Surgical Pitfalls in Carotid Endarterectomy: A New Step-By-Step Approach.

Carotid endarterectomy (CEA) is a surgical intervention that may prevent stroke in asymptomatic and symptomatic patients. Our aim was to examine the microsurgical anatomy of carotid artery and other related neurovascular structures to summarize the CEA that is currently applied in ideal conditions. The upper necks of 2 adult cadavers (4 sides) were dissected using ×3 to ×40 magnification. The common carotid artery, external carotid artery (ECA), and internal carotid artery were exposed and examined. The surgical steps of CEA were described using 3-D cadaveric photos and computed tomography ang

Application of medical adhesive inhibits intimal hyperplasia involving the downregulation of ERK1/2 and eNOS levels.

Vein graft remains the most broadly applied vascular material in coronary artery bypass surgery. However, the restenosis rate of the vein bridge following angioplasty is high. The present study investigated the effect of medical adhesive on vascular intimal hyperplasia, in addition to the signal transduction mechanism. A total of 36 New Zealand white rabbits were divided into three groups at random, including the normal group, the surgery group and the medical adhesive spray group. Following surgery for transplantation of the left external jugular vein to the ipsilateral common carotid artery

Unusual course and termination of common facial vein: a case report.

Given the common and increasing exploitation of superficial head and neck veins in a widening spectrum of clinical and surgical procedures, it is important to equally broaden understanding of the heterogeneity of the vascular anatomy of the region. We report an unusual course of the right common facial vein parallel to the course of external jugular vein, emptying into the ipsilateral subclavian vein in the lateral neck triangle behind the posterior border of the sternocleidomastoid muscle in a 78-year-old male cadaver. Such course may be hazardous for surgical procedures in the region given t

A cadaveric study on variations in branching pattern of external carotid artery.

Variations in the vascular anatomy of the carotid triangle have been reported in current scientific literature. The carotid arteries, being the major feeding arteries of the head and neck deserve special importance and protection from iatrogenic injury during radiological evaluations and surgical interventions. The present study was carried out over a period of 4 years from 2012-2016 to assess the variant anatomy of external carotid artery. The external carotid artery and its branches were dissected bilaterally in 40 formalin embalmed cadavers. The external carotid artery was traced from its o

Technical Nuances of Exposing Rat Common Carotid Arteries for Practicing Microsurgical Anastomosis.

BACKGROUND: Animal models are commonly used in training protocols for microsurgical vascular anastomosis. Rat common carotid arteries (CCAs) are frequently used for this purpose. Much attention has been paid to the technical details of various anastomosis configurations using these arteries. However, technical nuances of exposing rat CCAs have been understudied. The purpose of this study is to describe nuances of technique for safely and efficiently exposing rat CCAs in preparation for a vascular anastomosis. METHODS: Bilateral CCAs were exposed and prepared for anastomosis in 10 anesthetized

The Hybrid Technical Management of Large and Complicated Traumatic Arteriovenous Fistula of Preauricular Region.

BACKGROUND: Arteriovenous fistula (AVF) is defined as an abnormal communication between the high flow arterial system and the low flow venous network, which directly connects the arterial feeding vessels and the near draining veins without normal intervening capillary bed. Arteriovenous fistula incurs in preauricular region is exceeding rare. Most of these fistulae occur as a result of an iatrogenic injury, the volume is small, feeding and draining vessels of feeding and draining are simple, and can be cured easily. However, the treatment of the large and complicated AVF after incidental traum

Intravenous Administration of Bone Marrow-Derived Mesenchymal Stem Cell, but not Adipose Tissue-Derived Stem Cell, Ameliorated the Neonatal Hypoxic-Ischemic Brain Injury by Changing Cerebral Inflammatory State in Rat.

Perinatal hypoxic-ischemic (HI) brain injury occurs in 1 in 1,000 live births and remains the main cause of neurological disability and death in term infants. Cytotherapy has recently emerged as a novel treatment for tissue injury. In particular, mesenchymal stem cells (MSCs) are thought to have therapeutic potential, but little is known about the differences according to their origin. In the current study, we investigated the therapeutic effects and safety of intravenous injection of allogeneic bone marrow-derived MSCs (BM-MSCs) and adipose-derived stem cells (ADSCs) in a rat model of HI brai

A preclinical animal study of a novel, simple, and secure percutaneous vessel occluder for the treatment of varicose veins.

BACKGROUND: Secure, permanent occlusion of the great and small saphenous veins, their tributaries, and perforators is critical for the successful treatment of varicose veins. Current minimally invasive methods replacing surgery are all endoluminal and involve heat (radiofrequency or laser), chemicals (sclerosants and glues), or a combination of mechanical and chemical interventions. This study evaluated in a porcine model the performance of a percutaneous delivery of the Amsel Vessel Occluder (AVO; Amsel Medical Corp, Cambridge, Mass) using ultrasound guidance. The AVO has received United Stat

Surgical retrieval of a guide wire lost during central venous catheterization in a dog.

OBJECTIVE: To report a case of successful surgical removal of a guide wire lost during central venous catheterization. CASE SUMMARY: A 28 kg, 4-year-old female neutered mixed breed dog presented to the primary care veterinarian with diabetic ketosis. During the process of central venous catheterization, the guide wire was accidently released and the entire length of the guide wire slipped into the jugular vein. Due to the absence of nearby interventional radiology facilities, surgical intervention was proposed. An ultrasound was used to determine that the guide wire was located in the caudal

Dedifferentiated Fat Cells as a Novel Source for Cell Therapy to Target Neonatal Hypoxic-Ischemic Encephalopathy.

Neonatal hypoxic-ischemic (HI) encephalopathy (HIE) remains a major cause of mortality and persistent neurological disabilities in affected individuals. At present, hypothermia is considered to be the only applicable treatment option, although growing evidence suggests that cell-based therapy might achieve better outcomes. Dedifferentiated fat (DFAT) cells are derived from mature adipocytes via a dedifferentiation strategy called ceiling culture. Their abundance and ready availability might make them an ideal therapeutic tool for the treatment of HIE. In the present study, we aimed to determin

Ex Vivo Lung Perfusion Rehabilitates Sepsis-Induced Lung Injury.

BACKGROUND: Sepsis is the number one cause of lung injury in adults. Ex vivo lung perfusion (EVLP) is gaining clinical acceptance for donor lung evaluation and rehabilitation and may expand the use of marginal organs for transplantation. We hypothesized that 4 hours of normothermic EVLP would improve compliance and oxygenation in a porcine model of sepsis-induced lung injury. METHODS: We used intravenous lipopolysaccharide (LPS) to induce a systemic inflammatory response in a porcine model of lung injury. Two groups of 4 animals each received a 2-hour infusion of LPS through the external jugu

Investigation of the pathophysiology of cardiopulmonary bypass using rodent extracorporeal life support model.

BACKGROUND: Extracorporeal life support (ECLS) systems are life-saving devices used for treating patients with severe cardiopulmonary failure. In this study, we implemented a rat model of ECLS without the administration of inotropes or vasopressors. METHODS: The rats underwent 5 min of untreated asphyxial cardiac arrest and were resuscitated by ECLS for 30 min. The right external jugular vein and right femoral artery were separately cannulated to the ECLS outflow and inflow, respectively. Thereafter, ECLS was terminated, wounds were closed, and mechanical ventilation was provided for another

Co-administration of Cyclosporin A and Ondansetron decreases transient local cerebral ischemic injury in the mouse.

OBJECTIVES: Several studies indicate that cyclosporin A has a protective effect against brain infarction. In this study, we aimed to determine if co-administration of cyclosporin A and ondansetron could reduce damage caused by cerebral ischemia. METHODS: ICR (Charles River Laboratories) mice were subjected to transient focal cerebral ischemia and divided into 4 groups (O, V, C, and Mix). Immediately after reperfusion, each ligand was administered intravenously through the external jugular vein. Group V animals received 0.9% saline alone, group O animals received 0.1 mg/kg ondansetron solution

Anatomical variations of cervical vein drainage in candidates for neck surgery.

UNLABELLED: The knowledge of both normal and abnormal anatomy of the veins of the neck may be important for surgeons performing neck surgery, to avoid inadvertent injury to vascular structures. In a 75-year-old man candidated to carotid endarterectomy preoperative CT-scan showed a rare anomaly of the venous drainage in the area of the anterior jugular vein (AJV), that usually begins in the suprahyoid region via the confluence of several superficial veins, to open into the ipsilateral external jugular vein. A large left sided venous trunk, originating from an anomalous proximal confluence with
