Electrical stimulation over the human vertebral column: which neural elements are excited?

Supramaximal percutaneous electrical stimuli applied over the human cervical vertebral column produce maximal compound muscle action potentials (CMAPs) in abductor digiti minimi. It is important to know which neural elements are excited by these stimuli and experiments were performed to answer this question. With stimulating electrodes placed progressively lateral to the midline, submaximal CMAPs with the same latency are produced. With shocks over the cervical vertebrae in the midline, the threshold for excitation of arm muscles is much lower than for excitation of leg muscles. Comparison of

Computerised axial tomography in traction injuries of the brachial plexus.

Severe traction injuries may damage the brachial plexus at any level from the spinal cord to the axillary outlet. Investigation aims to determine the level of the injury for each of the nerves, trunks or cords, with particular reference to obtaining firm evidence of any intradural (pre-ganglionic) damage. We report the results of computerised axial tomography of the cervical spine in comparison with conventional myelography and with surgical exploration of the plexus. CT scanning with contrast enhancement greatly improves diagnostic accuracy, particularly at C5 and C6 root levels.

Recovery of nerve root function after complete quadriplegia from cervical spine fractures.

Thirty-five patients with complete myelopathies secondary to cervical spine fractures from C-4 to C-7 underwent spinal decompressions and fusions between 1975 and 1981. Twenty-five of these patients underwent simultaneous nerve root decompressions, 23 with an accompanying anterior decompression and fusion and 2 with an accompanying posterior fusion. Substantial recovery of nerve root function occurred in 15 of these patients. A posterior reduction and fusion without nerve root decompression was performed in each of the remaining 10 patients. None of these patients demonstrated a significant im

Short-latency somatosensory-evoked potentials from radial, median, ulnar, and peroneal nerve stimulation in the assessment of cervical spondylosis. Comparison with conventional electromyography.

A study of data on 30 patients with cervical spondylosis was carried out to determine whether short-latency somatosensory-evoked responses (SEPs) to median, ulnar, radial, and peroneal nerve stimulation provided additional information to that obtained by electromyography (EMG), late responses, and peripheral conduction studies. Peripheral studies, EMG results and SEPs were within normal limits in ten patients with pain, but without objective neurological deficit. By contrast, of ten patients who had objective signs of root compression, conventional EMG results were normal in nine, but abnormal

[Herniated cervical disk, with or without arthrosis--surgical management by simple discectomy or associated with spondylodesis?].

58 patients underwent surgery by an anterior approach for nerve root or spinal cord compression at one or more levels. The cause was a soft disc herniation, an osteophyte or the two combined. 44 underwent simple discectomy with 70.5% good or very good results. 14 also had interbody fusion by the method of Smith and Robinson, with good and very good results in 57.1%. In most cases the latter group had major cervical spondylosis and myelopathy. The bad results were due to severe neurologic lesions (myelopathy), duration of symptoms and associated illness. The anterior approach with use of the mi

[Thermographic studies with fluid crystals in peripheral nerve damage].

Common peripheral neurogenic lesion in various locations and of different origins were examined thermographically and the results were compared with those obtained by clinical and electromyographic methods. Lesions in the regions of the N. ulnaris and N. medianus can, except in the case of carpal tunnel syndrome, be identified without difficulty by thermography. The results obtained in the case of paresis of the radialis are non-specific, and thermographic examination of radicular lesions are also sometimes ambiguous. Damage in the plexus-brachialis and root region yield impressive results whi

Cervical cord trauma. The relationship between clinical syndromes and force of injury.

Several different syndromes, such as central cord, anterior cord, and root, have been described in injuries to the cervical spine. If stress analysis theory is used to analyze the applied forces that cause injury, the various syndromes appear to be interrelated and follow in an orderly progression that depend on the magnitude of the applied force. The modes of injury, either flexion or extension, theoretically produce similar injuries, dependent on the severity of the blow. These applied forces then cause the pathologic changes that have been described. This article applies commonly used princ

Somatosensory evoked potentials in cervical spondylosis. Correlation of median, ulnar and posterior tibial nerve responses with clinical and radiological findings.

Somatosensory evoked potentials (SEPs) following median, ulnar and tibial nerve stimulation were recorded from sites over the shoulders, neck and scalp in 34 patients with cervical spondylosis. Twenty control subjects were matched for sex and age. Detailed clinical and radiological data were assembled, with particular attention to the sensory modalities impaired and the locus and severity of cord compression. The patients were divided clinically into 4 groups: combined myelopathy and radiculopathy (6 cases), myelopathy alone (15), radiculopathy (6) and neck pain (7). Four cases are described i

Computed tomography of acute spinal epidural hematoma associated with cervical root avulsion.

A case of acute spinal epidural hematoma associated with post-traumatic cervical root avulsion is described. Surgical management of this condition depends to a large extent on its early recognition as well as accurate information about its site and extent. Computed tomography is the investigative procedure of choice to evaluate this entity.

Neurologic recovery following injuries to the cervical spinal cord and nerve roots.

From this literature, we have made the following conclusions: (1) Complete cord lesions do not recover cord functional motor control. (2) Complete lesions recover one nerve root level of function at the fracture site. (3) Partial lesions recover partially. (4) The less the injury, the greater the recovery. (5) Brown-Sequard lesions recover more than central cord syndromes, which recover more than anterior cord syndromes. (6) Reduction of dislocated facets facilitates nerve root recovery. (7) Better documentation of specific pathology and recovery rates are necessary to determine the surgical b

Cervical myelopathy due to atlantoaxial and subaxial subluxation in rheumatoid arthritis.

Four patients aged 41 to 73 years, who had had rheumatoid arthritis for eight to 25 years, had signs and symptoms of cervical myelopathy and radiculopathy due to either atlantoaxial dislocation with herniation of the odontoid through the foramen magnum, or subluxation of the middle to lower cervical vertebrae. Spastic paraparesis or quadriparesis, severe nuchal immobility and pain, and flaccid paresis of the upper limbs necessitated anterior medullary decompression and posterior cervical fusion. Postmortem examination disclosed old ischemic necrosis, atrophy, and gliosis in the low medulla and

Evaluation of cervical spine surgery by postoperative myelography.

After spontaneous remission of nerve root compression, a myelographic defect may persist. Similarly, myelopathy may remain nonprogressive for long periods despite appreciable myelographic deformity. Although operation may arrest or improve the symptoms of cervical disc lesions and spondylosis, the ultimate confirmation that entrapped neural elements have been relieved permanently can only be provided by postoperative myelography. Preoperative and postoperative myelography documents the significant improvement that can be achieved by using posterolateral and posterior approaches to the cervical

Myelopathy and radiculopathy due to cervical spondylosis: myelographic-CT correlations.

Forty patients with symptoms and signs of radicular disease or spinal cord involvement secondary to cervical spondylosis were studied with myelography (using nonionic water-soluble contrast medium) followed by computed tomographic (CT) myelography. In 17 patients CT was also performed before myelography. CT myelography adds useful information to the myelographic findings. Cord compression is better evaluated and osteophytes can be differentiated from disk herniation. Plain CT can demonstrate a herniated disk but with less accuracy than CT myelography. Cord and root compression are not seen dir

Dislocations at the cervicothoracic junction.

Dislocations of the cervicothoracic junction are frequently missed. Experience of this rare injury over 27 years at the Spinal Injuries Unit in Sheffield confirms that nearly two-thirds were not properly diagnosed on admission. Only two of the 14 dislocations studied were reduced by conservative methods and these were both associated with fractures of the posterior bony elements. Open reduction is necessary to replace pure dislocations at the C7-T1 level. It seems likely that the spinal cord lesion is not influenced by reduction of the displacement. The three patients who had an incomplete les

Digital nerve sensory potentials in lesions of cervical roots and brachial plexus.

Normal values for the amplitudes of digital sensory nerve action potentials were obtained from 190 control subjects, using an orthodromic surface-recording technique. Lower limits were calculated, taking into account laterality, age and sex; the ratios of amplitudes between different digits were also estimated. The results in control subjects were compared with those of 20 cases of trauma involving root avulsion or extraspinal injury, 38 patients with intraspinal disease, 6 patients with extraspinal disorder and 9 cases of neuralgic amyotrophy. The variability of the dermatomes is discussed an

The exaggerated supine oblique view of the cervical spine.

The technique of the 60 degree supine oblique view is described together with anatomic skeletal studies of this projection. The view is convenient for emergency room radiography and useful in other clinical radiography. The view separates widely the anterior and posterior portions of the vertebrae in a side to side projection. This makes for an elongated but detailed view of the articular processes, pedicles, and intervertebral foramina. In the cadaver specimen and clinically the view is shown to be useful in delineating fracture deformities of the articular process and visualizing constrictio

An unusual injury of the cervical spine caused by a fork lift.

A case of very unusual aetiology is reported. A severe traumatic injury of the cervical spine was associated with a suspended bilateral phrenic nerve paralysis, with limited sensory involvement, and weakness of the muscles of the right upper limb. The neurological status was otherwise normal, apart from slight weakness of the left upper limb. Artificial ventilation was necessary. Respiratory recovery was slow. The neural aetiology is discussed in relation to spinal cord and/or root involvement.

Neurological sequelae of cervical spine trauma.

It is estimated that there will be 5,000 new spinal cord injured patients each year. Prolonged survival has resulted from better understanding of the pathophysiology of cord damage, as well as from the advances in antibiotic and ventilatory therapy. Most well-motivated patients with lesions at or below C5 can attain a level of independence which obviates institutional care. Regional spinal injury centers have done much to increase the survival of and quality of life of cord injured patients. These realities make initial care of patients with acute traumatic lesions of the cervical spine of par

Surgical techniques of anterior decompression and fusion for spinal cord injuries.

Many patients who have static or only slowly improving neurologic deficits and significant compression of the spinal cord and nerve roots can benefit from anterior decompression. The improvement ranges from partial root recovery to very dramatic improvement in upper as well as lower extremities in the patient with quadriparesis. Intrinsic damage or contusion of the spinal cord cannot be reversed by decompression. Patients with motor sparing preoperatively attain a better functional improvement than those patients who have only slight distal sensory function initially. The same can be said of p
