High-Resolution Ultrasonographic Morphometry of the Superficial and Deep Peroneal Nerves in Healthy Young Adults: Reference Values From an Indian Cohort.

Introduction Peripheral nerve ultrasonography is an important diagnostic modality for evaluating nerve pathologies. Cross-sectional area (CSA) and perimeter measurements are key parameters for diagnosis and clinical management. The deep peroneal nerve (DPN) and superficial peroneal nerve (SPN) are prone to entrapment and traumatic injuries, yet normative morphometric data for these nerves in healthy individuals are limited. Understanding their relationship with anthropometric parameters such as body mass index (BMI) may help refine diagnostic criteria and guide procedures, including nerve bloc

Update in facial reanimation: neurorrhaphy, interposition grafting, nerve transfers and bioengineered options.

PURPOSE OF REVIEW: The purpose of this review is to summarize recent advances in facial nerve repair and facial reanimation as it relates to neurorrhaphy techniques, bioengineered options and nerve transfers. RECENT FINDINGS: Direct neurorrhaphy is the gold standard for facial nerve repair when tension-free repair is achievable. Interposition grafting with autologous donor nerves facilitates repair when a tension-free repair is not feasible. Adjunctive bioengineered materials have been developed to enhance nerve repair, but high-quality clinical evidence supporting their use in facial nerve r

Current evidence and future perspectives in the treatment of head and neck vagal paragangliomas: a systematic review.

BACKGROUND: Vagal paragangliomas (VPGs) are rare neuroendocrine tumors, typically benign but with unpredictable growth and local invasiveness. Their management is debated, particularly in asymptomatic or elderly patients, due to the lack of clear predictive indicators for progression and treatment response. This systematic review aimed to assess the clinical features and treatment outcomes of VPGs. METHODS: We conducted a systematic review of the literature, screening studies at multiple levels. A total of 41 articles were included, comprising data from 501 patients. Treatments were categoriz

Ultrasound-guided Interscalene Block as a Safer Alternative to General Anesthesia in a Patient with Ankylosing Spondylitis Undergoing Proximal Humerus Fracture Fixation.

Ankylosing spondylitis (AS) presents significant anesthetic challenges due to restricted cervical spine mobility and an anticipated difficult airway. General anesthesia may increase perioperative risk, particularly in emergency settings. We report the anesthetic considerations of a 58-year-old male of short stature with suspected AS and severely restricted neck movements, posted for emergency fixation of a proximal humerus fracture. Considering the anticipated difficult airway, ultrasound-guided interscalene brachial plexus block was administered as the sole anesthetic technique. Ultrasound-gu

Different approaches of dissecting the recurrent laryngeal nerve during total thyroidectomy affect postoperative parathyroid function.

PURPOSE: This study aimed to compare the effects of proximal versus distal dissection of the recurrent laryngeal nerve (RLN) on the preservation of parathyroid function in patients undergoing total thyroidectomy for malignant thyroid tumors. METHODS: In this retrospective study, 209 patients were categorized into two groups based on the RLN exposure technique: a proximal approach (identification within 1 cm of the laryngeal entry point, n = 59) and a distal approach (identification within the tracheoesophageal groove, n = 150). Serum parathyroid hormone (PTH) and calcium levels were measured p

Brainstem Correlates of Tinnitus and Hyperacusis in Normal-Hearing Listeners: Distinct Neural Signatures Linked to Cochlear Nerve Degeneration.

OBJECTIVES: To determine how cochlear nerve degeneration (CND) relates to tinnitus and sound hypersensitivity in normal-hearing listeners, and whether these symptoms are associated with distinct patterns of central gain within the auditory brainstem. DESIGN: In a cohort of normal-hearing adults with normal audiometric thresholds (≤20 dB HL from 0.25 to 8 kHz) and mean extended high-frequency thresholds ≤20 dB HL, we recorded click-evoked electrocochleography (ECochG) and assessed sound sensitivity using the Hyperacusis Handicap Questionnaire (HHQ). To separate peripheral from central contribu

Biomimetic Multichannel 3D MXene Nanofiber Sponges for Peripheral Nerve Regeneration.

Nerve guidance conduits play a critical role in bridging nerve gaps and facilitating neural repair. Recent advances highlight that structural optimization and functional enhancement can further enhance the regenerative outcomes. Herein, inspired by the structural features of celery stalks and native nerves, we propose a biomimetic multichannel three-dimensional (3D) MXene nanofiber sponge scaffold for reconstructing large-gap peripheral nerve defects. The scaffold integrates MXene-loaded sacrificial alginate microfibers, a gas-foamed 3D porous nanofiber sponge, and a gelatin methacryloyl hydro

Novel Injectable Supports Long-Term Recurrent Laryngeal Nerve and Partial Peripheral Nerve Repair.

BACKGROUND: Unilateral vocal fold paralysis (VFP) from recurrent laryngeal nerve (RLN) injury currently has no injectable treatment options that promote targeted reinnervation. In a prior pilot study, Agrin, neuregulin-1 (NRG1), and acetylcholine (ACh) enhanced early (1-month) reinnervation after RLN injury. The aims of the current study were to (1) assess if these effects persist in a 3-month RLN injury model and (2) test whether Agrin/ACh/NRG1 promotes regeneration after sciatic nerve (SN) transection, with relevance to nerve repair beyond the larynx. METHODS: Adult C57BL/6 mice underwent u

Long-term hearing and facial nerve outcomes after hearing preservation surgery for vestibular schwannoma: a systematic review and meta-analysis.

Hearing preservation remains a key goal in the management of vestibular schwannoma. Here, we aimed to evaluate long-term hearing and facial nerve outcomes following hearing preservation surgery for VS. A systematic review and meta-analysis were conducted following PRISMA guidelines. PubMed and Embase were searched for studies reporting long-term hearing outcomes after VS resection via the retrosigmoid or middle fossa approach. Studies with at least a 5-year follow-up were included. The primary outcome was long-term serviceable hearing, defined as American Academy of Otolaryngology-Head and Nec

Diaphragmatic Paralysis Following Thoracic Outlet Decompression: An Evaluation of Secondary Phrenic Nerve Reconstruction for Salvage.

BACKGROUND: Thoracic outlet syndrome (TOS) involves the compression of neurovascular structures within the thoracic outlet, defined as a space at the base of the neck, superior to the first rib and posterior to the clavicle. Surgical management most commonly requires a supraclavicular approach to allow for anterior and middle scalene muscle resection, cervical and first rib resection, and brachial plexus decompression. Pectoralis minor tenotomy and vascular decompression may also be indicated. While these interventions may successfully relieve compression within the thoracic outlet, they carry

Time-dependent and side-specific c-Fos responses of the nucleus tractus solitarius and hypoglossal nucleus after unilateral nerve transection in mice.

How the medullary nucleus tractus solitarius (NTS) and hypoglossal nucleus (HN) respond when peripheral input or output is lost is unclear. In adult mice, we unilaterally transected the superior laryngeal (input) and hypoglossal (output) nerves and mapped c-Fos in the NTS and HN at 12, 24 and 72 h. Both nuclei showed a transient c-Fos increase relative to sham. In the HN, c-Fos shifted over time from the ipsilateral to the intact contralateral side, whereas the NTS responded bilaterally. Because the contralateral HN response arose in non-axotomized neurons, it reflects an activity-dependent co

Prior surgical resection is a predictor of facial nerve function decrement after radiosurgery for vestibular schwannomas.

PURPOSE: Gamma Knife radiosurgery (GKRS) can serve as adjunctive treatment for vestibular schwannomas (VS) for residual or recurrent tumor after surgical resection. However, it is poorly understood whether prior surgical resection influences the risk of facial nerve injury from GKRS. METHODS: An institutional database of GKRS treating adult VS was queried for patients with > 2 years of follow-up post-GKRS. Two cohorts were identified: patients with surgical resection prior to GKRS and patients receiving only GKRS. Facial nerve function was graded with the House-Brackmann (HB) scale pre-GKRS a

Facial nerve outcomes following vestibular schwannoma surgery: Multivariate analysis of possible prognostic factors.

OBJECTIVE: Preservation of facial nerve (FN) function is one of the most important goals addressed during surgery of vestibular schwannoma (VS). The main target of the present study is to assess all possible influencing factors on FN outcomes and detect independent predictors via a multivariate analysis. METHODS: This study included 77 patients, retrospectively reviewed, who underwent excision of unilateral VS via a translabyrinthine approach in a tertiary referral center. All clinical, radiological and intraoperative factors were evaluated regarding their effect on FN. All included cases had

The role of the blink reflex in predicting facial nerve outcomes following vestibular schwannoma surgery: a 4-year retrospective review.

OBJECTIVE: The objective of this study was to evaluate the prognostic value of intraoperative blink reflex (BR) monitoring in predicting postoperative facial nerve outcomes following vestibular schwannoma (VS) resection. Given the limitations of existing intraoperative neuromonitoring (IONM) techniques, this study aimed to determine whether BR loss correlates with worsened facial nerve function, and to assess its potential as an adjunctive tool for surgical decision-making. METHODS: The authors conducted a retrospective review of adult patients who underwent VS resection between January 2021

Temporal patterns of facial nerve palsy after microsurgical resection of vestibular schwannoma: immediate vs. delayed onset, management, and outcomes.

Facial nerve (FN) palsy is a significant complication of microsurgical resection of vestibular schwannoma (VS) and can profoundly impact patient quality of life. Patients exhibit two distinct trajectories of FN dysfunction: immediate facial nerve palsy (IFNP), occurring early in the postoperative period, and delayed facial nerve palsy (DFNP), characterized by new-onset weakness after an initially normal postoperative examination. In this study, we aimed to identify preoperative, intraoperative, and postoperative predictors of facial nerve trajectory to inform patient counseling and improve lon

Pain Interference in Young Adults With Brachial Plexus Birth Injuries.

PURPOSE: Clinical management for brachial plexus birth injuries (BPBI) is often focused on childhood, leaving a gap in understanding of how this condition impacts individuals into young adulthood. Recent studies suggest that pain persists beyond pediatric care, although the long-term trajectory of pain and function remains poorly understood. This study compares pain interference in young adults with BPBI to matched controls. Secondary objectives were to compare pain intensity between groups, examine the relationship between upper limb function and pain, and identify common areas of pain using

Foot Drop Caused by an Intraneural Ganglion Cyst: A Case Report.

Foot drop (FD) refers to a clinical picture characterized by the inability to lift the foot against gravity due to weakness of the dorsiflexor muscles. Patients with FD typically present with an abnormal gait marked by exaggerated flexion of the knee and hip joints, along with internal rotation of the foot, which may increase the risk of falls and injuries. Intraneural ganglion cysts are benign, fluid-filled lesions located within the epineurium of peripheral nerves. We report the case of a 22-year-old male who presented with right lower limb pain and numbness and later developed acute weaknes

The relative contribution of trunk movements to Modified Mallet Scores in children with brachial plexus birth injury.

BACKGROUND: Children with brachial plexus birth injury often rely on compensatory movements due to limited shoulder mobility. In clinical practice, the Modified Mallet Scale (MMS) is frequently used to assess shoulder movements. However, MMS scores can be influenced by trunk contributions, preventing an accurate evaluation of pure shoulder complex motion. This study aimed to quantify the relative contributions of the extremity and trunk during MMS movements using wearable inertial sensors. METHODS: The study included 24 children (7.20 ± 1.41 years; range 4-10 years), and three-dimensional kin

Sixteen years of cochlear implant surgery in cochleovestibular malformation and cochlear nerve deficiency: Insights from northern Malaysia.

INTRODUCTION: Congenital sensorineural hearing loss associated with cochleovestibular malformations (CVM) and/or cochlear nerve deficiency (CND) presents distinct surgical and audiological challenges. Patients with CVM face elevated risks of intraoperative cerebrospinal fluid (CSF) gushers and aberrant facial nerve courses, while those with CND have historically been viewed as poor candidates for cochlear implantation (CI) due to concerns regarding neural sufficiency. This study aims to bridge this gap by evaluating the surgical safety and audiological outcomes of CI in a focused cohort of pat
