Mechanical complications of central vein catheterization for hyperalimentation typically become manifest shortly after insertion. We present a case report describing a delayed innominate vein perforation by a central venous catheter. Use of the adult internal jugular vein for central venous catheterization should be limited to situations in which a subclavian vein is unavailable. Ongoing watchfulness and a vigorous diagnostic and therapeutic approach to such complications are mandatory in the treatment of patients receiving total parenteral nutrition.

Three patients with disabling sequelae of venous obstruction in the upper extremity were treated surgically. In one, direct venous repair was used, whereas in the other two, the internal jugular vein was used to bypass the area of obstruction. Restoration of adequate venous return was associated with permanent symptomatic relief.

We present a case in which interosseous wiring of the mandible resulted in an A-V fistula and an injury to the mandibular ramus of the facial nerve. These two complications can serve as a reminder to all who desire to use interosseous wiring in the treatment of mandibular fractures.

Aantomical dissections in 9 human cadavers revealed the terminal pathway of the lymphatic system of the left ventricle to be constituted mainly by channels emptying into the right angulus venosus (junction of the internal jugular and subclavian veins) at the base of the right side of the neck. This observation has clinical implications because it has been shown that a sampling of cardiac lymph provides the best method for analyzing myocardial metabolic abnormalities and that drainage of cardiac lymph alleviates the myocardial changes produced by ischemic injury.

Central venous pressure (CVP) varies directly with circulating blood volume and vascular tone and inversely with right heart competency. Indications for central cannulation include cardiorespiratory arrest. The two general approaches to cannulation of central veins are peripheral and central. The physician's skill, patient's body habitus, clinical circumstances, age and thoracic deformity all influence the choice of technique. Three of the possible complications discussed are pneumothorax, arterial puncture and air embolus. Accurate measurement of CVP depends on the patient being supine, a pat

Jugular venography done to evaluate abnormalities at the base of the skull demonstrated three distinctly different patterns depending on whether there is occlusion, invasion, or growth with in the internal jugular vein. Improper technique results in a lack of intracranial dural sinus filling which may masquerade as venous occlusion. This problem is avoided by adequate neck compression along with proper volume and rate of delivery of contrast. Radiographically, an abnormal jugular vein at the base of the skull will show a concave defect in true occlusion, constriction or invasion of the vein by

A new method is described for the nonsurgical removal of foreign bodies from the right heart. By means of the percutaneous Seldinger Technique, endoscopic forceps were passed throught the internal jugular vein to remove a fragment of polyvinyl catheter from the right atrium of a patient who had had heart surgery. The procedure is atraumatic and can be performed on patients who are critically ill and those who recently have had surgery.

In order to avoid skin erosion and electrode infection in endocardial pacemakers placed through the external jugular vein, we direct the wires from their point of entry into the vessel to the pacing unit placed in the pectoral region by dissecting a retroclavicular tunnel. This can be done under general or local anesthesia, and so far we have not seen any injuries to the subclavian vein. In this way the entire pathway of the cables is deep enough so that they cannot be palpated through the skin, and the dangers of exposure through erosion are minimized.

This paper describes a new technique for the canulation of the internal jugular vein. A transsection of the neck at the level of C4 (upper border of the thyroid cartilage) (Figure 1) shows the relative position of the internal jugular vein, the carotid artery and the sternocleidomastoid muscle. The internal jugular vein at this level is more superficial than is usually imagined. A cutaneous point of entry placed at the level of the thyroid cartilage on the medial border of the sternocleidomastoid muscle would be directly above, at the zenith of the carotid artery. From that point, moving a nee

The arrangement of extraspinal sources of the spinal cord arterial supply in man is more complicated than previously described, especially with regard to the origin and branching of the aortic segmental arteries. The fact that other arteries in the neck than the vertebral artery, such as the costo-cervical trunk and the ascending cervical artery may contribute to the supply of the cervical cord is confirmed, and also the occurrence of two or more spinal branches from different sources entering the same intervertebral foramen. Frequent occurrence of two or more segmental arteries arising from a

Young male Sprague-Dawley rats fed a high cholesterol, thyroid-suppressive diet were subjected to drying injury of carotid artery endothelium; animals were sacrificed at various times up to 3 months after injury, and the vessels were examined by light, scanning, and transmission electron microscopy. The diet induced marked elevation of serum cholesterol mainly present in lipoproteins of density less than 1.063. The morphology and degree of intimal thickening in the injured carotids of such animals were compared with the changes found in control groups of normolipemic rats. In the control group

The authors present two cases of almost certain catheterization of the thoracic duct, by infusion catheters placed in the large veins at the base of the neck (once in the left internal jugular, once in the left subclavian). Their clinical manifestaztions are the existence of a rapidly progressive pleural effusion, apart from the abnormal course of the catheter. Removal of the catheter is sufficient to lead to the disappearance of the symptomatology, so that it is difficult to speak of an accident as such, but rather of an incident. These incidents could be avoided by systematic radiography fro

Indications for the occlusion of the extradural portions of the carotid and vertebral arteries with the Fogarty catheter are demonstrated in 5 cases of traumatic carotid-cavernous sinus fistulas, in 1 case of an extradural carotid aneurysm originating from the anterior portion of the carotid siphon, in 1 case of traumatic carotid-jugular vein fistula and vertebral artery aneurysm with a-v shunt at the level of the atlas, and in 2 cases of large tumours of the base of the skull extending into the cavernous sinus. The limitation of the method is shown in one case where the catheter could not be

Many problems arising during anaesthesia and intensive care therapy can be solved by an analysis of the cerebral venous blood. Retrograde venography of the internal jugular vein to the superior bulb was performed in 35 patients. The technique of catheterization, the radiological control of the position of the catheter, the anatomy of the cerebral venous system are described. The advantages of this technique over direct puncture of the superior bulb and the maximum time the catheter can be safely left in the vein are discussed.

The purpose of this paper is to bring together in a comprehensive, up-to-date, and relevant manner knowledge of the jugular bulb and sigmoid sinus. The gross and microscopic anatomy of the jugular bulb is discussed, and selected illustrations of temporal bone sections show the varying locations of the jugular bulb. Considerations in differential diagnosis are presented, and examples of surgical procedures are delineated wherein the jugular bulb was encountered, and the methods of management in these instances are detailed.
