The failure of a postoperative local infection to resolve after appropriate antibiotic therapy should prompt consideration of other diagnoses. Reported here is a case of pyoderma gangrenosum, a rare necrotizing disorder, occurring after reduction mammaplasty. The clinical presentation was one of progressive wound deterioration with associated erythema and intense pain. Endoluminal retrieval of foreign bodies in the cardiac laboratory is an uncommon and challenging procedure for the endovascular specialist, and its only alternative is an open exploration. Three female patients, 39, 39 and 46 years old, had recurrent, non-puerperal, subareolar abscesses caused by obstructed and dilated lactiferous ducts and resulting in paramamillary fistulas. Further recurrence was stopped by surgery. Based on the pathogenesis, surgery is the recommended treatment for this chronic ailment, i.e. fistulectomy using a radial incision through the centre of the nipple and excising the pathological nipple-areolar complex. Only the nipple is reconstructed, leaving the remainder of the wound open for healing by secondary intention and thus preventing recurrence.

Surgical site infections (SSIs) are a notable cause of hospital morbidity and mortality. Antibiotic prophylaxis has demonstrated a significant reduction in infection rate in clean-contaminated surgery and in clean surgery to a limited extent. To make antibiotic prophylaxis effective it is necessary to choose the right antibiotic, to administer it preoperatively and maintain sufficient serum and tissue levels through the operation.
