Clinical features, microsurgical treatment, and outcome of vestibular schwannoma with brainstem compression.

BACKGROUND: Presenting symptoms, treatment considerations, and outcome are strongly related to the extension of vestibular schwannomas (VS). The aim of the current retrospective study was to analyze the clinical features, microsurgical treatment, and outcome of VS with brainstem compression. METHODS: Forty-nine patients presented with VS (Hannover grading scale T4a or T4b) in our department. A subgroup analysis was performed among patients without (T4a) and with (T4b) compression and dislocation of the fourth ventricle. RESULTS: Patients with type T4b VS presented significantly more often wi

Morphological variation of the anterior inferior iliac spine affects hip range of motion in flexion after rotational acetabular osteotomy.

PURPOSE: Reduced range of motion (ROM) in flexion and internal rotation are associated with impaired activities of daily living (ADL) after rotational acetabular osteotomy (RAO). This study focused on the morphological variation of the anterior inferior iliac spine in developmental dysplasia of the hip (DDH) and its impact on post-operative bony ROM after RAO. This study aimed to investigate the association between bony ROM after RAO and pre-operative morphological factors of the pelvis and femur, including a positional variation of the anterior inferior iliac spine. METHODS: Bony ROM in 52 p

Pit Bull attack causing limb threatening vascular trauma -A case series.

INTRODUCTION: Non-fatal human dog bites are commonplace amongst animal attacks on human beings and these present with mainly skin and soft tissue injuries. However, they can also present with life threatening head and neck injuries, massive soft tissue trauma, as well as combined orthopedic and vascular extremity injuries where a high possibility of limb loss exists. PRESENTATION OF CASES: We present two adult dog bite victims with multiple bites inflicted by large canines identified as Pit-Bull Terriers. They were presented with deep lacerations to the axillary area resulting in limb ischemi

The Dislocated Hip Hemiarthroplasty: Current Concepts of Etiological factors and Management.

BACKGROUND: Hip hemiarthroplasty (HA) following an intracapsular neck of femur fracture is an increasingly common procedure as a result of an ageing population. Patients are often frail and so morbidity and mortality figures are significant. As a result the National Institute for Health and Clinical Excellence (NICE) has formulated guidelines and a Best Practice Tariff (BPT) in an attempt to improve the care of such patients. Dislocation following HA is a potentially devastating complication with a reported incidence ranging from 1 to 15%. Multiple causative factors have been cited and studied

Causes and Mechanisms of Death in Fatal Water Buffalo Attacks.

Deaths due to buffalo attack have not been well described. A 72-year-old man was trampled by a water buffalo (Bubalus bubalis) while attempting to move the animal within an enclosed area at an abattoir. At autopsy, there were numerous injuries involving the chest, head, neck, and left upper arm. Blunt force injury had resulted in multiple rib fractures with a flail chest and fracture/dislocation of the mid-cervical vertebrae caused either by physical crushing by the animal against the walls of the enclosure or by stomping. There was also evidence of crush asphyxia with bilateral conjunctival h

'Split and splayed C3'-traumatic lateral C2-3 dislocation without neurological deficits: unique case and its management.

BACKGROUND: Traumatic C2-3 dislocation has been often described in the antero-posterior plane but is extremely rare in the lateral plane. Such dislocations have been described in thoraco-lumbar and C1-2. The need to study the imaging in multiple planes has been highlighted to plan proper realignment. METHODS: A young male presented with neck pain alone following a train accident. The imaging showed a fracture of C3 splaying it. The C2-3 showed lateral dislocation in the lateral plane with locked C2-3 facets on one side and C3-4 facets on the other. Through the posterior approach, the dislocat

Total hip replacement in complex acetabular fractures using a coned hemipelvic acetabular component.

Acetabular fractures in the elderly are a challenging clinical problem due to both the complex nature of operative intervention, osteoporotic bone and the multiple comorbidities of the patients involved. They are associated with poor functional outcomes and high levels of morbidity and mortality. Treatment currently ranges from conservative treatment, open reduction internal fixation and variations of total hip arthroplasty. We present the surgical technique and early results of the use of a coned hemipelvic acetabular component in the primary treatment of these complex injuries. Five patients

First and Second Metatarsophalangeal Joint Open Dislocations: A Case Report.

Dislocation of multiple metatarsophalangeal joint is an uncommon injury. The mechanism of injury is a high energy force distal to proximal with foot in hyperextension at the metatarsophalangeal (MTP) joint. The acute hyperextension of the toe at the moment of injury causes avulsion of the plantar part of the capsule from the junction of head and neck of the metatarsal. If the collateral ligaments remain intact, they maintain the locked fibrocartilaginous plate over the dorsum of the head of the metatarsal, making closed reduction impossible. We report a case of simultaneous 1st and 2nd MTP joi

Bilateral femoral neck fractures associated with complex pelvic ring injuries in a pediatric patient: a case report.

UNLABELLED: Femoral neck and pelvic fractures are rarely encountered in the pediatric population secondary to the resilient nature of the immature skeleton. Both fracture types usually result from high-energy blunt trauma including motor vehicle collisions, motor vehicle-pedestrian accidents, and falls from height. Considerable studies have been published on the natural history, management, and complications of pediatric pelvis and femoral neck fractures. However, few case reports have documented both fracture types in the same patient. Management of concomitant injuries presents unique challe

Novel Antibiotic-Loaded Cement Femoral Head Spacer for the Treatment of Advanced Pyogenic Arthritis in Adult Hip.

BACKGROUND: The aim of this study is to present a treatment for end-stage pyogenic arthritis of the hip using a novel cement spacer. This spacer caused less damage to the proximal femoral bone and made the conversion to total hip arthroplasty (THA) more convenient while maintaining the advantages of current cement spacers loaded with antibiotics. METHODS: We retrospectively reviewed 10 consecutive cases of hip pyogenic arthritis with joint destruction. These cases occurred from September 2009 to June 2015. In these cases, we used an antibiotic-loaded, cement spacer that was formed in the shap

Is hip dysplasia a common deformity in skeletally mature patients with hereditary multiple exostoses?

BACKGROUND: Various deformities appear in hereditary multiple exostoses (HMEs). Deformities around the knee or ankle joints are easy to detect in this disease because such deformities are visible in appearance. However, deformities in the hip joints of skeletally mature patients are not well understood because their tumors are invisible. METHODS: To understand deformities around the hip joint in HMEs, we investigated 36 hip joints in 19 skeletally mature patients (12 males, 7 females). The mean age at last X-ray imaging investigation was 29.2 years (14.5-66.5 years). We evaluated the lesions

Update on mandibular condylar fracture management.

PURPOSE OF REVIEW: Fractures of the mandibular condyle have provided a lasting source of controversy in the field of facial trauma. Concerns regarding facial nerve injury as well as reasonable functional outcomes with closed management led to a reluctance to treat with an open operative intervention. This article reviews how incorporating new technologies and surgical methods have changed the treatment paradigm. RECENT FINDINGS: Multiple large studies and meta-analyses continue to demonstrate superior outcomes for condylar fractures when managed surgically. Innovations, including endoscopic t
