Role of magnetic resonance imaging features in diagnosing and localization of disc rupture related to cervical spinal cord injury without radiographic abnormalities.

STUDY DESIGN: A retrospective study. OBJECTIVE: Traumatic cervical spinal cord injury (TSCI) is often associated with disc rupture. It was reported that high signal of disc and anterior longitudinal ligament (ALL) rupture on magnetic resonance imaging (MRI) were the typical signs of ruptured disc. However, for TSCI with no fracture or dislocation, there is still difficult to diagnose disc rupture. The purpose of this study was to investigate the diagnostic efficiency and localization method of different MRI features for cervical disc rupture in patient with TSCI but no any signs of fracture o

Numerical Investigation of Spinal Cord Injury After Flexion-Distraction Injuries at the Cervical Spine.

Flexion-distraction injuries frequently cause traumatic cervical spinal cord injury (SCI). Post-traumatic instability can cause aggravation of the secondary SCI during patient care. However, there is little information on how the pattern of disco-ligamentous injury affects the SCI severity and mechanism. This study objective was to analyze how posterior disco-ligamentous injuries affect spinal cord compression and stress and strain patterns in the spinal cord during post-traumatic flexion and extension. A cervical spine finite element model including the spinal cord was used and different comb

Contribution of injured posterior ligamentous complex and intervertebral disc on post-traumatic instability at the cervical spine.

Posterior ligamentous complex (PLC) and intervertebral disc (IVD) injuries are common cervical spine flexion-distraction injuries, but the residual stability following their disruption is misknown. The objective of this study was to evaluate the effect of PLC and IVD disruption on post-traumatic cervical spine stability under low flexion moment (2 Nm) using a finite element (FE) model of C2-T1. The PLC was removed first and a progressive disc rupture (one third, two thirds and complete rupture) was modeled to simulate IVD disruption at C2-C3, C4-C5 and C6-C7. At each step, a non-traumatic flex

Management of irreducible unilateral facet joint dislocations in subaxial cervical spine: two case reports and a review of the literature.

BACKGROUND: Skeletal and soft tissue damage are often associated with unilateral facet dislocations, which undoubtedly lead to instability of the spine and further increase difficulties in cervical reduction. This type of irreducible facet dislocation is usually accompanied with potential catastrophic consequences including neurological deficit and severe disability. Therefore, a consistent and evidence-based treatment plan is imperative. CASE PRESENTATION: The literature regarding the management of traumatic unilateral locked cervical facet dislocations was reviewed. Two patient cases (a 30-

Recurrent Neurological Deterioration after Conservative Treatment for Acute Traumatic Central Cord Syndrome without Bony Injury: Seventeen Operative Case Reports.

The mechanisms of late recurrent neurological deterioration after conservative treatment for acute traumatic central cord syndrome (ATCCS) remain unclear. Seventeen operative cases sustaining late recurrent neurological deterioration after conservative treatment for ATCCS were reviewed to investigate the mechanisms. The assessment of neurological status was based on International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI). Gender, age, cause of injury, results of image, conservative treatment and operative data, and neurological status at different time points we

Morphological changes in the human cervical intervertebral disc post trauma: response to fracture-type and degeneration grade over time.

PURPOSE: In the first 24 h post-intervertebral disc (IVD) trauma, up to 75 % cell death has been reported. In addition, burst fractures cause post-traumatic disc degeneration by elevated pro-apoptotic and pro-inflammatory gene transcription. Moreover, some patients have pre-trauma degenerative disc disease. The aim of the study was to assess histological changes and cell-death over a time period of up to 1 year caused by mechanical and structural factors. METHODS: 116 anterior portions of IVDs of the cervical spine were studied histologically by light microscopy and ultrastructurally by trans

Recurrent carpal tunnel syndrome--analysis of the impact of patient personality in altering functional outcome following a vascularised hypothenar fat pad flap surgery.

BACKGROUND: Vesicular monoamine transporter 2 (VMAT2) inhibitors can improve hyperkinetic movements, and are effective treatment options for chorea of Huntington disease (HD). Tetrabenazine was assessed for treating chorea in the TETRA-HD trial, and while efficacious, there are tolerability concerns possibly due to its pharmacokinetic properties. Deutetrabenazine is a novel VMAT2 inhibitor that contains deuterium, which extends active metabolite half-lives and minimizes drug concentration fluctuations. In the First-HD trial, deutetrabenazine was efficacious in treating chorea and was generally

Severe hyperflexion sprains of the lower cervical spine in adults.

Severe sprains of the cervical spine result from a traumatic rupture of the intervertebral disc and ligaments. Although rare, these lesions may lead to a significant kyphotic deformity if they are not surgically treated. The treatment of such a kyphotic deformity may consist of surgical fixation of the lesion through either an anterior or posterior approach. A retrospective study has been done examining 44 severe cervical sprains in 41 patients surgically treated through a posterior approach, using Roy-Camille plates. With an average followup of 29 months (range, 6-60 months), 73% of the patie

[Value of MRI in traumatic disco-ligament instability of the lower cervical spine].

The diagnostic accuracy of magnetic resonance imaging (MRI) for traumatic discoligamentous instabilities of the lower cervical spine was evaluated. MRI findings were correlated with the intraoperative findings. Fifteen patients were included in the study (11 males, 4 females, 33 years old on average). Indication for surgery was established with dynamic a.p. instability under fluoroscopy. In the operation, all patients showed complete traumatic rupture of the intervertebral disc. In contrast, MRI sequences revealed only eight mild and four severe disc lesions. Intraoperatively ruptures of the a

Soft tissue injuries associated with traumatic locked facets in the cervical spine.

The clinical, radiological and operative findings in 10 consecutive patients with cervical spine trauma presenting with locked facets (bilaterally in nine patients) are reported. The treatment was cervical traction until reduction of the locked facets was achieved, followed by anterior surgical decompression and ostheosynthesis. Intervertebral disc herniation was present in nine of the patients, and ruptured calcified posterior longitudinal ligament was present in the remaining patient. Four patients improved after treatment (none of these had clinically complete tetraplegia preoperatively), a

Magnetic resonance imaging in cervical spine trauma.

The ability of magnetic resonance imaging (MRI) to diagnose locked facets was examined in a series of six patients with traumatic cervical spine fractures. Plain films, computed tomography, and magnetic resonance scanning were done immediately following injury. Cord edema, contusions, and acute disc herniation were well visualized on magnetic resonance images. Magnetic resonance proved to be equally effective in diagnosing unilateral and bilateral locked facets, and demonstrated the disruption of the posterior longitudinal ligament (PLL) with clarity. The magnetic resonance diagnostic criteria

Magnetic resonance imaging documentation of coexistent traumatic locked facets of the cervical spine and disc herniation.

The coexistence of traumatic locked facets of the cervical spine and a herniated disc is not well described. The authors present a series of patients with traumatic locked facets who demonstrated a high incidence of associated disc herniation documented on magnetic resonance (MR) imaging. Thirteen patients with either unilateral (four cases) of bilateral (nine cases) locked facets of the cervical spine were analyzed retrospectively. Immediate closed reduction using traction and/or manipulation was attempted in the first nine cases treated and was successful in only three; however, the procedur

[Double vertebral injury in a calf (case report)].

A double spinal cord trauma of an eight weeks old calf is reported, born on pasture from a heifer as a premature birth at the beginning of month 9. Observations made were rupture of intervertebral disk at the crossing of cervical to thoracic vertebrae followed by syndesmosis or synchondrosis resp., as well as a comminuted fracture of the 1st lumbar vertebra including both the adjoining vertebrae, with succeeding reactive callus formation. It is concluded that this traumatization has occurred during parturition. Motoric or sensory disturbances were observed neither.

Intradiskal pressure and volume recording: evaluation of normal and abnormal cervical disks.

Rupture of the normal intervertebral disk by manual force is difficult. When the pressure was raised to 9000 mm Hg. in 33 normal intervertebral disks, the volume received did not exceed 0.4 cc. A contained degenerated intervertebral disk can be ruptured by manual force without a simultaneous pressure-controlling device. In cases of traumatic rupture of the annulus fibrosis and of the posterior longitudinal ligament, simultaneous intradiskal pressure and volume recording were more informative than either EMG or myelography. Simultaneous intradiskal pressure or volume recording is capable of det

[Results of cervical discography compared with surgical findings (author's transl)].

In 220 patients in whom ventral fusion according to Cloward was performed because of traumatic or degenerative discopathy, the pre-surgery finding of discography was compared with the finding established during the operation. It was found that pre-operative discography enables to diagnose damage of the nucleus pulposus and anulus fibrosus, as well as tearing of the posterior and anterior longitudinal ligament. It is emphasized that in degenerative damage, escape of the contrast medium in ventral direction merely represents backflow of the contrast medium along the puncture channel and rarely r
