Partial Immersion Using Body Bags for Exertional Hyperthermia: A Randomized Crossover Study.

INTRODUCTION: Recent recommendations suggest emergency medical personnel should consider rapid whole-body cooling for heat stroke patients. Cold-water immersion maximizes cooling rate and subsequent outcomes following heat stroke. Body bags filled with ice and water may provide portable, accessible partial cold-water immersion for this setting. The purpose of this study was to determine whole-body cooling rates when using body bags to facilitate partial cold-water immersion to treat exertional hyperthermia. METHODS: A total of 9 healthy participants (8 male; mean [SD] age, 24 [4] years; heigh

Assessment of endothelial glycocalyx degradation in sepsis using direct visualization with sublingual microscopy.

BACKGROUND: The endothelial glycocalyx plays a crucial role in the pathophysiology of sepsis, but assessment of glycocalyx injury in clinical settings remains challenging. Perfused boundary region (PBR) is derived from sublingual intravital video microscopy and has been studied as a surrogate of endothelial glycocalyx barrier function. The goal of this study was to determine whether PBR is associated with circulating heparan sulfate (a biochemical marker of glycocalyx degradation), sepsis status, in-hospital mortality, and intravenous (IV) fluid administration. METHODS: This was an ancillary

Type I IFN promotes ILC2-to-ILC1 plasticity in pre-clinical radiation-induced lung injury.

Radiation-induced lung injury (RILI) is a common dose-limiting toxicity after radiotherapy for lung cancer, but the immunological mechanisms contributing to RILI pathogenesis remain poorly understood. Here we analyze a RILI mouse model using high-dose, restricted-field irradiation to the left lung. Flow cytometry and cytokine profiling analysis of innate lymphoid cells (ILC) reveals a radiation-induced shift from lung-resident type 2 ILCs (ILC2) toward pro-inflammatory type 1 ILCs (ILC1). Single-cell RNA-sequencing analysis of mouse lung ILC populations after radiation suggests that type I int

Extreme temperatures and the epithelial barrier theory.

Extreme temperatures are now recognized as significant global health threats, driving morbidity and mortality across diverse diseases by compromising epithelial barrier integrity and causing periepithelial inflammation. The epithelial barrier theory posits that environmentally induced barrier dysfunction, driven by modern industrial and urban exposures, leads to compromised surfaces of the exposed tissues accompanied by epithelial activation and barrier dysfunction, microbial dysbiosis, loss of commensal bacteria, opportunistic pathogen colonization, bacterial translocation, inflammation and i

Surgical stabilization of rib fractures in a high-altitude trauma cohort: in-hospital clinical outcomes and resource use in a propensity score-weighted retrospective cohort study.

PURPOSE: Surgical stabilization of rib fractures (SSRF) is increasingly used in the management of traumatic rib fractures, but its in-hospital outcomes in high-altitude trauma settings remain uncertain. We evaluated the association of SSRF with in-hospital pulmonary complications and length of stay in a high-altitude (≈ 3,500 m) trauma system. METHODS: We conducted a single-center retrospective cohort study of adults with CT-confirmed traumatic rib fractures admitted from 2015 to 2023 and managed with SSRF or non-operative management. Confounding was addressed using stabilized inverse probabi

Anterior Spinal Artery Aneurysms Associated with Coarctation of the Aorta: A Focused Narrative Review with an Illustrative Case.

Anterior spinal artery (ASA) aneurysms are rare causes of subarachnoid hemorrhage (SAH), and coarctation of the aorta (CoA) may recruit the spinal arterial circulation into a high-flow collateral network. We report the case of a 58-year-old woman who was resuscitated after pulseless electrical activity arrest. Noncontrast CT demonstrated SAH concentrated in the cervical spinal canal with limited intracranial extension. Cervically extended CT angiography identified a C5-6 ventral spinal aneurysm, severe thoracic CoA with extensive collateralization, and diffuse left subclavian artery hypoplasia

Ultraprocessed Food Intake and Adiposity, Vascular Brain Injury, and Cognition in Middle-Aged Adults.

IMPORTANCE: Ultraprocessed foods (UPFs) account for a substantial and increasing proportion of dietary intake in Canada. Although UPFs are consistently associated with adiposity and cardiometabolic disease, their associations with subclinical vascular brain injury (VBI) and cognitive performance in midlife are unclear. OBJECTIVE: To evaluate associations of UPF consumption with adiposity, magnetic resonance imaging (MRI)-defined VBI, and cognitive performance in middle-aged adults in Canada. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study included 8783 adults aged 35 to 69 year

ICU-acquired pneumonia beyond pathogen detection.

Intensive care unit-acquired pneumonia is often treated as a binary diagnosis, although clinical deterioration, imaging and microbial detection describe different aspects of disease. This Perspective proposes an explicit, graded and revisable assessment of whether a detected organism plausibly causes the current lung injury. The framework encompasses pneumonia arising during ventilation, pneumonia that precedes intubation and pneumonia in non-ventilated ICU patients. It places molecular results within clinical, anatomical, microbiological and host context and uses formal reassessment at 48-72

Clinicopathologic Features and Diagnostic Challenges of Mycophenolate Mofetil-Associated Enterocolitis in Lung Transplant Recipients.

Mycophenolate mofetil (MMF) is a commonly used immunosuppressant for lung transplant recipients. Although it efficiently suppresses transplant rejection and improves graft function, MMF has been found to occasionally cause gastrointestinal (GI) injury. The symptoms are nonspecific and include diarrhea, nausea, vomiting, and rare cases of bleeding. Endoscopic findings range from normal appearance to erythema to ulcers. The histopathologic features mimic several different disorders like graft-versus-host disease (GVHD), cytomegalovirus (CMV) colitis, ischemic colitis, or inflammatory bowel disea

Validation studies of ICD-10 diagnostic codes for identifying common causes of hospitalization: a scoping review.

BACKGROUND: International Classification of Diseases, Tenth Revision (ICD-10) diagnosis codes are widely used to identify clinical conditions in administrative health data for epidemiologic research, surveillance, and health services evaluation. However, the validity of ICD-10-coded diagnoses varies across conditions and healthcare settings, and validation studies differ substantially in design, case definitions, and reporting practices. OBJECTIVE: To characterize validation studies of ICD-10-based case definitions for eight common causes of hospitalization and identify methodological factors

Kommerell diverticulum in right aortic arch with aberrant left subclavian artery: Routine resection or selective management?

Right aortic arch with aberrant left subclavian artery (RAA-ALSA) is one of the most common complete vascular rings encountered in pediatric cardiovascular practice. The associated Kommerell diverticulum (KD), a remnant of the primitive fourth dorsal aortic arch from which the aberrant subclavian artery arises, forms part of the posterior ring and may contribute importantly to persistent airway and esophageal compression. Historically, repair consisted of division of the ligamentum arteriosum. Recognition that posterior compression can persist after ligamentum division has led many surgeons to

Radiation Vasculopathy of the Coronary Arteries: State-of-the Art Review.

Radiation-associated coronary artery disease (RACAD) is a distinct form of coronary artery disease with several specific biological and clinical aspects as well as key knowledge gaps. RACAD is prevalent in patients with cardiac exposure to radiotherapy. As radiotherapy is integral to the effective treatment of many thoracic malignancies, improved long-term survivorship is coupled with increased risk for RACAD. Ionizing radiation induces free radical generation and activation of proinflammatory cascades, leading to endothelial dysfunction and accelerated atherosclerosis. Risks for RACAD are det

Postoperative ossification progression after decompression for thoracic ossification of the ligamentum flavum: associated factors identified by serial CT analysis.

INTRODUCTION: Postoperative ossification progression after decompression for thoracic ossification of the ligamentum flavum (T-OLF) has been reported, but factors associated with its progression remain poorly understood. The role of additional instrumentation in preventing postoperative ossification progression also remains controversial. METHODS: We retrospectively reviewed 65 patients who underwent primary surgery for T-OLF between 2007 and 2024. Clinical characteristics and surgical outcomes were compared between patients treated with decompression alone and those treated with decompressio

Warfarin for Left Ventricular Thrombus in Nonischemic Cardiomyopathy With Acute Kidney Injury: A Case Report and Literature Review of Anticoagulation Strategies in Renal Impairment.

Left ventricular (LV) thrombus is a serious complication of cardiomyopathy with severely reduced ejection fraction, carrying significant risk of systemic embolization. While direct oral anticoagulants (DOACs) have emerged as a reasonable alternative to warfarin for LV thrombus treatment in the general population, no guideline-directed evidence exists for their use in patients with concomitant acute kidney injury (AKI) or significant renal impairment in this specific clinical context. We present the case of a 47-year-old male with newly diagnosed nonischemic cardiomyopathy (ejection fraction 25

Venous thoracic outlet syndrome in the presence of cervical rib: report of two cases.

Thoracic outlet syndrome is a rare condition caused by the compression of neurovascular structures in the region between the neck and chest. The venous form accounts for 2% to 3% of cases, with deep vein thrombosis most often caused by compression of the subclavian-axillary vein segment between the first rib and the clavicle. The occurrence of a cervical rib as a cause of venous deep vein thrombosis is even rarer. In this report, we present the diagnosis, treatment, and outcome of 2 cases of venous deep vein thrombosis resulting from 2 different mechanisms. The approach was based on a review o

Surgical repair of traumatic right subclavian artery pseudoaneurysm after war shrapnel injury: A case report.

A rare right subclavian artery pseudoaneurysm developed after a war-related shrapnel injury in a 46-year-old female with a prior thyroidectomy. There was no initial bleeding or neurologic deficit, and initial noncontrast computed tomography findings were negative. One month later, the patient presented with chest pain, hoarseness, and dysphagia. A chest X-ray showed tracheal compression, Doppler ultrasound revealed leakage from the posterior wall of the right subclavian artery, and computed tomography angiography confirmed a 6 × 7 cm pseudoaneurysm causing 70% tracheal narrowing.
