C5 Nerve Palsy After Posterior Instrumentation and Decompression in Cervical Spine Surgery: A Review of the Literature.

C5 nerve palsy is a well-documented postoperative complication of cervical spine surgery, particularly following posterior decompression and fixation procedures. With incidence rates reported between 4% and 30%, it poses significant clinical challenges due to its impact on upper limb function and patient quality of life. Typically emerging within one to four weeks post-surgery, C5 palsy is marked by deltoid and biceps muscle weakness and sensory deficits in the C5 distribution. The pathogenesis is multifactorial, involving spinal cord shift, reperfusion injury, and foraminal stenosis, with sur

Comparative evaluation of posterior interosseous nerve strain in lateral approach to radial head fixation: a cadaveric study.

BACKGROUND: The posterior interosseous nerve (PIN) is at significant risk during proximal radius exposure. This study investigates the risk of PIN traction injury by comparing PIN strain between the Kocher and Kaplan approaches during simulated plate fixation of radial head fractures using upper limb cadaveric models. METHODS: Fourteen fresh-frozen upper extremity specimens were dissected with Kaplan (n = 7) and Kocher (n = 7) approaches, and the PIN was exposed at the anterior elbow with a separate incision. A Microminiature Displacement sensor (M-DVRT-9; LORD MicroStrain, Williston, VT) was

Anatomy, Imaging, and Clinical Significance of the Cervicothoracic (Stellate) Ganglion.

Background/Objectives: The stellate ganglion (SG), formed by the fusion of the inferior cervical and first thoracic sympathetic ganglia in approximately 80% of individuals, plays crucial roles in cardiac innervation, pain management, and autonomic regulation. This review examines the anatomical variations, histological structure, clinical applications, and therapeutic implications of the SG and stellate ganglion block (SGB), presenting original high-resolution magnetic resonance imaging (MRI) evidence of SG visualization, an underutilized approach in autonomic nervous system research. Methods:

Nerve stimulation and neuromodulation for painful nerves: a narrative review.

Nerve injury primarily leads to neuropathic pain but may also have overlapping elements of nociplastic pain or ongoing nociceptive pain. Electrical stimulation is particularly effective in the treatment of neuropathic pain and may be effective for nociplastic and nociceptive pain. While multiple mechanisms contribute to the analgesic effect of electrical stimulation, the most widely accepted theory for the predominant effect is that of Melzack and Wall's gate control theory. According to this theory, non-painful sensory input carried by low-threshold large-diameter Aβ fibres disrupt the transm

CX3CR1 fate mapping in vivo distinguishes cochlear resident and recruited macrophages after acoustic trauma.

INTRODUCTION: Cochlear injury activates the resident macrophages (RM) and recruits the blood-circulating monocytes and monocyte-derived macrophages (Mo/Mo-M), but their specific functions in the injured cochlea are unknown. It is well-established that the chemokine fractalkine receptor (CX3CR1), expressed by cochlear macrophages, influences the density of those macrophages and promotes synaptic repair and spiral ganglion neuron (SGN) survival in the injured cochlea. As CX3CR1 is expressed on both RM and Mo/Mo-M, it remains unclear if CX3CR1- expressing RM and Mo/Mo-M are distinct and different

Brown-Sequard syndrome caused by posterior full-endoscopic cervical discectomy: A case report.

BACKGROUND: Posterior full-endoscopic cervical discectomy (PFECD) is an effective and safe technique for cervical radiculopathy. The primary complications of PFECD include temporary nerve root paralysis and dural rupture, while spinal cord damage is exceedingly rare. This study describes a rare case of Brown-Sequard syndrome (BSS) occurring following PFECD and investigates its potential etiologies and pathomechanisms associated with this procedure. METHODS: Notes and images were reviewed and the relevant literature was analyzed. RESULTS: A 50-year-old woman underwent PFECD for cervical radic

Safe distances for transforaminal posterior lumbar interbody fusion under one-hole split endoscopy: three-dimensional reconstruction measurement of 1-degree degenerative lumbar spondylolisthesis at the L4/5 segment.

BACKGROUND: One-hole split endoscopy (OSE) is a novel endoscopic technique that offers some advantages in spinal surgery. However, without a clear understanding of the safe zone for OSE, surgeons risk injuring nerve roots during the procedure. This study aimed to measure the safe distances among critical bone markers, the intervertebral space and nerve roots between 1-degree degenerative lumbar spondylolisthesis (DLS) and non-DLS at the L4/5 segment in patients via three-dimensional reconstruction and to compare the differences in relevant safety distances between the two groups. These finding

A multifaceted presentation of acute neck pain with thumb weakness: A case report.

BACKGROUND: Cervical radiculopathy often presents with neck and upper limb pain, sensory changes, and motor deficits, affecting 0.832 to 1.79 per 1,000 person-years. Concurrent musculoskeletal conditions, such as peripheral neuropathies and tendon injuries, can complicate diagnosis due to overlapping symptoms. This review highlights the diagnostic challenges through a case report of a 60-year-old female with left neck pain and upper limb symptoms indicative of cervical radicular arm pain and acute left thumb weakness. CASE PRESENTATION: The patient had a six-week history of left-sided neck, p

Nerve injuries in cervical spine surgery via anterior approach: a comprehensive review.

Anterior cervical spine surgery is widely performed to manage degenerative, traumatic, and compressive spinal conditions. Despite its effectiveness, iatrogenic nerve injuries remain a significant concern due to the complex anatomy of the cervical region. This review offers a focused narrative on nerve injuries associated with anterior cervical approaches, including recurrent and superior laryngeal nerve palsy, C5 nerve root palsy, and hypoglossal nerve injury. Less common complications such as Parsonage-Turner syndrome (PTS), C8-T1 radiculopathy, and Horner's syndrome are also addressed. Each

Bilateral and Multilevel Vertebral Artery Loop Formation Causing Cervical Radiculopathy: A Case Report and Literature Review.

We present a case of a 60-year-old man with bilateral and multilevel vertebral artery loop formation (VALF) presenting with chronic cervicobrachialgia and dizziness. Magnetic resonance imaging (MRI) revealed bilateral vertebral artery loops at C4-5 compressing the right C5 nerve root, with computed tomography angiography (CTA) identifying an additional loop at the left V1 segment. The patient's symptoms resolved completely with conservative management over a six-month follow-up period. A literature review comparing similar cases over the past two decades reveals successful conservative and sur

Anatomic Relationship Between the Greater Occipital Nerve and the Axis: Is It Possible to Safely Insert a Percutaneous C2 Screw Without Causing Occipital Neuralgia?

BACKGROUND AND OBJECTIVES: Harms' technique is a widely used method for atlantoaxial stabilization. In recent years, minimally invasive surgery (MIS) using various robotic systems for percutaneous C1 to C2 screw insertion has started to be used. However, MIS raises concerns about the precision required to avoid injury to vascular and neural structures. The greater occipital nerve (GON) primarily arises from the C2 spinal root, located between the posterior arch of the C1 vertebra and the lamina of the C2 vertebra. The first bend of the GON could potentially overlay the lateral aspect of the C2

Exploring the biomechanical mechanisms of cervical rotation manipulation in different lateral bending positions: a finite element analysis.

OBJECTIVE: To investigate the biomechanical mechanism of cervical rotation manipulation (CRM) in different lateral bending positions. METHODS: A 27-year-old Asian male patient with cervical spondylotic radiculopathy due to left posterior cervical disc protrusion and compression of nerve roots was recruited. The CT scan data of his skull and cervical spine were extracted to construct finite element models of the skull and whole cervical spine. After model verification, the key parameters of CRM were loaded into the model, and the effects of CRM on the structure of cervical spine were analyzed.

The Great Auricular Point: A Landmark for Safe Sternocleidomastoid Myotomy in Selective Cervical Denervation for Cervical Dystonia.

BACKGROUND AND OBJECTIVES: Cervical dystonia is the most common focal dystonia, characterized by involuntary muscle contractions leading to abnormal postures and significant discomfort. Although neuromuscular blockade with botulinum toxin and other agents is the primary treatment, selective cervical denervation, which includes sternocleidomastoid (SCM) and posterior cervical muscle denervation, is a surgical option for refractory cases. The aim of this study was to evaluate the great auricular point (GAP) as a reliable intraoperative landmark for selective cervical denervation, optimizing spin
