Controlled local air-balloon compression chest belt on postoperative seroma in breast cancer: study protocol for a randomised controlled trial in Zunyi, China.

BACKGROUND: Seroma formation is a common postoperative complication of breast cancer surgery, leading to delayed wound healing, infection, patient discomfort, and repeated outpatient visits. In severe cases, it can cause skin flap necrosis, imposing physical, psychological, and financial burdens on patients. Applying compression bandaging to the patient's postoperative wound with a chest strap is an effective measure to prevent seroma formation. However, the commonly used elastic chest straps cannot provide specific compression values, limiting their use. Therefore, we aim to compare the effic

"Simultaneous bulge repair and recontouring after abdominally based breast reconstruction: A well-fixed open transversus abdominis release approach".

An abdominal bulge is an often-underreported complication of abdominally based breast reconstruction (ABBR) with no gold standard treatment. A bulge confers substantial morbidity to patients through compromised core function and poor esthetic contour. Here, we present a new technique for bulge repair that addresses both aspects through an open transversus abdominis release (TAR) with a well-fixed, polypropylene mesh. An electronic data warehouse query was initiated using keywords "bulge" and "TAR" for the senior author's (GAD) patients from 2020-2025 for bulge repair after ABBR. A total of 11

Vaginal cuff dehiscence and bowel evisceration as a late-onset complication of vaginal vault prolapse: a case report.

BACKGROUND: Vaginal cuff dehiscence (and evisceration, VCDE) after hysterectomy is a rare, but potentially serious complication. Current medical literature describes the most critical risk factors for VCDE after hysterectomy as surgical technique, use of thermal energy, early mechanical stress (especially sexual intercourse), infection, smoking, and obesity. There is currently no evidence regarding a potential association between VCDE and anti-HER2 therapy. We report a late-onset VCDE in a patient receiving combined antihormonal and anti-HER2 therapy, and discuss the hypothesis-generating ques

Percutaneous Suction Drain Versus Conventional Incision and Drainage for the Management of Acute Breast Abscess: A Prospective Comparative Study.

Background Acute breast abscess is a common surgical condition requiring prompt and effective drainage for optimal resolution. Conventional incision and drainage (I&D), although widely practiced, is associated with significant postoperative morbidity, including increased pain, prolonged wound care, delayed healing, and unsatisfactory cosmetic outcomes. These limitations have led to growing interest in minimally invasive alternatives such as percutaneous suction drain (PSD) placement, which may offer comparable efficacy with reduced morbidity. The present study was undertaken to compare PSD pla

Superficial Circumflex Iliac Artery Perforator Flap Reconstruction for Extensive Skin Defects after Mastectomy.

Reconstruction of extensive chest wall skin defects following mastectomy remains challenging, and a variety of reconstructive options have been reported. Although skin grafts, local flaps, and pedicled musculocutaneous flaps are widely used, these approaches may result in suboptimal cosmetic or functional outcomes, carry a relatively high risk of partial flap necrosis, and require sacrifice of muscle tissue. Additionally, achieving stable wound closure without delaying the initiation of postoperative radiotherapy or chemotherapy is particularly important in such cases. Here, we report five pat

Early Use and Outcomes of Polydioxanone Mesh in Implant-based Breast Reconstruction.

BACKGROUND: As the diagnosis of breast cancer has increased during the past 2 decades, so too has the number of resection procedures, including mastectomies. Of the available reconstructive options postmastectomy, implant-based breast reconstruction (IBBR) predominates. Biologic scaffolds, such as human acellular dermal matrices, are routinely used to support tissue expanders and implants. However, human acellular dermal matrices have several limitations, including high infection rates, high cost, and supply concerns. Synthetic scaffolds address some of these concerns, though they differ signi

A Global Propensity Score-matched Analysis of 56,992 Nicotine-dependent Versus Nondependent Breast Reconstruction Cases.

BACKGROUND: Nicotine dependence is known to cause impaired wound healing and increase risk of surgical complications; nonetheless, the literature lacks evidence regarding the risks of capsular contracture and other implant-related complications. METHODS: A retrospective cohort study was conducted using data from the TriNetX global health research network. Patients undergoing breast reconstruction were categorized into 2 cohorts: those with nicotine dependence documented within 6 months before surgery and those without nicotine use. RESULTS: Propensity score matching was performed, resulting

Refined reduction mammaplasty for severe breast hypertrophy: Technique and outcomes of a modified free nipple-areolar complex graft approach.

BACKGROUND: Severe breast hypertrophy presents a significant surgical challenge, especially in high body mass index (BMI) patients, due to the risks of poor projection, wound complications, and nipple-areolar complex (NAC) necrosis. This study evaluates a refined technique combining free NAC grafting with dual fascial fixation and minimal suture stabilization to optimize outcomes in this high-risk population. METHODS: We retrospectively reviewed patients with severe breast hypertrophy who underwent reduction mammaplasty using the described technique between 2019 and 2023. Surgical indication

Beyond clinical closure: functional skin barrier recovery to prevent diabetic foot ulcer recurrence: a narrative review.

Diabetic foot ulcer (DFU) recurrence remains common after apparent wound healing, partly because conventional endpoints define healing as complete epithelialization without drainage rather than as durable recovery of skin function. This narrative review proposes functional closure as a clinically useful extension of current DFU healing assessment. Functional closure refers to a post-healing state in which the regenerated ulcer site has recovered sufficient epidermal barrier competence to limit transepidermal water loss (TEWL), maintain stratum corneum hydration, tolerate minor mechanical traum

Sweat gland carcinoma with neuroendocrine differentiation (SCAND) arising in the axilla: A case report highlighting diagnostic challenges and surgical management.

BACKGROUND: Sweat gland carcinoma with neuroendocrine differentiation (SCAND) is a rare, newly recognized cutaneous adnexal tumor arising from sweat glands and characterized by neuroendocrine features. Given its rarity and resemblance to benign lesions, clinical diagnosis can be challenging. CASE PRESENTATION: A 73-year-old man presented with a 40-year history of intermittent discharge from a right axillary mass, which had initially been diagnosed as an epidermal cyst. Following lesion excision along the tumor margin at a local clinic, histopathological analysis revealed apocrine carcinoma, a

Unmasking Ehlers-Danlos Syndrome Through Reduction Mammaplasty: An Illustrative Case and a Systematic Literature Review.

Ehlers-Danlos syndrome (EDS) is characterized by joint hypermobility, skin hyperextensibility, and tissue fragility, which may worsen symptomatic macromastia. Concerns about elevated surgical risk have historically discouraged elective procedures in EDS patients; however, postoperative complications may themselves prompt an EDS diagnosis, as illustrated by our case. This relationship prompted the conduct of a systematic review examining outcomes of reduction mammaplasty in EDS. A 37-year-old woman underwent bilateral inferior pedicle reduction mammaplasty for symptomatic mammary hyperplasia. P

The Role of ULK3 in Cancer Progression: A Pan-Cancer Bioinformatics Analysis Integrated with Experimental Validation in Prostate Cancer.

Unc-51-like kinase 3 (ULK3) is a key member of the ULK serine/threonine kinase family. Aberrant ULK3 expression has been increasingly linked to tumorigenesis and malignant progression in multiple cancer types. However, the precise role of ULK3 in tumor initiation and progression remains incompletely understood. Leveraging integrated multi-omics data from The Cancer Genome Atlas (TCGA), the Genotype-Tissue Expression (GTEx) project, and the Clinical Proteomic Tumor Analysis Consortium (CPTAC), we systematically characterized the expression of ULK3 at both the transcript and protein levels acros

Successful revision surgery after postsurgical pyoderma gangrenosum following reduction mammaplasty: A case report.

BACKGROUND: Postsurgical pyoderma gangrenosum (PSPG) is a rare neutrophilic dermatosis that occurs following surgical procedures and frequently mimics postoperative infection. Misdiagnosis may lead to repeated surgical debridement and disease progression due to pathergy. Reports describing safe reoperation following PSPG remain limited, creating uncertainty regarding subsequent reconstructive procedures. CASE: We report the case of an 18-year-old woman with well-controlled perinatal HIV who developed PSPG following bilateral reduction mammaplasty. On postoperative day (POD) 2 she developed re

Fungating Breast Carcinoma Complicated by Carbapenem-Resistant Pseudomonas aeruginosa Infection in a Patient With Severe Blood-Injection-Injury Phobia: A Case Report.

Fungating breast tumors represent advanced breast cancer and are frequently complicated by secondary infection. While bacterial colonization is common, infection with carbapenem-resistant Pseudomonas aeruginosa (CR-PSA) in breast wounds is rare and poses significant therapeutic challenges. We report the case of a 60-year-old woman who presented with a six-month history of a progressively enlarging fungating right breast mass, with delayed evaluation due to severe blood-injection-injury (BII) phobia. On admission, she was septic and profoundly anemic. Imaging demonstrated a large exophytic brea

Transient Phagocytic Dysfunction Mimicking Chronic Granulomatous Disease: A Rare Complication in Delayed Transverse Rectus Abdominis Myocutaneous Flap Breast Reconstruction.

Autologous breast reconstruction depends on intact wound healing. Chronic granulomatous disease (CGD) results from nicotinamide adenine dinucleotide phosphate oxidase dysfunction and predisposes patients to opportunistic infections; acquired forms can transiently mimic the congenital disorder. We report a case of transient phagocytic dysfunction that complicated delayed transverse rectus abdominis myocutaneous flap reconstruction. A 43-year-old woman with Stage IIB breast cancer underwent delayed reconstruction 18 months after cytotoxic chemotherapy. A delay procedure (inferior epigastric vess
