Treatment of a femoral neck fracture in a patient with dwarfism: a case report.

BACKGROUND: Femoral neck fractures in patients with pituitary dwarfism present significant surgical challenges due to anatomical variations, compromised bone quality, and limited therapeutic options. The management of such cases requires careful consideration of both the anatomical constraints of and the potential complications associated with various fixation methods. CASE PRESENTATION: This report presents the case of a 30-year-old female with congenital pituitary dwarfism who sustained a subcapital femoral neck fracture following trauma. After suffering a workplace injury due to a fall, th

Should the lumbar or femoral neck young adult mean percentage be measured in patients with distal radius fractures? A case-control study.

BACKGROUND: In the 2015 guidelines for the prevention and treatment of osteoporosis in Japan, the young adult mean percentage (%YAM) of the lumbar spine or femoral neck is required to diagnose osteoporosis in patients with distal radius fracture (DRF). When the %YAM of multiple locations is measured, the lower %YAM is adopted for the diagnosis. If the area with a lower %YAM is known, bone mineral densitometry tests may only need to be performed at one site. This study aimed to compare the lumbar and femoral neck %YAMs of patients with DRFs to find out the lower %YAM site. METHODS: We retrospe

Esophageal perforation more than 10 years after anterior cervical spine surgery: a case report and literature review.

Esophageal perforation is a rare but serious complication that can occur post-cervical spine surgery. This case report presents the clinical course, diagnostic challenges and management strategies of a patient who had a late-diagnosis esophageal perforation after anterior cervical spine surgery (ACSS). A woman in her 50s underwent ACSS for cervical spondylosis. Three months postoperatively, she experienced persistent right neck and shoulder pain. Despite multiple consultations, an esophageal perforation was only diagnosed 10 years later when a neck mass ruptured, discharging food debris. Surgi

Pasteurella multocida Spondylodiscitis in an Immunocompetent Patient.

We present the case of an 82-year-old woman, previously independent in activities of daily living, who developed fever, myalgias, and headache over one week. Two weeks earlier, she had been treated with antibiotics for a lower respiratory tract infection. The patient had no history of immunosuppression and was a pet owner. She was admitted to the emergency department (ED) with a fever and multiple perforating wounds on her hands. Laboratory findings revealed elevated inflammatory markers, including C-reactive protein and procalcitonin, without an obvious infectious source. During observation a

Femoral Neck Complications in Isolated Femoral Neck Fractures Versus Ipsilateral Femoral Neck and Shaft Fractures.

OBJECTIVES: There are limited data on the difference in outcomes between patients with isolated femoral neck fractures (IsoFN) and those with a femoral neck and ipsilateral femoral shaft fracture (IpsiFNS) in terms of osteonecrosis, nonunion, and revision surgery rates. We hypothesized that displaced IsoFN fractures would have higher rates of femoral neck nonunion, osteonecrosis, and hip revision surgery than displaced IpsiFNS fractures. Comparisons were made to determine the relationship between these outcomes and other variables. METHODS: A retrospective review of patients age 18 to 55 year

Bone Scintigraphy and Laboratory Clues to Osteomalacia in a Middle-Aged Man With Hip Fractures.

A 43-year-old man with a 1-year history of bilateral hip pain, worsened by standing and walking, presented with a limping gait. x-rays showed diffuse osteoporosis (osteoblastic-lytic), and a bone scan revealed symmetric uptake in the costochondral junctions (rosary beads), femoral neck, proximal tibia, and feet, with no visualization of the kidneys, suggesting a metabolic disorder or neoplasm. Computed tomography revealed osteolytic lesions in multiple bones. Suspecting metabolic bone disease or metastasis, laboratory tests were conducted. Magnetic resonance imaging confirmed bilateral femoral

Sequential Cranial and Overlooked Cervical Spine Injuries Due to Head Trauma: A Billiard-Like Mechanism.

A 55-year-old male construction worker presented with multiple fractures after falling downstairs and striking his forehead on a concrete wall, an incident preceded by consuming approximately 50 g of alcohol. Initial evaluation revealed a contaminated 7 cm forehead laceration and a depressed frontal bone fracture, with no neurological deficits or intracranial bleeding on computed tomography (CT), leading to discharge with follow-up instructions despite mild intoxication. The next day, neck pain prompted further imaging, uncovering a complex injury pattern: frontal bone fracture, occipital base

Fireworks and Injury in the United States: A Review of Epidemiology, Clinical Burden, and Policy Implications.

Fireworks are a hallmark of American celebrations, yet they generate a growing and under-recognized burden of injury. The aim of this narrative review is to integrate current United States evidence on firework-related harm, reveal clinical and economic patterns, and evaluate how policy shapes risk. Recent studies and national surveillance reports show a steady rise in emergency visits linked to consumer fireworks, with males comprising most patients. The hands and upper extremities sustain the largest share of harm, followed by the eyes and the head, face, and neck region. Although less freque

Transoral Robotic-Assisted Removal of a Submucosal Pyriform Sinus Foreign Body.

BACKGROUND: Submucosal foreign bodies of the medial pyriform sinus represent a challenge given limited visualization and difficult retraction with endoscopic approaches, and risk of injury to the recurrent laryngeal nerve with open approaches. Transoral robotic surgery (TORS) offers improved visualization and multiple working arms to facilitate retraction. We present a novel case utilizing the Da Vinci SP system for removal of an embedded fishbone of the medial pyriform sinus. RESULTS: An 84-year-old woman presented with five days of odynophagia and globus sensation after swallowing a fish bo

Costal Cartilage Chip Grafting for High-Arched Narrow Palate Deformity Secondary to Palatal Bone Fracture.

Palatal bone fractures are often associated with vertical maxillary fractures, but direct reduction of the palatal bone itself is rarely performed. We report a case in which a high-arched narrow palate deformity following palatal bone fracture associated with multiple facial fractures was successfully corrected by grafting costal cartilage chips to the hard palate. The patient was a 25-year-old man. At the age of 19, he sustained a cervical spinal cord injury at the C4 level and multiple facial fractures-including maxillary fracture with zygomatic and palatal bone fractures, as well as mandibu

Global burden of lower respiratory infections and aetiologies, 1990-2023: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Lower respiratory infections (LRIs) remain the world's leading infectious cause of death. This analysis from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2023 provides global, regional, and national estimates of LRI incidence, mortality, and disability-adjusted life-years (DALYs), with attribution to 26 pathogens, including 11 newly modelled pathogens, across 204 countries and territories from 1990 to 2023. With new data and revised modelling techniques, these estimates serve as an update and expansion to GBD 2021. Through these estimates, we also aimed to

Traumaplasty: When and How to Perform Acute Arthroplasty for Fractures Around the Hip in the Elderly Patient.

Acute fractures around the hip are prevalent injuries associated with potentially devastating outcomes. The growing utilization of arthroplasty for femoral neck fractures in the elderly is likely a result of improvements in reoperation rates and postoperative function. Compared to hemiarthroplasty, total hip arthroplasty is associated with a slight functional benefit that is unlikely noticeable for many patients, as well as minimal differences in complications and patient reported outcome measures. However, the evidence supporting cement use in femoral stem fixation is robust. Multiple high po

An Unexpected Case of Generalized Tetanus.

Tetanus is a disease of the nervous system caused by a toxin produced by Clostridium tetani, an anaerobe found in high concentrations in the soil. The occurrence of tetanus is related to contaminated traumatic wounds, and most patients have had some failure in their immunization. However, there are rare case reports of generalized tetanus in patients with proper vaccination schemes who failed to receive appropriate prophylaxis after high-risk exposure. A 79-year-old woman presented to the emergency room (ER) with a large wound on her leg caused by an iron pipe during agricultural work. Tetanus

Diagnosis and Treatment of Cervical Spine Fractures in Children.

BACKGROUND: Cervical spine injuries in children are relatively uncommon and account for approximately 1.5% of trauma admissions. The aim of our study was to determine the incidence and patterns of pediatric cervical spine fractures in a single Trauma Center over the last decade. MATERIAL AND METHODS: The hospital's medical database was queried for data from the period between January 2014 and December 2023. An initial search revealed that 511 patients had been hospitalized with neck injuries in that time frame. Among these records, we identified 24 patients with a fracture of the cervical spi

Consensus and Equipose in the Management of Military Trainee Femoral Neck Stress Fractures: A Survey of Military Surgeons.

BACKGROUND: Femoral neck stress fractures (FNSFs) are a unique injury pattern not commonly treated in the civilian trauma population; however, it is particularly high with military trainees engaged in basic combat training. To date, no study has surveyed a population of military orthopedic surgeons on treatment preferences for military service members (SMs) with FNSF. QUESTIONS: We aim to evaluate the extent of clinical equipoise that exists in the management of these injuries, hypothesizing that there would be consensus in the factors dictating surgical and non-surgical intervention for FNSF

Observational Study of the Clinical Impact of E-scooter Injuries at a Major Trauma Centre.

Background Due to the increased use of e-scooters in the UK, associated injuries have increased, and its cost has increased as well. Worldwide data on injuries related to e-scooter use are relatively limited, owing to the short duration since their first introduction to the public.  Aim There has been an increase in using e-scooters as a method of transport recently. It was noticed that the severity and frequency of its injuries are increasing as well. This study focuses on the frequency of e-scooter-associated injuries, especially open fractures, and to evaluate its burden on the health syste
