Novel continuous and quantitative intraoperative facial nerve-monitoring system for temporal bone lesions.

OBJECTIVE: Intraoperative systems for monitoring facial nerve function, in which temporal electrical stimulation is applied to the facial nerve through electrodes, are used in many surgeries requiring facial nerve preservation; however, continuous stimulation or quantitative evaluation of facial nerve function is difficult with this approach. We examined the usefulness of a continuous and quantitative facial nerve-monitoring system for temporal bone lesions by using our experience to modify the existing methods used for cases involving vestibular schwannomas. STUDY DESIGN: Retrospective obser

Characterization of facial nerve outcomes following radiosurgery for vestibular schwannoma: a meta-analysis.

PURPOSE: Gamma Knife radiosurgery (GKRS) is a precise and efficacious treatment modality for vestibular schwannoma (VS) with favorable cranial nerve preservation rates. This study aims to better characterize facial nerve (FN) outcomes in VS after GKRS. METHODS: A query of six medical databases was conducted following PRISMA guidelines. Eligible studies exclusively reported VS managed with single-fraction GKRS and included House-Brackmann (HB) scale assessments prior to and following GKRS. Data was analyzed using random-effects modeling, and FN preservation was defined as HB I or II at last fo

Expression Patterns of SMAD1-8 in the Peripheral Facial Nerve Following Compressive Nerve Injury or Axotomy.

Facial nerve injury can lead to significant functional impairment, emotional impacts, and difficulties in social and economic activities. Although peripheral nerves have the potential for recovery, incomplete regeneration can pose challenges. Suppressor of Mothers Against Decapentaplegic Homolog (SMAD) proteins are crucial in the nerve-regeneration process. The study aimed to investigate the changes in SMAD protein expression involved in peripheral nerve regeneration following facial nerve injury induced by compression or axotomy in a pre-clinical study conducted on Sprague Dawley rats. Facial

Electrophysiological and Behavioral Programming Parameters in Patients with Facial Nerve Stimulation Post-Cochlear Implantation.

BACKGROUND: The aim of this study is to compare patients who develop facial nerve stimulation (FNS) after cochlear implantation (CI) and are managed with a triphasic stimulation pulse pattern (TPP) to those who do not develop FNS regarding the behavioral mapping parameters including the most comfortable loudness level (MCL) charge and amplitude, and the threshold level (THR), as well as the electrophysiological mapping parameters including phase duration (PD) and impedance level. METHODS: A retrospective chart review of the patients who developed FNS at any point after device activation and w

Intraoperative Real-Time Fluorescence Labeling of Degenerated Facial Nerves with Bevonescein.

BACKGROUND: Bevonescein (ALM-488), a nerve-targeted peptide-dye conjugate, enables intraoperative fluorescence of degenerated peripheral nerves because of its extracellular matrix-binding mechanism. In contrast, myelin-based nerve-targeting agents would not be expected to label degenerated nerves because of demyelination. The authors compare the intraoperative fluorescence of chronically degenerated murine facial nerves produced by bevonescein and myelin-binding dye oxazine-4 and discuss its utility in peripheral nerve reconstruction. METHODS: Sixteen wild-type mice underwent transection of t

Non-Recurrent Laryngeal Nerve: Surgical Risk of Injury in Comparison With the Normal Recurrent Laryngeal Nerve-A Systematic Review and Meta-Analysis.

INTRODUCTION: The non-recurrent laryngeal nerve (NRLN) is a known anatomical variation of the normal recurrent laryngeal nerve (RLN). Its prevalence is estimated to be < 1% and has a higher risk of iatrogenic injury. The risk during thyroid surgery has been reported variably in current literature, from 0% to 12%. This study aimed to systematically review the incidence of NRLN injury and present the results through meta-analysis. The incidence of injury to NRLN and RLN was compared statistically to determine its significance. METHODS: The PRISMA protocol was followed for an online search of pr

A life-saving lesson: Hybrid surgical management of a catastrophic vertebral artery and pleural injury from a misplaced hemodialysis catheter.

When a neck dialysis catheter is misplaced, it may damage important peripheral blood vessels or nerves, potentially causing severe concomitant injuries. To date, no reported cases or experiences of vertebral artery and pleural injuries caused by dialysis catheter implantation have been documented, nor have the details of emergency open surgery following interventional procedures been thoroughly described. This case report details the management of a 31-year-old male patient with a misplaced dialysis catheter that damaged the vertebral artery and pleura. Despite successful balloon occlusion to

Anatomical variations of the recurrent laryngeal nerve and postoperative outcomes in thyroid surgeries conducted at a teaching hospital in Ethiopia.

Thyroid surgery requires a thorough knowledge of the neck anatomy and its anatomical variations. This is of utmost importance since it is well known that variations of the recurrent laryngeal nerve are prone to iatrogenic injuries. Injury to the recurrent laryngeal nerve is one of the most severe complications of thyroid surgery. Surgeons must comprehensively understand the anatomy of the recurrent laryngeal nerve during thyroid operation. To assess the anatomical variations of recurrent laryngeal nerves, with inferior approach using inferior thyroid artery as a consistent anatomical landmark,

The Great Auricular Nerve as a Nerve Graft Donor: An Anatomical and Clinical Study of the Maximum Harvestable Length and Branches.

BACKGROUND: For immediate facial nerve reconstruction during head and neck tumor resection, a great auricular nerve (GAN) graft can be potentially harvested in the same surgical field during tumor resection. However, it is often avoided because a GAN graft is only approximately 5 cm long without any branches, and a sural nerve graft is recommended for larger defects. We investigated the length of the GAN that can be harvested in 18 patients, along with the evaluation of postoperative facial nerve palsy. METHODS: We retrospectively analyzed 18 cases of immediate facial nerve reconstruction usi

Ethacrynic acid regulates gentamicin ototoxicity via the blood-labyrinth barrier.

Gentamicin (GM), a widely used aminoglycoside antibiotic, has its clinical utility significantly limited by ototoxicity, which may be further exacerbated by co-administered drugs. This study systematically investigated the ototoxic mechanisms of GM combined with ethacrynic acid (EA) and the protective effects of N-acetylcysteine (NAC) using C57BL/6 J mice. Results revealed dose-dependent GM-induced ototoxicity. Intravenous administration caused more severe damage than intraperitoneal injection. Co-administration of EA synergistically potentiated GM toxicity. This exacerbated cochlear hair cell

In vivo Anatomy of the Facial Nerve: Implications for Partial Parotidectomy in Neck Lift Surgery.

BACKGROUND: Partial parotidectomy during neck lift surgery enhances posterior jawline definition by deepening the retromandibular groove. This limited parotidectomy, performed without formal facial nerve trunk identification (Bravo type I), streamlines the overall procedure, saving time in a typically lengthy face and neck lift. There is at present no in vivo data on anatomical parameters relevant to a Type I parotidectomy. OBJECTIVES: The purpose of the authors of this study is to determine the anatomy of the facial nerve in vivo relevant to partial parotidectomy surgery in neck lift procedu

Retrospective evaluation of Facial nerve monitoring to prevent nerve damage during robotic drilling in the largest series of patients undergoing the HEARO-procedure.

INTRODUCTION: Robot-assisted cochlear implantation surgery (RACIS) involves the drilling of a keyhole access to the inner ear for cochlear implant placement to treat patients with severe-to-profound sensorineural hearing loss. RACIS with the HEARO-procedure does not require the drilling of a mastoidectomy and posterior tympanotomy to pass through the facial recess. Instead, it directly drills through it guarding a safe distance from both the facial nerve and chorda tympani. Cochlear implantation surgery involves a well described risk for facial nerve injury when passing through the facial rece

[Strategies for facial nerve protection and management in temporal bone cholesteatoma surgery].

Temporal bone cholesteatoma is a common invasive lesion affecting the temporal bone, leading to a series of extracranial and intracranial complications, with facial nerve paralysis being one. The treatment of temporal bone cholesteatoma typically requires surgical resection to prevent its complications. The article discusses the preservation of the facial nerve during temporal bone cholesteatoma surgery, the management of intraoperative facial nerve injury, and the treatment of cholesteatoma-associated facial paralysis.
