Biphasic Clinical Course Revealing Lemierre's Syndrome Following Acute Pharyngotonsillitis: A Case Report.

Lemierre's syndrome is a life-threatening septic thrombophlebitis that typically follows an oropharyngeal infection and is most commonly caused by Fusobacterium necrophorum (F. necrophorum). We report a case of a previously healthy woman in her 20s who developed septic shock and acute kidney injury after a recurrent episode of acute pharyngotonsillitis. She was initially treated at a local clinic with a short course of oral third-generation cephalosporin for presumed group A streptococcal pharyngotonsillitis, resulting in temporary clinical improvement. However, she subsequently experienced a

Venoarterial extracorporeal membrane oxygenation in adults with cardiogenic shock.

Cardiogenic shock in adults continues to carry a high mortality despite advances in cardiac care, interventions and critical care. A major advancement in cardiogenic shock care came with the advent of temporary mechanical circulatory support including the intra-aortic balloon pump, ventricular assist devices, and venoarterial extracorporeal membrane oxygenation (VA ECMO). VA ECMO is a type of temporary mechanical circulatory support used in selected patients with cardiogenic shock that have failed to be adequately supported by less invasive approaches including vasoactive medications, volume o

India's Stray Dogs as a Public Health Risk: An Autopsy Case Report and Comprehensive Review.

A major public health issue is stray dogs, particularly in urban and peri-urban areas. Particularly for small children, who are more vulnerable to serious injury and deadly assaults, stray dogs can present serious risks. While playing outside his house, a 5-year-old kid was fatally bitten by a stray dog. Severe head, neck, and back injuries, including deep cuts, exposed occipital bone, abrasions, and bruises, were discovered during an autopsy. The internal findings were a jugular vein tear, laryngeal perforations, vertebral avulsion, and neck muscle lacerations. The growing number of stray dog

Ultrasound Identification of Right Internal Jugular Vein Occlusion After Catheterization in a Premature Infant.

BACKGROUND Right internal jugular vein (RIJV) catheterization is a routine clinical procedure for gastrointestinal surgery. The conventional blind puncture method is often unable to detect vascular anatomical variations, particularly in premature infants. Through widespread adoption of ultrasound-guided puncture techniques, the success rate of catheterization has substantially increased, and vascular anatomical abnormalities can be promptly identified. Internal jugular vein occlusion after catheterization is rarely reported in premature infants. This case report describes RIJV occlusion in a p

Pre-existing vertebrojugular fistula unmasked following an uncomplicated internal jugular vein cannulation and its management.

A vertebrojugular fistula (VJF) is a vascular malformation characterised by abnormal communication between the vertebral artery (VA) and the internal jugular vein (IJV). It can occur following neck trauma, iatrogenic injury or spontaneously in connective tissue disorders, and rarely, it can be congenital. A young adult man in his early 20s with aplastic anaemia was planned for allogeneic haematopoietic stem cell transplantation (Allo-HSCT) for which right IJV cannulation was planned. After a single attempt at straightforward IJV cannulation, high flow was observed through all lumens; an anomal

Catheter malposition leading to superior vena cava perforation: A rare but serious complication of hemodialysis access.

BACKGROUND: Central venous catheterization through the internal jugular vein is a standard procedure for hemodialysis access in end-stage renal disease (ESRD). Although generally safe, serious and life-threatening complications can occur. CASE PRESENTATION: We present a rare case of superior vena cava (SVC) perforation during tunneled dialysis catheter (PermCath) insertion in a patient with chronic renal failure. The injury led to mediastinal hematoma, bilateral hemothoraces, and hemorrhagic shock, requiring emergency thoracotomy and surgical repair. DISCUSSION: Perforation of the SVC is an

Multimodal Imaging of a Post-Traumatic Cervical Arteriovenous Fistula with Concomitant Pseudoaneurysm After a Gunshot Wound Treated with a Covered Stent-Graft.

Background: Arteriovenous fistula is an abnormal connection between an artery and a vein. Post-traumatic fistulas are uncommon and usually occur after penetrating injuries, including gunshot wounds. Cervical arteriovenous fistulas are particularly rare and may coexist with a pseudoaneurysm, creating diagnostic and therapeutic challenges because of the proximity of major vascular, neurological, and aerodigestive structures. In hemodynamically stable patients, the limited external appearance of the wound may underestimate the extent of internal vascular injury; therefore, imaging is central to d

PICC migration and perforation of internal jugular vein after neck dissection.

Peripherally inserted central catheters (PICCs) were introduced as an alternative to centrally inserted central catheters (CICCs) inserted into the jugular vein to provide central venous access. In head and neck reconstructive surgery, PICCs are preferred over CICCs because they do not obstruct surgical access, such as during neck dissection. Additionally, PICCs offer a safer option than peripheral access for the infusion of vesicants, irritants, hyperosmolar solutions, antibiotics, prolonged parenteral nutrition, and chemotherapy agents. We present a case of perforation of the internal jugula

Reliable Flow, Cost-Effective, Comfortable: Tunneled Veno-Pulmonary Artery Extracorporeal Membrane Oxygenation Cannulation Technique.

Veno-venous (VV) extracorporeal membrane oxygenation (ECMO) has many limitations, including patient comfort and, oftentimes, mobility issues. Veno-pulmonary (VP) ECMO offers respiratory support without recirculation and right heart support. For longer mechanical circulatory support (MCS) and improved mobility, non-femoral cannulation is preferred. The current standard is a double-lumen single-cannula via the right internal jugular vein. This requires cannula fixation to the head with a headband, which limits head movement and is uncomfortable due to its size. Flow fluctuations can also occur w

Imaging classification of jugular bulb variants on HRCT temporal bone and CT head and neck angiography: prospective study on standardized reporting using lateral skull base landmarks and correlation with condylar vein.

Accurate imaging recognition of jugular bulb (JB) variants is essential for skull-base surgical planning and prevention of intraoperative vascular injury. This prospective study aimed to determine the prevalence and classification of JB variants using high-resolution CT (HRCT) temporal bone and CT head and neck angiography (CTA), evaluate the utility of standardized radiologic landmarks for consistent reporting and communication between radiologists and surgeons, and to assess associations with clinical symptoms and condylar canal size. A prospective observational study was conducted on 200 pa

Carotid Artery Injury With a Migrating Venous Missile Embolus: A Unique Sequela of Shotgun Neck Trauma.

Ballistic injuries to the neck carry a significant risk of injury to the airway, vascular, and aerodigestive structures. We report the case of a young adult who sustained a close-range shotgun injury to the left anterior neck. Despite hemodynamic stability and an initially intact airway, CT angiography (CTA) was unable to adequately visualize the carotid artery or internal jugular vein (IJV) because of extensive metallic artifact from multiple retained birdshot pellets. The patient underwent urgent operative exploration, during which numerous pellets were identified within the soft tissues, in

Anomalous Vascular Anatomy on Preoperative CT and Surgical Implications for Clavicle Fractures: A Case Report.

CASE: A 64-year-old woman sustained a right diaphyseal clavicle fracture along with ipsilateral rib fractures after being hit by a motor vehicle while cycling. She underwent open reduction and internal fixation of the right clavicle and was found intraoperatively to have an anomalous external jugular vein coursing anterior to the clavicle. CONCLUSION: Anomalous vessels around the clavicle are rare but can be a source of significant hemorrhage when injured. Cross-sectional imaging obtained as part of the trauma workup provides surgeons an opportunity to identify these vessels preoperatively; a

Intravascular Retrieval of a Kinked Inferior Vena Cava Cannula Complicating Venovenous Extracorporeal Life Support.

Cannula malpositioning is one of several complications recognized during the insertion of cannulas for extracorporeal life support that can lead to loss of cannula function or more serious injury due to vessel perforation. In the absence of vessel perforation, repositioning the cannula is often sufficient to restore function. We report a case of a kinked femoral drainage cannula with folding and retroflexion in the inferior vena cava (IVC) that was successfully repositioned with intravascular intervention. A 41-year-old woman was placed on venovenous extracorporeal life support (VV ECLS) for n

Characterizing Initial Cervical Spine and Neurovascular Findings in 84 Consecutive Patients with Hypermobile Ehlers-Danlos Syndrome: A Retrospective Study.

Background: Hypermobile Ehlers-Danlos syndrome (hEDS) can present as a complex interplay of widespread symptomatology and multisystem involvement, posing diagnostic and treatment challenges. Objective characterization of cervical spine and neurovascular findings in hEDS has been limited. Previous studies have emphasized upper cervical spine complications in hEDS, yet the relevance and mechanisms underlying associated symptomatology have not been elucidated. This study examined objective test findings in patients with hEDS at an outpatient neck clinic to explore cervical spine and neurovascular

Internal jugular vein catheterization and catheter insertion slipped into the cervical spinal canal: a special case report.

This paper reports a case of accidental entry of an internal jugular vein catheter into the cervical spinal canal. A male patient in his fifties sustained injuries in a road traffic accident. During the placement of a catheter into the right internal jugular vein, the catheter inadvertently entered the cervical spinal canal, though no damage was caused to the spinal cord or nerves. A second successful puncture and catheter placement was performed under ultrasound guidance, and the patient eventually made a full recovery and was discharged. This case is extremely rare and of particular clinical

A Potentially Lethal Course of a Nasogastric Tube: Case Report.

Nasogastric tube (NGT) insertion is a common procedure, often performed blindly. We report a rare, near-fatal complication in a 44-year-old woman where an NGT traversed the internal jugular vein, superior vena cava, and perforated the right atrium into the pericardial cavity. Prompt imaging and surgical retrieval via sternotomy prevented tamponade or exsanguination. No esophageal injury was found; the likely entry point was the nasopharynx. A sealed duodenal perforation was managed conservatively. This case emphasizes the importance of verifying NGT placement before use and the role of confirm

A surgical technique for lesions involving the ultra-high-riding carotid sheath based on the anatomical principles of the prevertebral fascia: cadaveric dissection and case illustrations.

BACKGROUND: The classic extreme lateral approach (ELA) requires posterior retraction of the sternocleidomastoid muscle (SCM) to expose the ultra-high-riding carotid sheath (UHRCS), which is precisely defined as the segment of the carotid sheath (CS) extending from the site where the sigmoid sinus transitions into the jugular bulb (JB) and exits the skull base to the transverse process of the C2 vertebra. Doing so easily injures the parotid gland (PG) and carotid sheath (CS) and causes significant traction on the internal jugular vein (IJV) and accessory nerve. Thus, this study aimed to describ
