Medical management of acute partial skin necrosis following nipple-sparing mastectomy using an M101-based oxygenating dressing: Two case reports.

Postmastectomy skin-flap necrosis after nipple-sparing mastectomy (NSM) is common and exceeds rates after skin-sparing mastectomy (SSM) (0-19.5 %), influenced by patient factors and modifiable intraoperative variables. Presentations range from superficial necrosis to full-thickness necrosis requiring surgical debridement. These events drive reoperation or implant loss, may delay adjuvant therapy, and burden patients psychologically. Clinical examination and ancillary tests poorly discriminate depth; thus, management usually entails close observation until demarcation, followed by targeted inte

Surgical stabilization of rib fractures in conjunction with veno-venous ECMO for traumatic chest wall injury: a multi-institutional case series from the Chest Wall Injury Society (CWIS).

INTRODUCTION: Rib fractures are prevalent in 10% of blunt injury admissions and contribute significantly to trauma-related mortality. Severe cases, such as flail segments or multiple displaced fractures, frequently necessitate surgical stabilization of rib fractures (SSRF) to mitigate pain and improve outcomes. Pulmonary contusions, present in about 25% of chest wall injuries, can be exacerbated by severe rib fractures, leading to acute respiratory distress syndrome. These cases can lead to a combination of therapies-SSRF and veno-venous extracorporeal membrane oxygenation (VV-ECMO)-that is ra

Bilateral Pre-expanded Angel-Wing Lower Trapezius Myocutaneous Flaps for Extensive Cervicothoracic Defect Reconstruction.

BACKGROUND: Lower trapezius myocutaneous flap (LTMF) is widely used for reconstructing head and neck defects. However, a single LTMF, even after pre-transfer tissue expansion, is often insufficient to cover defects involving the bilateral neck and chest wall. This study presents our experience with the use of bilateral pre-expanded LTMFs for repairing extensive cervicothoracic defects. METHODS: A retrospective study was conducted of patients who underwent bilateral pre-expanded LTMF reconstruction between March 2013 and August 2024. Patient demographics, defect features, flap characteristics,

Early results from the use of an innovative vertical fascial traction system for the management of patients with open abdomen.

BACKGROUND: The combination of negative pressure wound therapy (NPWT) with dynamic fascial traction is currently considered the preferred method for temporary closure of the open abdomen (OA). However, this approach often requires repeated returns to the operating room for further fascial approximation. The aim of this study was to present our institution's experience with a vertical fascial traction device (VTD) for OA management and early closure. METHODS: This is a prospective registry of patients treated with the VTD between May 2023 and the present. The system used is commercially named

Targeting osteosarcopenia and multimorbidity for frailty prevention through identification and deep phenotyping methods in healthy ageing and high-burden disease cohorts (OPTIMA-C): a longitudinal observational cohort study protocol for neuromusculoskeletal muscle health.

INTRODUCTION: Sarcopenia and frailty have been identified as negative predictors of health outcomes. Patients with stroke, traumatic brain injury (TBI), knee osteoarthritis (OA) and breast cancer commonly experience low physical activity levels in the chronic phase of recovery. This prospective study aims to explore the feasibility of multimodal screening and longitudinal tracking of various biomarkers from the acute to chronic phase of disease to determine the relationship with frailty outcomes. METHODS AND ANALYSIS: A prospective longitudinal observational cohort study involving Asian popul

Robotic Repair of a Missed Traumatic Diaphragmatic Injury in a Prior Gastric Bypass Patient With Herniation of the Gastric Remnant.

Traumatic diaphragmatic injuries following blunt or penetrating trauma have a low incidence rate. Symptoms can be obscured by distracting injuries or, in some cases, patients can be asymptomatic, making diagnosis difficult. A 57-year-old female who underwent a robotic Roux-en-Y gastric bypass with a hiatal hernia repair in 2021, presented as an alerted trauma in February 2024 following a motor vehicle accident (MVA). She was an unrestrained driver traveling 35 mph with airbag deployment. During an assessment, it was noted that she had extensive left breast and chest ecchymosis; however, trauma

Designing a multi-arm, multi-stage platform trial for venous leg ulceration - Venous leg ulcers: management and eradication, the VEIN platform study.: Designing a platform/adaptive trial for venous leg ulcers (VLU) - The VEIN platform.

BACKGROUND: Venous leg ulceration (VLU) is the most severe form of venous disease and an important burden to patients and society. Many treatments for VLU exist, including wound therapies, medications, and surgical interventions. However, high-level randomized trial data supporting leg ulcer treatments are lacking, limiting their adoption in clinical practice. We developed a platform trial assessing multiple interventions for VLU comprising multiple multi-arm multi-stage trials. METHODS: Scoping reviews, surveys, meetings, and focus groups were conducted over five work streams to inform the d

Right Subclavian Vein Pseudoaneurysm Following Percutaneous Axillary Lymph Node Biopsy: A Case Report.

Subclavian vein pseudoaneurysms are rare vascular lesions, and only a few cases have been reported in the literature, with most attributed to blunt trauma. We describe the first reported case of a subclavian vein pseudoaneurysm following percutaneous axillary lymph node biopsy. A 42-year-old woman presented with a progressively enlarging, painless right neck mass 18 months after treatment for node-positive breast cancer. Imaging with duplex ultrasound and contrast-enhanced computed tomography demonstrated a large saccular pseudoaneurysm arising from the right subclavian vein without arterial c

Study of Treatment Modalities and Clinical Outcomes of Screen-Detected Cancers at a Tertiary Care Unit in the UK.

Introduction Breast cancer is the most common type of cancer in women worldwide, and early detection plays a key role in improving survival and treatment outcomes. National breast screening programs help identify both invasive and non-invasive cancers, such as ductal carcinoma in situ (DCIS). This study aimed to compare the one-year outcomes of screen-detected invasive breast cancer and DCIS in women diagnosed through a regional screening program. Methods A retrospective cohort study was conducted at our tertiary center in the UK. Patients diagnosed with screen-detected breast cancers from Jan

Thigh-based Breast Reconstruction: A Decade of Experience With the Diagonal Upper Gracilis Flap.

BACKGROUND: The thigh is an alternative donor site for autologous breast reconstruction when abdominal flaps are unsuitable, though limited tissue and donor-site complications pose challenges. To improve volume and reduce morbidity, thigh flaps evolved from the transverse upper gracilis to the diagonal upper gracilis (DUG) designs. This study presented the largest DUG flap review, evaluating outcomes and donor-site morbidity. METHODS: A retrospective review of patients undergoing DUG flap breast reconstruction was performed between 2012 and 2022. Demographics, surgical details, and complicati

De-escalation of axillary surgery and targeted axillary dissection following neoadjuvant chemotherapy: multicentre prospective regional audit.

BACKGROUND: Emerging evidence supports axillary de-escalation in patients with clinically node-positive breast cancer with low-volume residual disease following neoadjuvant chemotherapy, avoiding axillary node clearance in selected patients. Targeted axillary dissection, which retrieves a known metastatic, clipped node alongside standard sentinel node biopsy aims to reduce false-negative rates. This study evaluated axillary surgery after neoadjuvant chemotherapy across NHS Greater Glasgow and Clyde, and examined 10-year trends. METHODS: Patients with node-positive breast cancer receiving neoa
