PURPOSE: Spinal accessory nerve (SAN) dysfunction persists after nerve-preserving neck dissection. We characterized the longitudinal SAN conduction trajectory and its association with surgical extent and adjuvant therapy.
METHODS: This prospective study enrolled 43 patients with oral cavity squamous cell carcinoma undergoing neck dissection with SAN preservation - selective neck dissection (SND; n = 35) or modified radical neck dissection (MRND; n = 8). Compound motor action potential (CMAP) amplitude, motor latency, and a novel, exploratory Spinal Accessory Asymmetry Index (SAI) were assesse

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

PURPOSE: Radiation-induced optic neuropathy (RION) occurs in about 4% of extraocular tumors. Current predictive models reliably identify grade-4 RION when blindness is irreversible. We developed a sensitive multivariable Normal Tissue Complication Probability (NTCP) model integrating quantitative visual outcomes and dosiomics to predict earlier grade 2+ RION in a pencil beam scanning proton therapy cohort.
METHODS AND MATERIALS: This prospective study included patients with paraoptic head and neck cancer and patients with central nervous system tumor who underwent standardized ophthalmologic

INTRODUCTION: Hand motor control depends on intricate brain-spinal cord interactions that regulate muscle activity. Hand function can be disrupted by injury to the brain, spinal cord, and peripheral nerves leading to weakness and impaired coordination. Functional MRI (fMRI) can map motor-related neural activity and potentially characterize the mechanisms underlying hand weakness and diminished coordination. Although brain motor control has been extensively studied, spinal cord mechanisms remain less explored. Here we use simultaneous brain-spinal cord fMRI to map neural activity related to han

Chemotherapy-induced peripheral neurotoxicity (CIPN) is a severe dose-limiting adverse event of chemotherapy. Presently, the mechanism underlying the induction of CIPN remains unclear, and no effective treatment is available. In this study, through metabolomics analyses, we found that nab-paclitaxel therapy markedly increased serum serotonin [5-hydroxtryptamine (5-HT)] levels in both cancer patients and mice compared to the respective controls. Furthermore, nab-paclitaxel-treated enterochromaffin (EC) cells showed increased 5-HT synthesis, and serotonin-treated Schwann cells showed damage, as

There is a clinical need for an effective nerve guidance conduit to treat peripheral nerve injuries. Many studies have explored different materials and active cues to guide neural regeneration, with some success. However, none have demonstrated a comparable or better functional recovery than the clinical standard autograft. Autografts are often insufficient for reconstruction of an injury to long nerves such as the sciatic or brachial plexus. Synthetic nerve guidance conduits (NGCs) have been investigated for these injuries to guide axonal regeneration and lead to functional recovery. We have

Peripheral nerve injury is challenging to self-repair and often leads to severe functional impairment. Utilizing neural tissue engineering scaffolds to promote nerve regeneration presents a promising strategy. However, current nerve scaffolds generally lack the ability to accurately replicate the viscoelastic properties of native neural tissues. Here, aligned carbon nanotubes (ACNTs) were integrated onto the surface of gelatin methacryloyl (GelMA) with a 30% MA substitution degree (GelMA30), exhibiting viscoelastic properties closest to those of neural tissue, to fabricate GelMA30-ACNT (GM30-A

Perineural invasion (PNI) is a well-established factor of poor prognosis in multiple cancer types1, yet its mechanism remains unclear. Here we provide clinical and mechanistic insights into the role of PNI and cancer-induced nerve injury (CINI) in resistance to anti-PD-1 therapy. Our study demonstrates that PNI and CINI of tumour-associated nerves are associated with poor response to anti-PD-1 therapy among patients with cutaneous squamous cell carcinoma, melanoma and gastric cancer. Electron microscopy and electrical conduction analyses reveal that cancer cells degrade the nerve fibre myelin

Peripheral nerve injuries affect over one million individuals annually worldwide due to various causes such as trauma, metabolic disorders, and autoimmune diseases. While autologous nerve grafting remains the gold standard for treating large-gap nerve injuries, its limitations, including limited tissue availability, donor site morbidity, infection risk, and suboptimal functional recovery, have spurred interest in alternative approaches. Among these, allogeneic nerve grafting has emerged as a promising option, offering structural and functional advantages due to the native architecture of donor

Stem cell therapies are gaining attention as a potential strategy for promoting recovery in peripheral nerve injuries. These injuries, particularly in complex anatomical regions, often lead to long-term functional deficits. There is a pressing need to explore novel therapeutic approaches. Experimental studies have shown that various stem cell types possess regenerative capabilities. This review synthesizes existing experimental and clinical evidence, highlighting the types of stem cells investigated, their mechanisms of action, and reported functional outcomes. Among them, adipose-derived and

Oral squamous cell carcinoma (OSCC) is one of the most painful cancers, with patients frequently reporting spontaneous, neuropathic-like pain. While sympathetic and sensory nerves have been individually implicated in cancer progression, whether and how these systems interact to drive pain and tumor growth has remained unclear. Here, we integrate prospective human data with reverse-translational mouse models to reveal that cancer-induced nerve injury unveils crosstalk between sympathetic postganglionic neurons and trigeminal sensory afferents in the tumor microenvironment. In patients, circulat

We present an interesting case of a 74-year-old female who presented with spinal accessory neuropathy following an ipsilateral right-sided carotid endarterectomy 6 months after the initial injury. Subsequent surgical exploration revealed a surgical clip that had been placed directly across the spinal accessory nerve. Clip removal and neurolysis was subsequently performed, leaving the nerve intact. After surgical intervention, she immediately demonstrated increased bilateral shoulder shrug motion and continues to improve her function with physical therapy. This case demonstrates a fascinating c

Several studies have described the anatomical course of the spinal accessory nerve (SAN) and its associated landmarks. In particular, the SAN course through the sternocleidomastoid (SCM) has been reported as either intramuscular or branching without penetration of the SCM. However, it remains unclear what the occurrence of either anatomical course is, as various studies have reported inconsistent findings. For example, Lee SH et al. (South Korea) and Kierner SA et al. (Austria) reported that the SAN more commonly penetrates the SCM (54.1% and 63%, respectively) rather than branching in a clini

Iatrogenic spinal accessory nerve (SAN) injuries can occur as complications of posterior triangle of the neck surgery, resulting in lateral scapular winging and a shoulder droop. Rarely has this complication been described after a cosmetic rhytidectomy (face-lift procedure). Here, we present two cases of iatrogenic SAN injuries that occurred after cosmetic rhytidectomies. The use of electrodiagnostic and ultrasound examinations provides invaluable information to assess peripheral nerve injuries. Electrodiagnostic testing can confirm the presence and severity of the injury, while ultrasound can

BACKGROUND: Poor recovery of active glenohumeral external rotation (aGHER) after brachial plexus birth injury (BPBI) is common. Late spinal accessory nerve-to-infraspinatus motor branch (SAN-IS) transfer has been reported as effective. The authors investigated its efficacy in children older than 4 years with BPBI.
METHODS: This case series involved children with no aGHER undergoing SAN-IS transfer. Indications included absent aGHER in shoulder abduction and a positive external rotation drop test. Subscapularis release was done for passive GHER less than 30 degrees in patients without glenohum

Panoramic radiographs (PRs) are widely used in assessing the relationship between the mandibular third molar (MM3) and the inferior alveolar nerve (IAN). The relationship of MM3 and IAN is a critical consideration in oral and maxillofacial surgery due to the risk of nerve injury that can lead to anesthesia in the lower lip or chin. This study aimed to evaluate the performance of different transfer learning models in classifying these relations into four classes after detecting and segmenting the MM3 and the IAN using a new method with the UNet model. 714 PRs from more than 500 patients, repres
