PICC migration and perforation of internal jugular vein after neck dissection.

Peripherally inserted central catheters (PICCs) were introduced as an alternative to centrally inserted central catheters (CICCs) inserted into the jugular vein to provide central venous access. In head and neck reconstructive surgery, PICCs are preferred over CICCs because they do not obstruct surgical access, such as during neck dissection. Additionally, PICCs offer a safer option than peripheral access for the infusion of vesicants, irritants, hyperosmolar solutions, antibiotics, prolonged parenteral nutrition, and chemotherapy agents. We present a case of perforation of the internal jugula

[Trochanteric fixation nail advanced for trochanteric femoral fractures : Central cut-out as a new problem?].

INTRODUCTION: Trochanteric femoral fractures (TF) are mostly treated with intramedullary nail osteosynthesis. An inherent but rare complication of the system is the central cut-out of the femoral neck helical blade, i.e., the dislocation through the femoral head towards the acetabulum. After switching from proximal femoral nail antirotation (PFNA) to the trochanteric fixation nail advanced (TFNA) by the manufacturer, this rare type of complication has repeatedly been observed. MATERIAL AND METHOD: The retrospective single center case-control study was based on a database which includes all TF

What is the risk of dislocation after hip arthroplasty for patients with neuromuscular disease: a systematic review and meta-analysis.

PURPOSE: Recent research has demonstrated excellent functional outcomes for patients with neuromuscular (NM) disease undergoing hip arthroplasty (HA). However, significant concerns persist regarding a potentially elevated risk of postoperative instability and dislocation in this specific cohort. Therefore, our objective was to synthesize the existing evidence through a systematic review and meta-analysis to evaluate the risks of dislocation in NM patients after HA. The purpose of this study was to answer the following question using a systematic review and meta-analysis: for patients undergoin

Consequences of Periprosthetic Fracture After Hemiarthroplasty of the Hip.

Introduction Low-energy femoral neck fracture (FNF) in the elderly is a serious injury. For patients with multiple existing comorbidities and frailty, the risk of complications, poor functional outcomes, and premature death may be higher. The treatment aims for immediate weight-bearing, preferably by hemiarthroplasty (HA). Previous research has focused on prosthetic joint infection and dislocation after HA. In this study, we aimed to describe the possible surgical consequences of periprosthetic fracture after HA. Methods HA patients with a periprosthetic fracture occurring between 2009 and 201

Risk factors for instability and dislocation after reverse total shoulder arthroplasty: a systematic review.

BACKGROUND: The number of reverse total shoulder arthroplasties has increased due to demographic changes, leading to more revision surgeries. Postoperative instability or dislocation is a frequent and challenging complication that impairs clinical outcomes. This study aimed to identify risk factors for instability following reverse total shoulder arthroplasty and to evaluate the underlying mechanisms described by Abdelfattah et al. METHODS: A systematic literature review was conducted using PubMed and Google Scholar with MeSH terms and keywords including "shoulder replacement," "shoulder arthr

Ankle Sprains in the Workers' Compensation System: Trends, Costs, and Predictors of High-Expense Claims: A 9-Year Analysis.

OBJECTIVES: To evaluate trends, costs, and predictors of high-cost ankle sprain claims in the workers' compensation system. METHODS: Among 26,773 ankle injury claims (2016-2024), 18,991 sprains were identified and analyzed using descriptive and analytical multivariate methods. RESULTS: Sprains comprised about 70% of all ankle injuries, with increasing proportions among medical-only claims during the 9 years. Litigation, some comorbidities (diabetes and thyroid conditions), and coexisting injuries were strongly associated with high-cost claims (≥$50,000). Sprains without coexisting injuries r

Coronoid fractures : how do we best classify them and what is the 'true terrible triad'?

The 'terrible triad' injury of the elbow is defined as a dislocation of the elbow with concomitant fractures of the radial head and coronoid process. The term was based on reports of poor outcomes following this pattern of injury. However, the 'terrible triad' concept has meant that some very complex elbow fracture-dislocations are considered alongside more benign injuries, leading to problems with our understanding and interpretation of the literature. This annotation explores how our knowledge of elbow stability has evolved, and how the importance of the coronoid to this is becoming increasi

Factors associated with hip deformity in children with nonambulatory spastic cerebral palsy.

Hip deformities, including subluxation, dislocation, and coxa valga, are common in nonambulatory children with spastic cerebral palsy (CP), especially those classified as Gross Motor Function Classification System IV and V. These deformities impact daily activities and quality of life. This study aimed to identify factors associated with hip deformity and explore rehabilitation strategies to prevent complications. A cross-sectional study (August 2018-May 2020) included 46 children with spastic CP (Gross Motor Function Classification System IV-V, aged 2-10 years). Radiographic and physical meas

Horns, nails, and leaky kidneys: A rare case of congenital nephrotic syndrome.

Nail-patella syndrome (NPS) is an uncommon autosomal dominant condition marked by nail dysplasia, skeletal abnormalities, and variable renal manifestations, resulting from mutations in the LMX1B gene. We report a rare case of a 23-year-old male presenting with nephrotic-range proteinuria, characteristic skeletal manifestations of NPS, and a family history of renal failure. Genetic testing identified a previously unreported heterozygous missense variant in the homeodomain of LMX1B (c.791A>C; p.Gln264Pro), supporting its pathogenicity. The absence of patellar hypoplasia in our patient highlights

Surgical hip dislocation and sliding hip screw system for a femoral neck osteochondroma in multiple hereditary exostoses: a case report.

BACKGROUND: Multiple Hereditary Exostoses (MHE) is a rare autosomal-dominant disorder characterized by the development of multiple osteochondromas. While often asymptomatic, these lesions can cause pain and functional limitations due to mechanical impingement or malignant transformation. Osteochondromas of the femoral neck pose unique challenges due to their proximity to critical neurovascular and weight-bearing structures. CASE PRESENTATION: We present the case of a 43-year-old Italian woman with MHE who developed severe femoroacetabular impingement due to a large osteochondroma located at t

Does THA Offer Clinically Important Benefits Over Hemiarthroplasty for Femoral Neck Fracture in Patients Age 65 Years or Older? A Systematic Review and Meta-analysis.

BACKGROUND: Multiple meta-analyses have compared THA with hemiarthroplasty for displaced femoral neck fractures in patients over 65 years of age; however, clinical equipoise persists. We posit that this persistent uncertainty does not reflect a paucity of evidence but rather stems from critical methodological limitations in prior syntheses-including incomplete evidence searches, inadvertent inclusion of duplicate outcome data from overlapping patient cohorts, and inappropriate pooling of RCTs with nonrandomized or retrospective studies. An updated, rigorously conducted meta-analysis of RCTs-de

Comparison of patient demographics and implant complications in patients with multiple sclerosis undergoing total hip arthroplasty versus hemiarthroplasty for femoral neck fractures.

INTRODUCTION: Multiple sclerosis (MS) is a chronic neurological condition characterized by muscle spasticity, which may influence the outcomes of hip arthroplasty procedures, particularly in patients undergoing total hip arthroplasty (THA) or hemiarthroplasty for femoral neck fractures. It is unclear whether implant complications and revision rates differ between these two surgical approaches in patients with MS. The aim of this study was to compare patient demographics and 2-year implant complications in patients with MS undergoing THA versus hemiarthroplasty for femoral neck fractures. METH

Non-Mosaic Trisomy 9: Further Delineation of the Clinical Phenotype.

Non-mosaic trisomy 9 (NMTS9) is a rarely described chromosomal abnormality because most affected pregnancies result in first trimester spontaneous abortions, although survival to delivery is possible. In contrast, the phenotypic features of mosaic trisomy 9 have been well described in the literature as these individuals can survive to birth and beyond. Therefore, a better understanding of the phenotypic spectrum of NMTS9 is needed to provide appropriate perinatal counseling. The phenotype from three fetal and one neonatal case of NMTS9, as defined by chromosome analysis in multiple tissues, is

Treatment for Atlanto-occipital Dislocation, Vertical Atlanto-axial Dislocation, and Acute Subdural Hematoma Presenting with Out-of-hospital Cardiac Arrest: A Case Report.

A male patient in his 50s had a head-on collision while driving. Prehospital emergency services recorded pulseless electrical activity on an electrocardiogram, and chest compressions were initiated. Before hospital arrival, return of spontaneous circulation was achieved after 17-min resuscitation during transport. His Glasgow Coma Scale score was 6, with unequal-size pupils unresponsive to light stimuli. A head computed tomography scan revealed a left acute subdural hematoma with a marked midline shift, and computed tomography of the cervical spine showed that the atlanto-occipital and atlanto

Posterior Interosseous Nerve Injury From Chronic Monteggia Fracture Dislocation Requiring Nerve Reconstruction.

UNLABELLED: Monteggia fracture-dislocations are rare elbow injuries in the pediatric population. Multiple case reports have described posterior interosseous nerve (PIN) palsies in the setting of Monteggia fracture-dislocations, usually due to stretch neuropraxia. In this report of a Bado Type III Monteggia fracture-dislocations with delayed diagnosis of an entrapped PIN, we highlight the importance of timely diagnosis. Surgical reconstruction using a sural nerve autograft was required when the PIN was found to be entrapped in the radiocapitellar joint and encircling the radial neck. KEY CONCE

Temporary Cemented Spacer for Complex Radial Head Fractures: A Case Report.

BACKGROUND The management of comminuted and displaced radial head fractures (Mason III), particularly when associated with dislocation (Mason IV) or Essex-Lopresti injury, presents a complex surgical challenge. Treatment options include conservative management, surgical fixation, arthroplasty, or radial head resection; however, no consensus exists regarding the optimal approach for all injury types. Radial head resection is often preferred in complex cases with multiple fragments where reconstruction is unfeasible. While functional and radiological outcomes vary widely, there is a growing tren

Concurrent Salter-Harris type II distal tibia fracture with ipsilateral talus and calcaneus fractures in a paediatric patient .

Paediatric bones exhibit greater elasticity due to their distinct biomechanical properties, enabling them to absorb and dissipate mechanical forces more effectively than adult bones. This inherent flexibility provides increased resistance to fractures under comparable loading conditions. However, despite this resilience, high-energy trauma can still result in complex injury patterns. Fractures involving the physis are of particular concern, as they carry a significant risk of growth disturbances and long-term deformities. We present a rare case of a girl in early childhood (preschool) who susta
