Postoperative C5 Palsy Following Posterior Cervical Decompression and Fusion: A Case Report.

INTRODUCTION: Posterior cervical decompression and fusion (PCDF) is a well-established surgical procedure for the management of multilevel cervical spinal cord compression. Although generally safe, postoperative C5 palsy is a recognized complication that can significantly affect functional recovery. CASE REPORT: We report the case of a patient with multilevel cervical compressive myelopathy who underwent posterior cervical decompression and fusion. The immediate postoperative period was complicated by new-onset weakness of shoulder abduction and elbow flexion, consistent with C5 palsy. Postop

Cervical Spine Pathology in the Context of Intimate Partner and Interpersonal Violence: A Case Series and Focused Narrative Review.

Intimate partner violence (IPV), interpersonal violence, and caregiver abuse are common, frequently under-disclosed contexts for neurotrauma and delayed injury recognition. Relative to traumatic brain injury and hypoxic/anoxic injury after nonfatal strangulation, IPV-associated spinal injury is less well characterized, and existing spine literature has largely emphasized thoracolumbar compression-type fractures rather than cervical spine injuries. We present three neurosurgical cases of cervical spine pathology with cord or nerve root compromise occurring in the context of disclosed IPV or sus

Direct spinal cord injury during cervical epidural and nerve root blocks: A case series and pathophysiological reevaluation.

Cervical epidural and nerve root blocks effectively manage radicular pain but pose risks of severe neurologic complications, including acute myelopathy. Although vascular embolization of particulate steroids is often implicated in these events, direct mechanical spinal cord injury remains an underrecognized etiology. Identifying the true pathophysiologic mechanism is essential to prevent permanent neurological deficits. We retrospectively reviewed four patients who developed acute spinal cord injuries immediately following cervical nerve blocks or targeted acupuncture. We analyzed clinical pre

Ultrasound-Guided Selective Nerve Root Block for Surgical Planning in Multilevel Cervical Disc Disease: A Technical Report.

Selecting the appropriate surgical level in patients with multilevel cervical disc disease remains challenging when relying solely on clinical examination and magnetic resonance imaging (MRI) findings. Fluoroscopy-guided selective nerve root blocks (SNRBs) carry risks of vascular injury from inadvertent intra-arterial injection. This technical report describes the technical aspects of ultrasound-guided SNRB for preoperative surgical-level localization and provides a reproducible protocol for clinical implementation. The procedure utilizes a high-frequency (12 MHz) linear transducer with color/

A novel mouse model simulating C5 palsy after cervical surgery.

BACKGROUND: The incidence of C5 palsy after cervical surgery (C5P) was high (>5%), although its pathogenesis is still not clear. Although most patients (>95%) could have apparent enhancement, there are still some patients who cannot achieve satisfactory functional recovery, and it seriously affects their quality of life. At present, there is no appropriate animal model that could be used to research new interventions for permanent C5P. METHODS: Following anatomical validation confirming precise exposure of the C5 ventral root, we established a novel mouse C5P model using a posterior intradura

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

The Neurophysiological Effects of Cervical Transcutaneous Spinal Cord Stimulation With and Without a High Frequency Carrier in Able-Bodied Adults.

BACKGROUND: Transcutaneous spinal cord stimulation (tSCS) is a promising avenue in spinal cord injury (SCI) rehabilitation; however, high currents are required to excite afferents in the spinal cord roots, which patients may not tolerate. Modulating tSCS pulses with a kHz carrier frequency (kHz-tSCS) may be used to reduce discomfort; however, the way that kHz-tSCS interacts with neural networks, compared to unmodulated pulses (conv-tSCS), is largely unknown. METHOD: Ten able-bodied participants received conv-tSCS, kHz-tSCS, and sham interventions for 20 min over the C7/T1 vertebrae. Charge de

The effect of the number of single door segments in the posterior cervical canal on the traction of the cervical 5 nerve root.

BACKGROUND: The posterior cervical canal plays a critical role in the protection and function of the cervical nerve roots, particularly the C5 nerve root. Variations in its anatomical structure, notably the number of single-door segments, may impact the traction forces experienced by the C5 nerve root during surgical interventions or pathological conditions. This study aims to quantify how the number of single-door segments affects traction forces on the C5 nerve root. METHODS: A biomechanical model was developed to simulate traction forces on the C5 nerve root based on varying anatomical con

C5 Nerve Palsy After Posterior Instrumentation and Decompression in Cervical Spine Surgery: A Review of the Literature.

C5 nerve palsy is a well-documented postoperative complication of cervical spine surgery, particularly following posterior decompression and fixation procedures. With incidence rates reported between 4% and 30%, it poses significant clinical challenges due to its impact on upper limb function and patient quality of life. Typically emerging within one to four weeks post-surgery, C5 palsy is marked by deltoid and biceps muscle weakness and sensory deficits in the C5 distribution. The pathogenesis is multifactorial, involving spinal cord shift, reperfusion injury, and foraminal stenosis, with sur
