BACKGROUND: Atlantoaxial dislocation (AAD) is a rare but potentially life-threatening condition characterized by an abnormal alignment between the atlas and axis. Traumatic AAD are commonly associated with severe ligamentous injury and may result in spinal cord compression or neurological deficits. Posterior atlantoaxial dislocation with an intact odontoid process and without associated fracture is extremely uncommon and has rarely been reported in literature.
OBJECTIVE: To report a rare case of traumatic posterior atlantoaxial dislocation with an intact odontoid process and describe its mana

BACKGROUND: Concomitant maxillofacial, laryngeal and cervical spine (MLC) injuries in trauma remains a rare occurrence. It can result in mortality if not diagnosed and managed expeditiously in the field. An algorithm for efficient diagnosis and early management of patients with MLC injuries can be lifesaving. METHODS: We conducted a narrative literature review of PubMed and Cochrane databases and integrated the findings with a retrospective case series of nine consecutive patients with concomitant MLC injuries over a 23-year period at the National University Hospital, Singapore. The primary ob

BACKGROUND: Vertebral fractures are relatively common in cats, affecting the thoracic or lumbar spine more frequently than the cervical region. Patient history and clinical signs at presentation often guide the initial diagnostic suspicion; however, advanced imaging techniques are usually essential for establishing a definitive diagnosis. The management of these cases is often complex, and not all patients require surgical intervention. When surgery is indicated, the primary goal is to decompress the spinal cord and prevent further secondary injury.
CASE DESCRIPTION: A 3-year-old female domes

Solitary plasmacytoma is a hematologic malignancy in which about 50% of cases progress to multiple myeloma within 10 years. Local radiation therapy is effective for solitary plasmacytoma treatment, but there are cases of spinal cord paralysis due to osteolytic pathological fractures in the spine. Here, we report a 63-year-old male with spinal cord injury caused by pathologic fracture progression after radiotherapy. A 63-year-old male presented with neck pain and left hemiplegia. Imaging studies revealed a pathological fracture of the C5 vertebral body, and biopsy confirmed the diagnosis of sol

BACKGROUND: McCune–Albright syndrome (MAS) is a rare sporadic disorder caused by post-zygotic activating mutations in GNAS, classically characterized by café-au-lait macules, endocrine hyperfunction, and fibrous dysplasia of bone (FD). MAS presents challenges in anesthetic management due to airway difficulties, cervical spine deformities, and bone lesions. CASE PRESENTATION: A 40-year-old female with a history of growth hormone-secreting macroadenoma, for which she underwent transsphenoidal resection, presented with a pathological cervical spine fracture. Enhanced cervical MRI showed destructi

Fatal injuries caused by defective airbag inflators are increasingly recognized as a distinct forensic and medicolegal issue. We describe the case of a 51-year-old male driver who was transported by emergency medical services following a low-speed frontal collision with a palm tree and was declared dead on arrival at the hospital. Review of surveillance footage showed that the driver abruptly swerved in an attempt to avoid a motorcyclist immediately before impact. Emergency services were alerted by witnesses, and police investigation documented limited frontal damage localized to the front bum

BACKGROUND: Hangman's fractures are bilateral fractures of the C2 pars interarticularis produced during hyperextension injuries. The Levine-Edwards classification divides these fractures into 4 categories determined by injury stability. While stable fracture patterns are typically managed conservatively, prolonged traction required in unstable fractures may be superseded by surgery in its practicality. Surgical approaches can be divided into anterior and posterior: the anterior approach allows access to the disc and is used for anterior cervical discectomy and fusion (ACDF); the posterior appr

BACKGROUND AND PURPOSE: Compared to healthy controls, adult patients with Sickle Cell Disease (SCD) are anemic, and therefore have higher cardiac output and Cerebral Blood Flow (CBF) to maintain brain oxygenation. They also demonstrate comparatively more cognitive deficits due to either overt strokes or silent cerebral ischemia. However, there are few correlative studies between CBF and cognitive deficits, specifically processing speed in SCD. Such studies are important to develop biomarkers of central brain processing and ischemia for diagnosis, prognosis, and evaluating the effectiveness of

BACKGROUND: Guidelines for the management of acute spinal cord injury are based on a young age group suffering spinal fracture dislocations caused by high-impact trauma. The purpose of this study was to characterize our recent cervical spinal cord injury (cSCI) experience and compare it to this traditional demographic. We hypothesized that most cSCI patients now fall outside the scope of published guideline principles.
METHODS: Cross-sectional cohort study. A detailed review was performed of cSCI patients admitted to our level 1 trauma institution over a 12-month period identifying demographi

The unique anatomy and flexibility of the cervical spine (C-spine) pose a risk of injury. Trauma to the C-spine encompasses a diverse range of injuries, ranging from minor muscular strains to life-threatening fractures and dislocations associated with spinal cord lesions. This case report concerns a man in his seventies who presented to the emergency department immediately after a road traffic collision with only a minor laceration on his forehead. The patient was discharged after a thorough evaluation and returned the next day, reporting left-hand paresthesia, although the examination yielded

UNLABELLED: Unstable cervical spine (c-spine) fractures are of high concern in traumatic incidents because they may result in significant morbidity and mortality. This is a case of a 44-year-old male who presents to the Emergency Department (ED) with neck pain after recreational wrestling and was found to have an unstable C-spine fracture. His treatment course was complicated by multiple interrupted hospital stays due to leaving against medical advice (AMA) and subsequent returns to the emergency department. The patient received both CT and MRI imaging and ultimately underwent occiput to C3 fu

Background/Objectives: Equestrian activities can result in spine injuries. Most studies are from single centers, and none use a national database. It was the purpose of this study to describe the demographics, injury mechanisms, and types of equestrian-associated spinal injuries using a US national ED database. Methods: The National Electronic Injury Surveillance System database was queried for equestrian-related spine injuries from 2000-2023. ED disposition was categorized as discharged or not discharged. Statistical analyses accounted for the weighted, stratified nature of the data to obtain

In forensic practice, differences in hemorrhage volume among multiple injuries are often used to estimate the timing of trauma. We report a rare case in which multiple injuries sustained simultaneously exhibited marked discrepancies in hemorrhagic presentation. A man in his 30s was found deceased at the scene of a single-vehicle accident, in which his car had collided with a barrier. Forensic autopsy revealed complete bilateral transection of the common carotid arteries, cervical spinal cord transection, and cervical spine fractures. Additional injuries included a liver laceration and a lower

Trauma in geriatric patients (traditionally defined as adults aged 65 and older) is associated with high morbidity and mortality. Although older adults have lower average Injury Severity Scores (ISS) than younger patients, their mortality rates are higher. There are multiple hypotheses to explain these disparities; however, there is an incomplete consensus on how to best care for these patients in the prehospital setting. To address this issue, the National Association of Emergency Medical Services Physicians (NAEMSP) conducted a structured, rapid review of the literature to develop evidence-b

BACKGROUND: Motorbike collisions (MBC) pose significant public health challenges in low- to middle-income countries (LMICs), particularly in urban settings. This study examines the injury patterns of patients involved in MBCs at the Charlotte Maxeke Johannesburg Academic Hospital (CMJAH) trauma unit over a two-year period (May 2022 to April 2024).
METHODS: This was a retrospective review of all patients involved in MBCs.
RESULTS: A total of 134 patients were identified, predominantly male (97.1%), with a mean age of 32 years (SD ± 8.8). Most of these incidents were work-related, highlighting

Cervical spine fractures present considerable challenges in both diagnosis and treatment. With the increasing incidence of such injuries and the limitations of conventional diagnostic tools, there is a pressing demand for more precise and effective detection methods. This study proposes a robust Multi-class Classification model for Cervical Spine Fractures (MC-CSF) using Computed Tomography (CT) images to enable the precise identification of fracture types. The process of MC-CSF starts with preprocessing input images using an Enhanced Wiener Filtering (EWF) technique to minimize noise while re

BACKGROUND: Three-column unstable fractures represent unstable spinal injuries. Chance fractures, while traditionally occurring in younger populations due to high-energy trauma, are increasingly being observed in older patients following injury caused by low-energy mechanisms. Management of this type of fracture in nonagenarians presents unique challenges due to multiple comorbidities, frailty, and limited evidence-based guidelines for this vulnerable population.
OBSERVATIONS: The authors present the case of a 92-year-old male with baseline dementia who sustained a C5-6 three-column unstable
