing endotracheal intubation at the scene of injury, at a referring hospital or in our emergency department. Twenty-eight patients with displaced femoral neck fractures were operated on with multiple pinning combined with cancellous bone grafting from the ipsilateral greater trochanter. A 33 year old zoo keeper was attacked by a Sumatran tiger in captivity. Apart from severe lacerations and penetrating wounds to the head and neck, the patient sustained comminuted fractures of C1 and C2 vertebrae with resultant high laceration of the spinal cord. Two hundred twelve patients were treated for facial or skull trauma at the West Virginia University Hospital between the years 1977 and 1987. Sixty-six of these patients had frontal sinus or nasofrontal duct trauma. The term 'deux étages fracture' is used in different meanings. Therefore, an exact definition seems necessary. 38 own cases of this kind treated in the last 5 years are analysed. Special problems of diagnosis and treatment of this type of fracture are dealt with. Because of the risky complications primary definitive treatment is emphasized especially in cases of compound fractures and in cases with severe simultaneous lesions.
