Accurate identification and evaluation of spinal column injuries in post-mortem settings are crucial for forensic investigations. While traditional autopsies have long been the gold standard, post-mortem computed tomography (PMCT) has emerged as a promising non-invasive and beneficial tool. This systematic review aims to compare the findings of PMCT scans with autopsies in cases of spinal column injuries. Out of 428 articles screened, 11 studies matched the inclusion criteria. PMCT was superior in visualizing fractures in the atlas, axis, and upper cervical spine compared to autopsies. Moreove

Adefovir dipivoxil (ADV), an antiviral agent for chronic hepatitis B virus infection, has been associated with nephrotoxicity, particularly Fanconi syndrome, even at low doses (10 mg/day). Fanconi syndrome is a generalized proximal tubular dysfunction leading to phosphate wasting and hypophosphatemic osteomalacia. Here, the author reports a case of a 58-year-old man with a 2-year history of bone pain involving the left foot, upper back, and left scapula, later extending to the right foot and chest wall, without antecedent trauma. Magnetic resonance imaging revealed insufficiency fractures in b

BACKGROUND: Surgical treatment for unstable atlas fractures is still controversial. Harm's atlantoaxial fusion or more extensive occipitospondylodesis reduces quality of life and functional status. Biomechanical rationales for anterior and posterior C1 osteosynthesis are based on small sample, and the role of ligamentous complex in C1-C2 segment remains understudied.
OBJECTIVE: To evaluate the effectiveness of a new method of surgical treatment for unstable fractures of the first cervical vertebra.
MATERIAL AND METHODS: Biomechanical analysis enrolled 10 anatomical specimens. Various injurie

Fracture of the anterior arch of the atlas, accompanied by posterior atlantoaxial dislocation, typically presents on CT imaging as the odontoid process penetrating through and causing a high-energy shear fracture of the anterior arch. This injury may occur with or without retained fracture fragments. Its characteristic appearance resembles that of a plough traversing the earth, and it has historically been termed the plough fracture. Published case reports documenting this specific fracture pattern remain extremely rare in the literature. In our research group's earlier study on traumatic post

Background/Objectives: Posterior C1-C2 fusion is commonly used for unstable traumatic odontoid injuries, but it is less commonly used for patients who initially undergo nonoperative management and later require salvage fusion. This study compared hospital length of stay, short-term complications, and postoperative radiographic alignment between salvage posterior C1-C2 fusion after failed nonoperative management and primary posterior C1-C2 fusion. Materials and Methods: A retrospective cohort study was performed of 106 adult patients who underwent posterior C1-C2 instrumented fusion for traumat

PURPOSE: Some cases of isolated unstable Jefferson's fracture of the Atlas verbebra require surgical fixation. Historical constructs with occipitocervical fixation or C1C2 posterior Harms-Goel fixation remain associated with functional impairment owing to limited postoperative cervical range of motion. Since 2004, motion-preserving C1 ring lateral mass screws with transverse rod (C1 LMF) has emerged as a possible technical alternative.
METHODS: Retrospective multicentric study of patients operated on for isolated unstable Jefferson's fracture of the Atlas vertebra using C1 LMF in two tertiary

BACKGROUND: Unstable C1 (atlas) fractures pose a substantial surgical challenge due to their proximity to critical neurovascular structures and the unique biomechanical role of the atlantoaxial complex. Disruption of the transverse atlantal ligament (TAL) destabilizes the C1 ring, and traditional fusion constructs sacrifice substantial cervical motion. C1-ring osteosynthesis with a rod and lateral mass screw construct offers a motion-preserving alternative, though screw placement can be technically demanding. In this report, we present the use of stereotactic navigation-assisted open reduction

STUDY DESIGN: Retrospective cohort study.
PURPOSE: To compare three cervical sarcopenia metrics and frailty as preoperative predictors of unfavorable postoperative outcomes following surgical fixation of odontoid fractures.
OVERVIEW OF LITERATURE: Sarcopenia is increasingly recognized as an indicator of physiologic vulnerability in spine surgery and thoracolumbar trauma. However, its prognostic significance in odontoid fractures remains unclear, and no previous study has directly compared cervical sarcopenia measurement sites in this population.
METHODS: Adult patients who underwent surgica

STUDY DESIGN: Global cross-sectional survey.
OBJECTIVE: To establish a surgical algorithm for upper cervical spine injuries using the AO Spine Upper Cervical Injury Classification (UCIC) system, defining operative thresholds using a novel proposed Upper Cervical AO-Spine Injury Score (UC-AOSIS).
SUMMARY OF BACKGROUND DATA: The AO Spine UCIC system provides a hierarchical, morphology-based classification for upper cervical spine trauma. Although reliability has been previously demonstrated, there is no established consensus on operative versus nonoperative management for specific UCIC subtype

OBJECTIVE: To demonstrate the feasibility of a minimally invasive technique for direct C1 screw fixation and to evaluate short- and long-term clinical and radiological outcomes in a case series.
MATERIALS AND METHODS: This study included a consecutive series of patients with type II or III fractures of the C1 vertebra according to the Landells classification. The surgical technique is described in detail, along with short- and long-term outcomes. Owing to the absence of comparison groups, only descriptive statistical methods were used.
RESULTS: This case series included 8 patients (5 men and

STUDY DESIGN: Retrospective multicenter cohort study.
BACKGROUND: Type II odontoid fractures (TTOFs) are common in older adults. Surgical fixation increases union but carries surgical risk, while external immobilization avoids surgical risk but has variable union rates. Pulsed electromagnetic field-based bone growth stimulation (Bone-Stim) is a noninvasive and collar compatible adjunct that has not previously been tested in patients with TTOFs.
OBJECTIVE: To compare radiographic and clinical outcomes of TTOF patients treated with and without adjunct Bone-Stim.
METHODS: A retrospective cohor

INTRODUCTION: Cardiac etiologies are one of the main causes for out of hospital cardiopulmonary arrest (OHCA). Other etiologies might be easily overlooked, leading to delayed causal treatment. One such representative is neurogenic shock following upper cervical spine fractures after minor impact trauma in the elderly with little data on prevalence and risk factors, a drawback that we address in the present study. MATERIAL AND METHODS: Between 01/2013 and 12/2023, all patients ≥ 65 years with a fracture of the first and/or second vertebra after minor impact trauma were retrospectively reviewed

UNLABELLED: This study shows that an artificial intelligence tool can identify patterns of reduced bone mineral density in routine CT scans that would otherwise remain unreported. Compared to radiologists, the AI system flagged substantially more cases with potential signs of reduced bone mineral density, offering a scalable approach to support earlier identification of individuals who may benefit from further assessment.
PURPOSE: Osteoporosis often goes unrecognized in routine computed tomography (CT) scans performed for unrelated indications, leading to diagnosis only after a fragility frac

BACKGROUND: Surgical treatment of C2 fractures often involves fixation across C1-2, which restricts atlantoaxial motion. Because the C1-2 segment plays a major role in cervical mobility, motion-preserving strategies may be desirable in selected cases. We report a case of a complex C2 fracture treated with navigation-assisted motion-preserving fixation.
CASE DESCRIPTION: A 54-year-old man sustained multiple injuries in a motor vehicle collision, including a C2 fracture involving the vertebral body and posterior elements. He presented with neck pain but no neurological deficits. Computed tomogr

Tumor-related severe atlantoaxial dislocation is rare but may rapidly endanger the cervicomedullary junction when axis destruction, pathological fracture, and craniovertebral instability coexist. This single-case technical note describes a posterior-only occipital plate-assisted reduction technique for a selected patient with severe but reducible tumor-related atlantoaxial dislocation in whom the atlas and axis could not be used as reliable reduction anchors. Conventional reduction strategies for congenital, inflammatory, traumatic, or degenerative atlantoaxial dislocation commonly depend on t
