Transcutaneous stimulation is a proven effective way to relieve pain. Its optimal use requires an accurate patient diagnosis. Treatment of pain as a symptom only is likely to fail. There must be a careful psychosocial evaluation, for the majority of patients who come to the doctor complaining of pain have major psychological, social, or behavioral factors that are most important in the genesis of the complaint. The authors reviewed 129 cases of fractures of the neck of the fifth metacarpal. The radiological work-up of this fractures should include an anterio-posterior film with the hand supinated, and an oblique ulnar view with the hand pronated. A retrospective analysis was performed of 89 patients, dead of blunt trauma. Not included in the study were patients with only burns, only head injury, or only a femoral neck fracture and patients who died before or within 1 hr of admission. Correlations were sought between time of death, cause of death, and severity of injury. Lhermitte's sign was experienced by 33.3% of 114 patients with multiple sclerosis, and in 16% it occurred in the first episode. Eleven patients with subacute combined degeneration of the cord due to pernicious anaemia also were reviewed, and one reported Lhermitte's sign. Review of the literature suggests that the symptom rarely occurs in other conditions except subacute combined degeneration of the cord, neck trauma, radiation myelitis and prolapsed cervical disc.

Lesions of the cervical carotid and vertebral arteries resulting from closed craniocervical trauma are being recognized with increasing frequency. This paper describes a patient with severe stenosis and aneurysm formation of both distal internal carotid arteries and stenosis of the left vertebral artery, who presented with only head noise. A multiple vessel angiographic examination is necessary to demonstrate the extent of vascular injury and complications and the collateral circulation.

A series of 18 patients with odontoid fractures due to metastatic cancer were treated at Memorial Sloan-Kettering Cancer Center between 1974--1980.
