Anatomical variation in the cervical branch of the facial nerve: a systematic review.

BACKGROUND: There is a paucity of literature on the anatomical variations of the cervical branch of the facial nerve (CBFN). Poor awareness of its anatomy and variations risks intraoperative damage, misdiagnosis of facial nerve injury and undesirable functional and aesthetic outcomes for patients. PURPOSE: To review anatomical studies on the CBFN and synthesise data on its course, innervations and common variations. METHODS: A PRISMA-compliant systematic review of anatomical studies of the cervical branch of the facial nerve was conducted. PubMed, Embase, Scopus, Cochrane Library, Web of Sci

Morphometric cadaveric assessment of suprascapular and axillary nerve trajectories in relation to glenohumeral landmarks relevant to reverse total shoulder arthroplasty.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is increasingly used for complex shoulder pathology but carries a risk of iatrogenic nerve injury, particularly to the suprascapular (SSN) and axillary nerves (AXN). Detailed morphometric data on their relationship to glenoid landmarks remain limited. METHODS: Forty embalmed adult cadavers (60 shoulders) were dissected at PGIMER, Chandigarh, after ethics approval (PGI/IEC/2024/EIC000563). The SSN and AXN were exposed, and distances from nerves to glenoid "clock-face" positions and to osseous landmarks were measured with digital calipers.

Development and internal validation of a nomogram for predicting lymph node metastasis posterior to the right recurrent laryngeal nerve in papillary thyroid carcinoma: a single-center retrospective study.

BACKGROUND: The lymph nodes posterior to the right recurrent laryngeal nerve (LN-prRLN) lie deep in the right central compartment. Clearing them is technically demanding and adds to the risk of injury to the nerve and parathyroid glands, whereas occult metastases left behind may cause persistent or recurrent disease. Knowing in advance which patients are likely to have LN-prRLN metastasis would help tailor the extent of central neck dissection. METHODS: We retrospectively reviewed 320 patients with papillary thyroid carcinoma (PTC) treated at a single tertiary center between August 2021 and F

Cell-inspired dual-tandem catalytic nanoreactors drive targeted hearing protection.

Sensorineural hearing loss is driven by disruption of cochlear homeostasis, where dysregulated redox, ionic, and metabolic factors lead to hair cell degeneration and synaptic injury. Current strategies lack the capacity to restore the enzymatic clearance of reactive oxygen species or penetrate the blood-labyrinth barrier. Here, we report a cell-inspired dual-tandem catalytic nanoreactor (HSC@Se) integrating GPx-mimetic diselenide with superoxide dismutase and catalase to reconstruct the detoxification cascade. We show that the albumin scaffold stabilizes the nanoarchitecture and enables active

Protocol of a multi-centre randomized controlled trial to compare pericapsular nerve group block, fascia-iliaca compartment block and femoral nerve block for pain management in patients with a hip fracture in the emergency department (CPFF-ED).

BACKGROUND: Patients presenting in the emergency department (ED) with a hip fracture are often in significant pain. Opioids are frequently prescribed as part of standardized pain protocols, but are associated with many adverse events, especially in the elderly. Regional anaesthesia with a femoral nerve block (FNB), Fascia Iliaca compartment block (FICB) or Pericapsular nerve block (PENG) may provide a reasonable alternative to these patients. We designed a prospective multicentre study to compare the efficacy and safety of these blocks in the ED setting, which has been registered in the Centra

Sublytic pyroptosis blocks Schwann cell remyelination through caspase-1-mediated Tyro3 cleavage.

BACKGROUND: Inflammasomes canonically trigger pyroptotic cell death through GSDMD-mediated membrane rupture. This study investigates a distinct sublytic pyroptosis in Schwann cells following peripheral nerve injury, where inflammasome activation causes functional impairment without extensive cell death. METHODS: Using a mouse sciatic nerve transection-repair model combined with in vitro and in vivo analyses, we examined NLRP3 inflammasome activation and cell fate. Primary Schwann cell cultures and RSC96 cells were used for biochemical assays, while ultrastructural analyses were performed on sc

Perineural invasion: A neuroimmune axis mediating immunotherapy resistance across malignancies.

Perineural invasion has traditionally been regarded as a distinct anatomical route of tumor spread, following direct invasion, hematogenous dissemination, and lymphatic metastasis. However, accumulating evidence indicates that perineural invasion represents more than a passive conduit for cancer dissemination. Perineural invasion is frequently observed in aggressive malignancies, including head and neck squamous cell carcinoma, pancreatic ductal adenocarcinoma, gastric cancer, melanoma, prostate cancer and breast cancer, and is strongly associated with tumor progression, immune evasion, therap

Perioperative analgesia with combined Pericapsular Nerve Group (PENG) and lateral femoral cutaneous nerve blocks in pediatric femoral neck surgery: a two-case report.

BACKGROUND: The importance of regional anesthesia techniques is increasing as part of multimodal analgesia strategies for managing postoperative pain in pediatric hip and femur surgeries. CASE PRESENTATION: We aim to present two cases, aged 10 years and 18 months, scheduled for surgery due to femoral neck fractures. As part of multimodal pain management, ultrasound-guided PENG + LFCN blocks were performed in both cases. Postoperative pain was assessed using VAS and FLACC scales. CONCLUSIONS: Our observations in these two cases suggest that the combined administration of PENG and LFCN blocks

The anatomy of the cutaneous branches of the cervical plexus and its clinical significance: supraclavicular nerves. A systematic review.

BACKGROUND: Supraclavicular nerves (SCNs) emerge from the cervical plexus and provide sensation to the skin over and below the clavicle. Detailed knowledge of their course and variation is especially useful in surgeries of the clavicular and neck regions. Anatomical descriptions of the SCNs remain inconsistent across studies. This systematic review aims to provide a comprehensive, evidence-based analysis of SCN anatomy, including its course, branching pattern, and surgical relevance. MATERIALS AND METHODS: A systematic search was conducted across major electronic databases (PubMed, Scopus, Em

Bilayer nerve guidance conduits for continuous delivery of NGF@ZIF-8 nanoparticles for peripheral nerve injury repair.

Peripheral nerve injury remains a major clinical challenge due to limited regenerative capacity and insufficient therapeutic strategies. In this study, we developed biomimetic nerve guidance conduits composed of high-concentration gelatin methacryloyl (GelMA) hydrogel (GMH), providing mechanical support and structural integrity. The conduits lumen was filled with a softer, low-concentration GelMA hydrogel (GML) incorporating zeolitic imidazolate framework-8 (ZIF-8) nanoparticles encapsulating nerve growth factor (NGF), enabling sustained neurotrophic delivery. This bilayer design combines mech

Multifunctional Inverse Opal Nerve Guidance Conduits Loaded with Mesenchymal Stem Cells for Peripheral Nerve Repair.

Nerve guidance conduits (NGCs) hold considerable value in the field of nerve regeneration, yet current approaches display constraints regarding the diversity of loaded factors and their short sustained-release duration. Herein, a type of multifunctional inverse opal NGC integrating bone marrow mesenchymal stem cells (BMSCs) is presented for peripheral nerve repair. The generation of inverse opal scaffolds leverages the inversion of a monodisperse emulsion droplet template crafted by microfluidic technique. Ascribed to the biocompatibility and the cell-concentrating properties of the inverse op

Distal course and variability of the spinal accessory nerve: a cadaveric study with surgical implications.

PURPOSE: The spinal accessory nerve (SAN) is most vulnerable to injury along its distal extracranial course in the posterior cervical triangle. While proximal anatomy is well described, distal branching patterns and cervical plexus contributions to trapezius innervation remain poorly defined. This study aimed to characterise distal extracranial SAN branching and cervical plexus contributions and to assess whether proximal configurations predict distal trapezial innervation. METHODS: Ten sides from five adult cadavers were dissected from the emergence of the SAN at the posterior border of the

Non-iatrogenic spinal accessory nerve palsy due to lymph node compression.

Injury to the spinal accessory nerve (SAN) is most commonly of iatrogenic or traumatic origin. The incidence of SAN palsy resulting specifically from lymph node compression is difficult to ascertain, as such cases are typically reported in association with malignancies or following surgical procedures. We report the case of an adolescent who presented with clinical features consistent with SAN palsy, including spontaneous shoulder asymmetry, in the absence of prior trauma or surgical intervention to the cervical region. Surgical exploration revealed compression of the SAN by enlarged lymph nod

Cochlear Implant Insertion Trauma Is Associated With Spiral Ganglion Neuron "Dead Zones" in the Human.

HYPOTHESIS SIGNIFICANT: Cochlear implant (CI) insertion trauma, as evidenced by fracture of the osseous spiral lamina (OSL), is associated with localized "dead zones" and focal spiral ganglion neuron (SGN) loss. BACKGROUND: Hearing and structure preservation approaches to CI insertion aim to minimize trauma and preserve residual SGNs. In cases of significant insertion trauma, peripheral axons running through the bony OSL inherently become damaged if the OSL is fractured. The current investigation sought to determine if the relative location of OSL fracture was associated with focal areas of S

Astragaloside IV attenuates cisplatin-induced ototoxicity by preserving mitochondrial function and activating the Nrf2 signaling pathway.

BACKGROUND: Cisplatin-induced ototoxicity represents a major dose-limiting adverse effect of chemotherapy, leading to irreversible sensorineural hearing loss. Astragaloside IV (AS-IV), a bioactive saponin derived from Astragalus membranaceus, exhibits potent antioxidant and cytoprotective properties in various pathological settings. This study aimed to elucidate the protective effects and underlying mechanisms of AS-IV in cisplatin-induced cochlear injury. METHODS: In vitro, HEI-OC1 cells, cochlear basilar membrane explants, and spiral ganglion neurons were treated with cisplatin in the prese
