STUDY DESIGN: Systematic Review.
BACKGROUND: A variety of C2 (axis) and C2/3 historical classification systems have been proposed but there still remains a need for a rational hierarchical means of characterizing these traumatic spinal injuries which involve the axis. We evaluate the utility of existing classification systems and discuss the evolution of the validated AO Spine Upper Cervical Injury Classification (UCIC) System.
METHODS: A comprehensive search of MEDLINE, EMBASE and Cochrane Databases was performed in keeping with the Preferred Reporting Items for Systematic reviews and Meta-

Floating shoulder associated with multiple rib fractures with cervical dorsal spine fractures and lung injury has not been reported in the literature. Patient underwent single stage elective clavicle and scapula fracture stabilization. Good functional outcome at 6 months postoperative with near normal range of movements. Surgery allows early mobilization and better functional outcome. A CT scan of thorax and spine is highly recommended in such high velocity shoulder injuries to detect associated rib, spinal fractures and lung injury.

Injuries of the upper cervical spine represent 1/3 of all cervical spine injuries and approximately 40 % result by the death. Every level of the cervical spine can be injured - fractures of condyles of the occipital bone (CO), atlantooccipital dislocation (AOD), fractures of the Atlas (C1), atlantoaxial dislocation (AAD) and fractures of the axis (C2). Most of cases in younger patients are caused by high-energy trauma, while by elderly people, because of the osteoporosis, is needed much less energy and even simple falls can cause the injury of the cervical spine. That´s why the etiology of inj

BACKGROUND: Polytrauma requiring intensive care remains a leading cause of morbidity and mortality, particularly among patients with orthopedic injuries. Despite advances in trauma systems and surgical management, outcomes for this cohort are influenced by complex interactions between injury burden, physiological status, and pre-existing comorbidities. There remains limited evidence focused specifically on predictors of outcome within orthopedic trauma patients admitted to the intensive care unit (ICU).
METHODS: This retrospective cohort study analyzed all adults (≥18 years) admitted with ort

BACKGROUND: Clinical practice guidelines recommend screening for blunt cerebrovascular injury (BCVI) based on the Denver Criteria. BCVI is typically treated with antithrombotic therapy, which may be high-risk in patients with concomitant brain or spine injuries. At our center, we often perform additional imaging in high-risk patients after positive initial screening for BCVI. Our study investigates how frequently follow-up imaging changes diagnosis and management of patients screening positive for BCVI.
METHODS: A cross-sectional study of all trauma patients admitted to a level 1 trauma cente

Instrumented spinal fusion is a common treatment for degenerative spinal disease but may lead to progressive mechanical and biological complications, especially in long multilevel constructs. We report the case of a 62-year-old woman who developed serial adjacent segment degeneration, pseudarthrosis, implant failure, vertebral fractures, and progressive deformity over 11 years following posterior lumbar fusion. Multiple revision surgeries with stepwise cranial extension to the upper thoracic spine were required. Despite advanced fixation techniques-including multi-rod constructs, iliac fixatio

Treatment of severely injured patients is challenging, and timely reading of whole-body computed tomography (WBCT) images therefore crucial. Artificial intelligence is increasingly used to prioritize and detect acute injuries in this context. Algorithms focusing on the cervical spine and compression fractures have been deployed successfully. However, tools for whole spine assessment and the entirety of fracture morphologies are lacking. We aimed to investigate the capabilities of an algorithm to detect spine fractures on WBCTs and factors contributing to the difficulties in its development. A

OBJECTIVES: Isolated cervical transverse process fractures (TPFs) are increasingly identified in trauma. Although generally considered stable and nonoperative, current guidelines provide inconsistent recommendations on when to pursue spine consultation or computed tomography angiography (CTA) to assess for blunt cerebrovascular injury (BCVI). This scoping review aimed to determine which clinical or radiographic features of isolated cervical TPFs have been used or reported in association with spine consultation and/or BCVI screening.
METHODS: A comprehensive search of 5 databases and gray lite

BACKGROUND: The optimal surgical strategy for unstable hangman's fractures remains controversial, particularly in cases where posterior fixation is unsuitable and evidence for alternative approaches is lacking.
CASE PRESENTATION: A 58-year-old man presented with a C2 vertebral body fracture, right C2 pedicle fracture, and concurrent C2/C3 disc injury, which manifested as a chronic fracture (4 weeks after trauma). Despite multiple courses of conservative treatment and use of a cervicothoracic orthosis, fracture displacement progressed. No significant neurological deficits were noted. The fract

BACKGROUND: Vertical atlantoaxial distraction (AAD) represents a rare and severe form of craniocervical injury caused by high-energy trauma. It involves vertical separation of the atlas and axis due to complete rupture of the ligamentous stabilizers and poses an immediate risk of neurological or vascular compromise.
CASE DESCRIPTION: A 33-year-old male sustained multiple injuries following a suicidal jump from 10 m, including a Type III vertical AAD with 6 mm distraction between the C1 and C2 lateral masses. Imaging revealed complete rupture of the alar, apical, and posterior longitudinal lig

Blunt cerebrovascular injury (BCVI) is an uncommon but potentially life-threatening complication of cervical trauma that may lead to delayed ischemic stroke. Early diagnosis is often challenging because neurological symptoms may initially be absent or mild. We report a case of traumatic internal carotid artery dissection associated with cervical fracture that resulted in large-vessel occlusion and required both endovascular treatment and intensive neurocritical care. A 57-year-old woman fell down stairs and sustained cervical trauma. Initial computed tomography revealed a fracture of the atlas

INTRODUCTION: Accurate segmentation and classification of cervical spine fractures are essential for timely diagnosis and clinical decision-making in trauma care. Existing deep learning approaches often require extensive manual annotations and struggle to maintain anatomical consistency across vertebral levels, limiting their reliability and generalization.
METHODS: We propose a unified three-stage framework that integrates self-supervised representation learning, structure-aware segmentation, and vertebra-level classification. A bone-window 3D masked autoencoder first pretrains the encoder t

Accurate identification and evaluation of spinal column injuries in post-mortem settings are crucial for forensic investigations. While traditional autopsies have long been the gold standard, post-mortem computed tomography (PMCT) has emerged as a promising non-invasive and beneficial tool. This systematic review aims to compare the findings of PMCT scans with autopsies in cases of spinal column injuries. Out of 428 articles screened, 11 studies matched the inclusion criteria. PMCT was superior in visualizing fractures in the atlas, axis, and upper cervical spine compared to autopsies. Moreove
