Sympathetic nerves inhibit dental pulp stem cell differentiation and tertiary dentine formation via α1B-adrenergic receptor.

AIM: The dental pulp is extensively innervated. Our previous study showed that the α1B-adrenergic receptor ADRA1B is the primary sympathetic neurotransmitter receptor in human dental pulp stem cells (hDPSCs) and its expression was enhanced in inflamed pulp tissues. The sympathetic nerve system (SNS) negatively impacts the proliferation and migration capacities of hDPSCs via ADRA1B. In this study, we aimed to explore the role of the SNS in odontoblast differentiation of hDPSCs and tertiary dentine formation during dentine-pulp injury repair. METHODOLOGY: We investigated in vitro function of AD

Cancer-induced Nerve Injury Unveils a Sympathetic-to-Sensory Nerve Axis in Head and Neck Cancer.

Oral squamous cell carcinoma (OSCC) is one of the most painful cancers, with patients frequently reporting spontaneous, neuropathic-like pain. While sympathetic and sensory nerves have been individually implicated in cancer progression, whether and how these systems interact to drive pain and tumor growth has remained unclear. Here, we integrate prospective human data with reverse-translational mouse models to reveal that cancer-induced nerve injury unveils crosstalk between sympathetic postganglionic neurons and trigeminal sensory afferents in the tumor microenvironment. In patients, circulat

Heart rate variability in patients with incomplete spinal cord injury during a single session of paired associative stimulation.

Heart-rate variability (HRV) reflects the balance between sympathetic and parasympathetic nervous systems and is commonly used as a biomarker for the state of the autonomous nervous system (ANS). Acute pain can also be detected by HRV. We applied a single session of paired-associative stimulation (high-PAS) combining high-intensity transcranial magnetic stimulation (TMS) and high-frequency peripheral-nerve stimulation (PNS) on patients with incomplete cervical spinal-cord injury (SCI). High-PAS has shown promising results in improving motor function after incomplete SCI. The cardiovascular eff

Efficacy of Suprascapular Versus Interscalene Block for Post-Operative Pain Management in Shoulder Surgeries: A Narrative Review.

PURPOSE OF REVIEW: Analgesic techniques which provide sufficient post-operative shoulder pain relief while minimizing complications is crucial for helping patients achieve optimal recovery. The interscalene brachial plexus block (ISB) has long been considered the gold standard in terms of providing patients with adequate pain control following shoulder operations. However, ISB's expansive neural blockade poses several potential risks, including delayed sensorimotor rehabilitation to the upper extremity, partial blockade of the cervical sympathetic chain, and, most notably, diaphragmatic paresi

Resolution of Iatrogenic Spinal Accessory Neuropathy Following Delayed Surgical Intervention.

We present an interesting case of a 74-year-old female who presented with spinal accessory neuropathy following an ipsilateral right-sided carotid endarterectomy 6 months after the initial injury. Subsequent surgical exploration revealed a surgical clip that had been placed directly across the spinal accessory nerve. Clip removal and neurolysis was subsequently performed, leaving the nerve intact. After surgical intervention, she immediately demonstrated increased bilateral shoulder shrug motion and continues to improve her function with physical therapy. This case demonstrates a fascinating c

Anatomic variation of the spinal accessory innervation pattern of the sternocleidomastoid muscle: A cadaveric study.

Several studies have described the anatomical course of the spinal accessory nerve (SAN) and its associated landmarks. In particular, the SAN course through the sternocleidomastoid (SCM) has been reported as either intramuscular or branching without penetration of the SCM. However, it remains unclear what the occurrence of either anatomical course is, as various studies have reported inconsistent findings. For example, Lee SH et al. (South Korea) and Kierner SA et al. (Austria) reported that the SAN more commonly penetrates the SCM (54.1% and 63%, respectively) rather than branching in a clini

Diagnostic Neuromuscular Ultrasonography of Spinal Accessory Nerve Injury After Cosmetic Rhytidectomy.

Iatrogenic spinal accessory nerve (SAN) injuries can occur as complications of posterior triangle of the neck surgery, resulting in lateral scapular winging and a shoulder droop. Rarely has this complication been described after a cosmetic rhytidectomy (face-lift procedure). Here, we present two cases of iatrogenic SAN injuries that occurred after cosmetic rhytidectomies. The use of electrodiagnostic and ultrasound examinations provides invaluable information to assess peripheral nerve injuries. Electrodiagnostic testing can confirm the presence and severity of the injury, while ultrasound can

Very Late Spinal Accessory Nerve-to-Infraspinatus Nerve Transfer to Restore Glenohumeral External Rotation in Brachial Plexus Birth Injury.

BACKGROUND: Poor recovery of active glenohumeral external rotation (aGHER) after brachial plexus birth injury (BPBI) is common. Late spinal accessory nerve-to-infraspinatus motor branch (SAN-IS) transfer has been reported as effective. The authors investigated its efficacy in children older than 4 years with BPBI. METHODS: This case series involved children with no aGHER undergoing SAN-IS transfer. Indications included absent aGHER in shoulder abduction and a positive external rotation drop test. Subscapularis release was done for passive GHER less than 30 degrees in patients without glenohum

Automated assessment and detection of third molar and inferior alveolar nerve relations using UNet and transfer learning models.

Panoramic radiographs (PRs) are widely used in assessing the relationship between the mandibular third molar (MM3) and the inferior alveolar nerve (IAN). The relationship of MM3 and IAN is a critical consideration in oral and maxillofacial surgery due to the risk of nerve injury that can lead to anesthesia in the lower lip or chin. This study aimed to evaluate the performance of different transfer learning models in classifying these relations into four classes after detecting and segmenting the MM3 and the IAN using a new method with the UNet model. 714 PRs from more than 500 patients, repres

Activation of NR2A-Wnt-TLR2 Signaling Axis in Satellite Glial Cells of the Dorsal Root Ganglion Contributes to Neuropathic Pain Induced by Nerve Injury in Diabetic Mice.

Diabetic peripheral neuropathic pain (DPNP), a common diabetic mellitus (DM) complication, may result from the activation of satellite glial cells (SGCs) in the dorsal root ganglion (DRG), potentially enhancing peripheral sensitization. The N-methyl-D-aspartate receptor (NMDAR) subtype NR2A and Toll-like receptor (TLR)2 play key roles in neuroimmune interactions. However, their roles in SGCs of DRG and the precise mechanisms mediating peripheral sensitization in DPNP remain unclear. Here, we found that the expression of glial fibrillary acidic protein (GFAP), NR2A, and TLR2 in SGCs from DRG si

Apelin-13 attenuates optic nerve damage in glaucomatous mice by regulating glucose metabolism.

BACKGROUND: In patients with advanced glaucoma, lesions frequently extend beyond the eye and affect other organs. However, early distal consequences of elevated intraocular pressure (IOP) remain unclear. This study aimed to observe glucose uptake in the optic cortex during the early stages of elevated IOP and to investigate the mechanism by which Apelin13 exerts neuroprotective effects. METHODS: This study used a single anterior chamber injection of polystyrene microbeads and triblock copolymer hydrogel in 6- to 8-week-old male C57BL/6J mice and observed glucose uptake in the optic cortex dur

TNF-α Enhanced Activity of Sympathetic Neurons in Superior Cervical Ganglion to Promote Chronic Sleep Deprivation-Related Hyperalgesia.

The mechanisms underlying the association between sleep deprivation (SD) and hyperalgesia remain incompletely understood. In this study, the Modified Horizontal Platform Method was employed to induce chronic SD. Neuropathic pain was induced using chronic constriction injury of the sciatic nerve. Pain-like behaviors were assessed through measurements of mechanical allodynia and thermal hyperalgesia, while gait analysis was used to evaluate motor function. Immunofluorescence and western blot analyses were conducted to examine the expression of TNF-α, Iba-1, TH, neurons and c-Fos. Apoptosis was a

Sonographic Anatomy and Imaging of the Extracranial Component of the Hypoglossal Nerve (CNXII).

The hypoglossal nerve (HN) provides motor innervation to tongue muscles responsible for tongue movement, speech, mastication, swallowing, respiratory functions and management of oral secretions. Injury, compression, entrapment or lesions of the HN at any point along its path can result in HN palsy and subsequent dysphagia, dysarthria and tongue muscular atrophy. A combined imaging approach is required to investigate the HN and causes of HN palsy. Magnetic resonance imaging (MRI) and computed tomography (CT) imaging are used to investigate the intracranial HN and where it emerges in the upper n

NR2A-Wnt-TLR2 signaling axis mediates neuronal mitochondrial dysfunction contributing to neuropathic pain induced by nerve injury in diabetic mice.

Diabetic neuropathy, a prevalent complication of diabetes mellitus (DM), induces neuropathic pain (NP) through spinal dorsal horn (SDH) central sensitization, with mitochondrial dysfunction emerging as a pivotal contributor. While N-methyl-d-aspartate receptor (NMDAR) subunit NR2A and Toll-like receptor 2 (TLR2) are implicated in NP pathogenesis, their mechanistic interplay in DM-associated mitochondrial dysregulation remains undefined. Here, we systematically investigated their involvement in central sensitization using gain-of-function and loss-of-function strategies for NR2A and TLR2. First

Tumor-infiltrating nociceptor neurons promote immunosuppression.

Small extracellular vesicles (sEVs) released from tumors recruit nociceptor neurons to the tumor bed. Here, we found that ablating these neurons in mouse models of head and neck carcinoma and melanoma reduced the infiltration of myeloid-derived suppressor cells (MDSCs). Moreover, sEV-deficient tumors failed to develop in mice lacking nociceptor neurons. We investigated the interplay between tumor-infiltrating nociceptors and immune cells in head and neck squamous cell carcinoma (HNSCC) and melanoma. Upon exposure to cancer-derived sEVs, mouse dorsal root ganglion (DRG) neurons secreted increas
