Unusual case of death by grinder.

Suicides by power tools such as grinders are rare and present significant challenges in forensic investigation. Accurate differentiation between suicidal, homicidal, or accidental injuries is crucial, particularly when the circumstances surrounding the death are ambiguous. We report the case of a 64-year-old man found dead in his workshop. The body was discovered lying near a grinder, with a deep cervical wound. The scene revealed multiple blood droplets scattered across the room, along with two large blood pools located approximately one meter from the body and a stepladder securing the disco

Manipulation of the Lower Cranial Nerves and Superior Jugular Bulb During Subtotal Temporal Bone Resection for Advanced Temporal Bone Malignancies: Anatomical Considerations and Surgical Strategies.

INTRODUCTION: En bloc subtotal temporal bone resection (STBR) is challenging. En bloc resection aiming for negative margins is considered the most reliable treatment strategy for advanced squamous cell carcinoma of the external auditory canal. However, the literature indicates that STBR carries a high perioperative risk. This report retrospectively examines 5 cases of en bloc STBR, considering the manipulating of the lower cranial nerves and the superior jugular bulb (SJB) from a microsurgical anatomical perspective. METHODS: Retrospective observational study. This study includes 5 cases of S

Arteriovenous malformation with unique drainage through the emissary vein of the foramen ovale: illustrative case.

BACKGROUND: As part of the laterotrigeminal venous system (LTVS), the emissary vein of the foramen ovale (EVFO) is an underrecognized venous structure communicating between the cavernous sinus and pterygoid plexus. The sphenobasal sinus is an anatomical variation of the sphenoparietal sinus that drains directly into the EVFO. The authors present the case of a ruptured arteriovenous malformation (AVM) with a unique drainage pattern through the sphenobasal sinus and EVFO. OBSERVATIONS: A 9-year-old female initially presented with loss of consciousness and was subsequently found to have a ruptur

Establishment of an inflammation-related acute lung injury/acute respiratory distress syndrome rat model supported by venovenous extracorporeal membrane oxygenation.

BACKGROUND: The major concerns for patients who have acute lung injury (ALI) or acute respiratory distress syndrome (ARDS) associated with coronavirus disease 2019 or sepsis and undergone successful venovenous extracorporeal membrane oxygenation (VV ECMO) include a low survival rate and an uncontrollable inflammatory response. This study aimed to introduce an inflammation-related ALI/ARDS rat model supported by VV ECMO that is more suitable for clinical application to assess the immune response and thereby further improve survival after VV ECMO. METHODS: Rats were randomly divided into three

[Reconstruction of the superior vena cava in trauma and thrombosis in a woman with a mediastinal tumor (case report)].

Presented herein is a clinical case report of successful treatment of an 18-year-old female patient with an iatrogenic injury and thrombosis of the superior vena cava having occurred during mediastinal tumor biopsy. While undergoing videothoracoscopy and mediastinal tumor biopsy she developed profuse bleeding from the superior vena cava. The vein defect together with the surrounding tissues was sutured during thoracotomy. This was complicated by the development of thrombosis of the superior vena cava and marked edema of soft tissues of the face, neck and upper half of the trunk. External jugul

Challenges in Managing a Case of Penetrating Neck Injury Complicated with Vascular Injury and Unrevealed Retained Foreign Body.

Penetrating neck injury is an emergency requiring urgent radiological and surgical attention. Delay in treatment results in significant morbidity and mortality. While computed tomography angiography (CTA) is a commonly used diagnostic tool, it has limitations and may fail to detect high-density foreign bodies or active bleeding due to compression by blood clots. Here, we present a case of a 39-year-old male with a penetrating neck injury complicated by vascular injury secondary to unrevealed foreign body. Despite being haemodynamically stable and CTA suggesting vascular injury without active b

Compact Arterial Monitoring Device Use in Resuscitative Endovascular Balloon Occlusion of the Aorta (REBOA): A Simple Validation Study in Swine.

Introduction Hemorrhage is the leading cause of preventable death in trauma in both the military and civilian settings worldwide. Medical studies from Operation Enduring Freedom (OEF) and Operation Iraqi Freedom (OIF) informed change in military prehospital medicine by influencing widespread tourniquet distribution and training on their use to stop life-threatening extremity hemorrhage. In the military setting, there has been a significant reduction in preventable death due to extremity exsanguination since the widespread implementation of tourniquets within the Department of Defense. However,

Targeting LncRNA LLNLR-299G3.1 with antisense oligonucleotide inhibits malignancy of esophageal squamous cell carcinoma cells in vitro and in vivo.

Accumulating evidence has indicated that long non-coding RNAs (lncRNAs) play critical roles in the development and progression of cancers, including esophageal squamous cell carcinoma (ESCC). However, the mechanisms of lncRNAs in ESCC are still incompletely understood and therapeutic attempts for in vivo targeting cancer-associated lncRNA remain a challenge. By RNA-sequencing analysis, we identified that LLNLR-299G3.1 was a novel ESCC-associated lncRNA. LLNLR-299G3.1 was up-regulated in ESCC tissues and cells and promoted ESCC cell proliferation and invasion. Silencing of LLNLR-299G3.1 with AS

Trimmed central venous catheters do not increase endothelial injury in an ovine model.

INTRODUCTION: Central venous catheters (CVCs) are often trimmed during heart transplantation and pediatric cardiac surgery. However, the risk of endothelial injury caused by the cut tip of the CVC has not been evaluated. We hypothesized that there is no difference in the degree of endothelial injury associated with trimmed CVCs versus standard untrimmed CVCs. METHODS: In four adult male sheep, the left external jugular vein was exposed in three segments, one designated for an untouched control group, one for the trimmed CVC group, and one for the untrimmed CVC group. Trimmed and untrimmed CVC

Supraclavicular cephalic vein draining into the internal jugular vein via the external jugular vein.

PURPOSE: The aim of this study is to report rare anatomical variations of the cephalic vein (CV) in a 77-year-old Korean male cadaver. CASE REPORT: On the right upper arm, the CV located lateral to the deltopectoral groove passed anterior to the clavicle at the lateral one-fourth of the clavicle without anastomosis to the axillary vein. It was connected to the transverse cervical and suprascapular veins by two communicating branches in the middle of its course at the neck, and opened into the external jugular vein at its junction with the internal jugular veins. The suprascapular and anterior

A Modified Technique for Arteriovenous Fistula Construction in Rabbits.

Juxta-anastomotic stenosis is a challenging problem that often causes non-maturation and decreases the patency of an arteriovenous fistula (AVF). Injury to the veins and arteries during the operation and hemodynamic changes can lead to intimal hyperplasia, leading to juxta-anastomotic stenosis. To reduce injury to the veins and arteries during the operation, this study proposes a new modified no-touch technique (MNTT) for AVF construction that can decrease the rate of juxta-anastomotic stenosis and improve the AVF patency. To unravel the hemodynamic changes and mechanisms of the MNTT, this stu

Normothermic ex vivo heart perfusion with NLRP3 inflammasome inhibitor Mcc950 treatment improves cardiac function of circulatory death hearts after transplantation.

BACKGROUND: The utilization of donation after circulatory death (DCD) hearts can enlarge the donor pool. However, DCD hearts suffer from serious ischemia/reperfusion injury (IRI). Recent studies found that the activation of NLRP3 inflammasome could play a significant role in organ IRI. Mcc950, which is a novel inhibitor of the NLRP3 inflammasome, can be applied to treat various kinds of cardiovascular diseases. Therefore, we hypothesized that the treatment of mcc950 could protect DCD hearts preserved with normothermic ex vivo heart perfusion (EVHP) against myocardial IRI via inhibiting NLRP3 i

Gunshot Wound to the Posterior Fossa With a Transcerebellar Retromesencepahlic Bullet Path, Transient Mutism, and Unexpected Functional Recovery: The Pivotal, Energy-Absorbing Function of the Petrous Bone and Tentorial Leaflet.

Given the abundance of vital neurovascular structures, gunshot wounds (GSWs) to the posterior fossa are generally fatal. We present a unique such case where a bullet entered the petrous bone, traversed the cerebellar hemisphere and overlying tentorial leaflet, and reached the dorsal aspect of the midbrain, resulting in transient cerebellar mutism with an unexpectedly favorable functional recovery. A 17-year-old boy sustained a GSW to the left mastoid region with no exit wound and presented with agitation and confusion, ultimately leading to a coma. Head CT revealed a bullet trajectory through

Reconstruction for Bilateral Internal Jugular Vein Perfusion Disruption.

When resecting the internal jugular veins bilaterally in surgery for head and neck cancer, it is necessary to perform neck dissection in two stages or to reconstruct the internal jugular veins in one stage. Reconstruction of the internal jugular vein using grafting or direct anastomosis to the external jugular vein have both been reported. We report the case of a 53-year-old man with accidental injury to the left internal jugular vein after resection of the right internal jugular vein for supraglottic cancer. The left internal jugular vein was damaged near the inflow of the subclavian vein, ma

In an endotoxaemic model, antibiotic clearance can be affected by different central venous catheter positions, during renal replacement therapy.

BACKGROUND: In intensive care, different central venous catheters (CVC) are often used for infusion of drugs. If a patient is treated with continuous renal replacement therapy (CRRT) a second catheter, a central venous dialysis catheter (CVDC), is needed. Placing the catheters close together might pose a risk that a drug infused in a CVC could be directly aspirated into a CRRT machine and cleared from the blood without giving the effect intended. The purpose of this study was to elucidate if drug clearance is affected by different catheter placement, during CRRT. In this endotoxaemic animal mo

Trauma mechanisms and patterns of blunt cervical vascular injury: A descriptive study using a nationwide trauma registry.

OBJECTIVE: Blunt cervical vascular injury (BCVI) is a non-penetrating trauma to the carotid and/or vertebral vessels following a direct injury to the neck or by the shearing of the cervical vessels. Despite its potentially life-threatening nature, important clinical features of BCVI such as typical patterns of co-occurring injuries for each trauma mechanism are not well known. To address this knowledge gap, we described the characteristics of patients with BCVI to identify the pattern of co-occurring injuries by common trauma mechanisms. METHODS: This is a descriptive study using a Japanese n

Epidemiology, repair technique, and predictors of stroke and mortality in penetrating carotid artery injuries.

OBJECTIVE: Penetrating carotid artery injuries (PCAI) are significantly morbid and deadly, often presenting in extremis with associated injuries and central nervous system deficit. Repair may be challenging with arterial reconstruction vs ligation role poorly defined. This study evaluated contemporary outcomes and management of PCAI. METHODS: PCAI patients in the National Trauma Data Bank from 2007 to 2018 were analyzed. Outcomes were compared between repair and ligation groups after additionally excluding external carotid injuries, concomitant jugular vein injuries, and head/spine Abbreviate

Drainage Pattern of Craniofacial Veins With Emphasis on its Influence on Facial Reconstruction Procedures.

As the facial transplantation procedures are becoming more popular and frequent in recent years, for repairing facial trauma, variations in the veins of head and neck needs to be reported time and again. This study was undertaken to examine the course and drainage pattern of the facial vein and external jugular vein on this context and emphasize its surgical implications. The authors studied the head and neck region of 50 embalmed cadavers of both sexes to document normal and variant anatomy of facial, retromandibular, and external jugular veins. In 30% of the head and neck regions, different
