Diagnosis of brachial plexus traction lesions by sensory nerve action potentials and somatosensory evoked potentials.

Forty-two patients with unilateral brachial plexus traction lesions were investigated by recording sensory nerve action potentials (SNAPs) from the lower arm and somatosensory evoked potentials (SEPs) from the clavicle, the cervical spine and the scalp overlying the contralateral somatosensory cortex, in response to electrical stimulation of peripheral nerves. The median and radial nerves were assumed to derive principally from the C6 and C7 roots, and the ulnar nerve from the C8 and T1 roots. Combination of SEP and SNAP findings suggested a location for the lesion (preganglionic, postganglion

[The effect of the sympathetic nervous system on cervical spondylosis (author's transl)].

A study on the effect of injury to the cervical spine, especially the so-called whiplash injury of the neck, has revealed that many patients who have sustained cervical trauma complain of, or later develop, vertigo similar to that encountered in Ménière's syndrome. Some authors offer clinical evidence to show that vertigo results from cervical nerve root irritation which is due to hypertrophic arthritis of the cervical spine and which responds to cervical traction. The etiology of this symptom-complex, however, remains unknown and, presenting a controversial problem, while some authors point o

Traumatic spondylolisthesis of the axis: Hangman's fracture.

Traumatic spondylolisthesis of the axis is classified either as Type 1, an undisplaced fracture involving the posterior elements only, or Type 2, a displaced fracture involving the posterior elements plus the anterior structures. This classification is the basis of a rational treatment program. Neurologically, not one in a series of 42 patients was made worse by the proposed program of treatment. All fractures healed per primam without operative intervention. There was no evidence of late instability. The incidence of spinal cord and nerve root involvement was low although the incidence of ass

La maladie de Grisel: a rare occurrence of "spontaneous" atlanto-axial subluxation after pharyngoplasty.

A review of the literature, with a short discussion of the possible aetiology, diagnosis and treatment of "la maladie de Grisel" is presented with a case report of atlanto-axial subluxation occurring in a child after pharyngoplasty. The occurrence of a "spontaneous" subluxation after pharyngoplasty has not been previously reported. The possible scale of neurological complications which may range from relatively minor nerve root compression to a potentially lethal major cord compression emphasises the importance of this condition especially as a minor trauma may precipitate a major disaster. Correction of spinal instability and recovery of neurologic loss following cervical vertebral body replacement. A case report.

While replacement of cervical vertebral bodies may be required when they are compromised by infection, tumor, or injury, or when they impinge on the neural elements, this procedure does not add immediate stability and may cause additional problems. In this case, displacement of a large fibular graft into the spinal canal appeared to cause additional spinal cord and nerve root injury. Despite a long delay, decompression and firmly fitted and wired anterior and posterior grafts seemed to enhance the neurologic recovery and permitted rapid mobilization. The diagnostic value of computerized tomogr

Ct in spinal trauma.

The radiological evaluation of patients with acute spinal trauma has always been a challenging problem. Multiple radiological procedures are often necessary for complete evaluation of the extent of spinal injury. CT provides an ideal modality whereby accurate assessment of displacement of bony fragments as well as associated spinal cord and nerve root injury can easily be performed, eliminating the need for difficult radiological procedures.

Examination of the cervical spine.

Examination of the cervical spine is discussed briefly in relation to history, neurological examination, vertebrobasilar insufficiency and x-rays. The general principles of examining cervical movements are presented. Details of the examination are discussed in more detail under the following headings: the upper cervical spine and its examination for cervical headache; the middle cervical spine with its examination being related to spondylosis/arthrosis; the middle and lower cervical spine and its examination related to wry neck, nerve root irritation or compression, and whiplash injury.

Cervical myeloradiculopathy caused by arthrotic hypertrophy of the posterior facets and laminae.

Five patients with typical cervical myeloradiculopathy caused by focal cervical spinal stenosis are presented. Dorsal intrusions into the spinal canal by hypertrophied apophyseal joints and thickened laminae resulted in cord and nerve root compression. Minor spondylotic changes were present in the floor of the spinal canal. Laminar decompression with formainotomy and facetectomy relieved the patients of their symptoms. An anterior approach should not be considered in the management of this disorder. Our findings of severe apophyseal arthrosis with lesser degrees of associated spondylosis are s

[Pantopaque-myelographic findings in brachial plexus root avulsion (author's transl)].

The myelograms of 7 patients with traction injury of the brachial plexus are compared with the operation findings. It appears that not only the roentgenological sign of the "traumatic meningocele" but also other myelographic abnormalities are to be considered as indications of a nerve root lesion. These changes are demonstrated. The myelogram of a 6-months-old child showing a nerve root avulsion due to birth trauma is represented. Myelographic examination of each patient with posttraumatic brachial palsy should be performed as soon as possible. A sufficient myelographic diagnosis of the nerve

[The narrow spinal canal - medicolegal aspects (author's transl)].

The constitutional anomayl of a narrow spinal canal was found in a neuroradiological department in 31 cases. The cervical stenosis can be defined in the following way: Inside diameter of cervical canal (anterior-posterior) in relation to diameter of vertebral body. In normal cases the quotient is over 1 - in pathological narrowing under 1. Clinical symptoms mainly appear from 45th year onwards, when reactive-degenerative changes increase the space problem. In wiplash injuries or other adequate cervical trauma 7 cases were seen and described with acute incomplete tetraplegia or/and multilocular

Upper limb involvement in cervical spondylosis.

Analysis of 200 cases reveals that the two neurological syndromes, brachial neuritis and myelopathy, associated with cervical spondylosis are distinct with relatively little overlap. While upper limb motor and sensory loss are doubtless due to nerve root compression in cases of "pure' brachial neuritis, they are more likely to be due to cord damage in cases with myelopathy (with spastic paraparesis of lower limbs). In either group of cases, neurological features in the upper limbs are not very helpful in localizing the level of significant intervertebral disc pathology. Contrast radiology (mye
