Central venous access is an essential part of patient management in many clinical settings. Traditionally this has been achieved by a blind, external landmark guided technique which may not correlate exactly with the location of the vessel. A new method of interpositional venous graft in microvascular tissue transfer is described. The interpositional vein is transferred to the vessels of the recipient site primarily, forming an arteriovenous shunt. Later, when the microanastomosis to the flap is created, the shunt is divided into an arterial branch and a venous branch. This method reduces the ischemic time of the free flap and allows the use of interpositional grafts of optimal length.

In this experimental study, the model of bullet wounds on both hind legs of dog complicating shock was used. The characteristics of the transfusion by external jugular vein intubation were observed and compared in three groups of dogs: those came from plateau area (n = 30). The result showed that the concentration of heparin which was used to prevent coagulation in the silica gel tube in plateau area should be higher than that in plain area. A series of eight patients with large arteriovenous fistulae involving the external carotid artery and the jugular vein that were treated with the endovascular technique are reviewed. The origin of the fistula was congenital in six patients, spontaneous in one, and iatrogenic in one. The evaluation and management of potential arterial injuries in penetrating neck trauma are controversial. Routine surgical exploration or arteriography can be very expensive and time-consuming and can overburden available resources if used in all patients.
