Transmanubrial L-shaped mini-sternotomy for vascular repair in hemodynamically stable Zone I neck injuries.

ABSTRACT: Penetrating neck injuries in Zone I that involve thoracic outlet structures are rare but carry significant risks, including major airway compromise and massive hemorrhage. Exposure of vascular structures in the thoracic outlet is challenging and typically achieved through median sternotomy for right-sided injuries and left thoracotomy for left-sided injuries. These approaches are familiar to most trauma surgeons, but they may be overly invasive and unnecessary in hemodynamically stable patients. To date, less invasive alternative surgical approaches for Zone I neck injuries requiring

Iatrogenic vertebro-jugular fistula: Case report.

Internal jugular vein (IJV) cannulation is one of the most commonly used procedure in intensive care units and hemodialysis setting. With ultrasound-guided cannulations, the incidence of complications has significantly decreased. We report a patient who developed vertebro-jugular fistula post internal jugular vein cannulation and was successfully treated with a stent graft placed using an endovascular technique. In the literature, very few similar cases have been reported so far.

Continuous renal replacement therapy in extremely low birth weight infants: a series of 5 cases.

BACKGROUND: Continuous renal replacement therapy (CRRT) plays a pivotal role in delivering renal support to critical patients with acute kidney injury (AKI). However, administering CRRT to extremely low birth weight (ELBW) infants, defined as those with a birth weight under 1000 g, presents distinct challenges. We summarized our experiences of the ELBW infants who were treated with CRRT at a tertiary medical center in China, and evaluated its feasibility and safety in this group of patients. METHODS: ELBW infants were performed CRRT at the neonatal intensive care unit (NICU), the Third Xiangy

A Case Report of Dialysis Catheter Placement in an Anomalous Pulmonary Vein.

Background: Partial anomalous pulmonary venous connections are embryologic defects in which the normal connection between the pulmonary veins and left atrium is disrupted. These rare anomalies are often asymptomatic and identified incidentally. The most common variant is a connection between the left upper pulmonary veins and the left innominate vein. Although typically asymptomatic, these variants are important to be aware of, particularly when performing procedures involving the venous anatomy. Case Presentation: We present the case of a 52-year-old female with a previous history of colon ca

Penetrating Pellet Gun Injury to the Neck and Embolization to the Right Ventricle.

BACKGROUND: Air pellet rifles are marketed and available in the United States as toys despite their ability to cause significant injury. Over 360,000 children were seen in the emergency department for penetrating injuries involving nonpowder firearms between 1990 and 2016. CASE SUMMARY: We report a 10-year-old boy with a penetrating injury to the neck, resulting in embolization of the pellet to the right ventricle through the right internal jugular vein. The patient was successfully treated with removal of the pellet by sternotomy. DISCUSSION: Missile embolization through the venous system i

Different management methods for tunnel abscess in tunneled cuffed catheters with catheter-related bloodstream infections and central venous stenosis: two case reports.

BACKGROUND: Tunneled cuffed catheters (TCCs) are critical vascular access tools for hemodialysis patients, but they are associated with complications such as tunnel infections (TIs) and catheter-related bloodstream infections (CRBSIs). Current guidelines recommend removing infected TCCs, but this can exacerbate central venous stenosis (CVS) and complicate subsequent catheter reinsertion. This case series proposes a staged management protocol for complex TCC tunnel abscess infections complicated by CVS. CASE PRESENTATION: We present two cases of complex TCC tunnel abscess infections with concu

A Single-Incision Method for the Removal of Vagus Nerve Stimulators: A Single-Institution Retrospective Review.

Vagal nerve stimulators (VNSs) improve seizure control in up to half of the patients who have them implanted. In non-responding patients, VNS removal may be necessary. Removal is traditionally accomplished through two incisions. We present our experience removing VNSs through a single incision. Background/Objectives: To determine if VNS removal can be safely performed through a single incision. Methods: The medical records of 73 consecutive patients who underwent VNS removal at our institution from 2012 to 2024 were reviewed. Patients were divided into single-incision and two-incision treatmen

Microgravity as a contributing factor to venous thromboembolism development: Risks and prevention strategies.

Venous Thromboembolism (VTE) being the third most occurring cardiovascular disease in the world after Myocardial Infarction and Stroke is very fatal. Being a multi-factorial disease, several risk factors in the terrestrial condition plays crucial part in the disease topography. Once exposed to microgravity, the weightlessness influences several thrombotic parameters, thus making astronauts in-flight to develop higher blood viscosity and increased risk of VTE. An astronaut travelling to the International Space Station developed a blood clot in the left internal Jugular vein, according to a rece

Comparison of peripherally inserted central catheters and central venous catheters in burn patients: a retrospective cohort study.

BACKGROUND: Central venous access devices, including peripherally inserted central catheters (PICCs) and central venous catheters (CVCs), are common treatment measures in burn patients. However, how to choose between PICCs and CVCs remains unclear. This study compared the clinical outcomes of CVCs and PICCs in burn patients. METHODS: This retrospective cohort study was performed in a major burn center in southwest China with 1500 burn patients admitted per year. All the burn patients receiving PICCs or CVCs were included. Catheter-related venous thrombosis (CRT), catheter-related infection (C

Comparison of changes in arterial blood pressure and cardiac output during cardiogenic shock development in a porcine model.

BACKGROUND: Low systolic blood pressure (SBP) is a key criterion for diagnosing cardiogenic shock (CS) caused by a reduction in stroke volume and cardiac output (CO). The temporal interaction between changes in pressure and flow has not been well described in the development of CS. In a large animal model, we assessed the temporal relationships of SBP, CO, and blood flow in the carotid artery during induction of CS. METHODS: Fifteen adult Danish landrace pigs (median weight 71 kg) underwent CS induction by stepwise injection of polyvinyl alcohol microspheres into the left main coronary artery

A Rare Cause of Internal Jugular Vein Thrombosis: Deep Tissue Massage.

BACKGROUND: Upper extremity deep vein thrombosis accounts for a small percentage of all deep vein thrombosis cases, and internal jugular vein thrombosis is a rare subset within that category. Typically, internal jugular vein thrombosis is found in patients with predisposing risk factors such as infections, local trauma secondary to venous catheterization, intravenous drug use, and malignancy. CASE REPORT: We describe the case of a patient with no typical risk factors for thrombus formation who developed internal jugular vein thrombosis following a deep tissue massage with a focus on the neck

Fatal hemothorax following complicated VV-ECMO cannulation in an anticoagulated patient with severe COVID-19 pneumonia.

Venovenous extracorporeal membrane oxygenation (VV-ECMO) is a lifesaving procedure for patients with acute respiratory distress syndrome (ARDS) due to severe coronavirus disease (COVID-19) pneumonia. COVID-19 infection is a risk factor for the development of coagulopathy and anticoagulant treatment is recommended. Moreover, anticoagulant therapy is often used to prevent circulatory coagulopathy during ECMO therapy. A man on dual antiplatelet therapy was hospitalized with severe COVID-19 pneumonia and treated with standard therapy. Apixaban was administered for a suspected coagulopathy. His med

Feasibility of Veno-Venous Extracorporeal Membrane Oxygenation Cannulation via the Neck in a Patient With Spinal Cord Injury Wearing a Halo Vest: A Case Report.

To our knowledge, no previous reports have described the use of veno-venous extracorporeal membrane oxygenation (V-V ECMO) in patients wearing a halo vest. We present a case in which neck cannulation was safely performed through a modified approach. A 38-year-old woman with intractable epilepsy sustained a C5 anterior dislocation and high cervical spinal cord injury after a seizure-related fall. Conservative management was chosen, and sedation with mechanical ventilation was initiated while maintaining a halo vest. On day 20, she developed septic shock complicated by neurogenic shock, leading
