From Evidence to Practice: Quilting for Seroma Prevention After Mastectomy and Surgeons' Acceptance.

Introduction Seroma formation is a frequent complication after simple mastectomy due to the creation of a large dead space. Multiple aspirations are required to manage patients' symptoms. At present, breast surgeons lack consensus on effective preventative strategies. To address this issue, we have implemented a technique called quilting, which involves suturing the mastectomy flap to the chest wall muscle to convert the dead space into smaller compartments, thereby reducing the possibility of seroma.  Method An audit was carried out from April 2020 to June 2021 to establish postoperative sero

Beyond the T-junction: Reinforce strategic sites in superomedial pedicle mammoplasties using a dual dermal flap technique.

INTRODUCTION: Breast reduction using superior or superomedial pedicles provides favorable long-term aesthetic outcomes, particularly regarding breast shape and projection. However, wound dehiscence remains a concern at the T-junction and the vertical-areolar junction, with reported rates ranging from 4 % to 20 %, contributing to a higher postoperative burden, suboptimal aesthetic results, and increased healthcare costs. The development of preventive techniques is a key aspect as scar quality is considered one of the most important factors by patients. Only a few techniques in the literature de

A scoping review of the Goldilocks mastectomy.

BACKGROUND: The Goldilocks mastectomy was originally developed as a "middle ground" between a breast reconstruction and a modified radical mastectomy, particularly for obese women who are poor candidates for direct breast reconstruction. This scoping review aims to provide an overview of surgical techniques, complications, cosmetic outcomes and patient-reported quality of life following the Goldilocks mastectomy. METHODS: Following the PRISMA-ScR guidelines a systematic search was conducted in March 2025, using PubMed and Embase. The identified studies were screened by two independent reviewe

Mitigating heat stress in commercial broilers through feeding management approaches: Insights into growth, serum biochemical indices, meat quality, and gut histomorphology.

BACKGROUND: Heat stress (HS) significantly affects tropical broiler farming by impairing growth, physiology, meat quality, survival, and profitability. Since feed constitutes a major production cost, adopting feeding management approaches may reduce costs while enhancing bird resilience under HS without compromising productivity. AIM: This study aimed to evaluate the effectiveness of different feeding management approaches on growth, rectal temperature, water intake, serum biochemistry, meat quality, gut histomorphology, and profitability in heat-stressed broilers. METHODS: A total of 750 br

Dietary nitrate and nitrite protect against doxorubicin-induced cardiac fibrosis and oxidative protein damage in tumor-bearing mice.

Anthracycline-induced cardiotoxicity remains a major limitation in cancer therapy, affecting long-term cardiovascular health in survivors. Dietary nitrate supplementation has shown cardioprotective effects in preclinical models of doxorubicin (Dox)-induced and ischemia-reperfusion injury, but it is unclear whether nitrate and/or nitrite (NOx) would have adverse effects on the anticancer efficacy of the drug. To evaluate Dox efficacy against triple-negative breast cancer (TNBC) in the presence of dietary nitrate and nitrite, tumor-bearing BALB/c mice (N = 5 mice per group, 10 mice total) were t

Managing Drainless Mastectomy With Closed Incision Negative Pressure Wound Therapy Using Full-coverage Foam Dressings.

BACKGROUND: In the United Kingdom, the rise in simple mastectomies without drain placement has contributed to improving rates of day-case surgery. However, this practice has resulted in increased rates of symptomatic seromas and greater resource burden in clinics. For postoperative incision management, we have implemented the use of closed incision negative pressure therapy (ciNPT) with foam dressings that cover the entire incision and surrounding tissues. The aim of this study was to establish whether the use of full-coverage ciNPT after mastectomy reduced seroma-related interventions. METHO

Enhancing full-thickness skin graft take in nipple reconstruction using PICO™ negative pressure therapy: A technical note.

Nipple-areola complex (NAC) necrosis remains a challenging complication following nipple-sparing mastectomy, with significant emotional and aesthetic consequences for patients. Full-thickness skin grafting (FTSG) offers a viable reconstructive option in secondary procedures. We report our experience using PICO™ single-use negative pressure wound therapy (NPWT), applied directly over the breast, to enhance graft take in three consecutive cases of nipple reconstruction using groin-derived FTSG. The technique was effective, cost-conscious, and well-tolerated by patients during the recovery period

Extensive Mastectomy Skin Flap Necrosis Triggered by Hypertension, Psychological Stress, and Prolonged Operative Time Due to Intraoperative Needle Loss: A Case Report.

Mastectomy skin flap necrosis (MSFN) is a frequent complication influenced by multiple factors. We report a 73-year-old woman with untreated hypertension and preoperative anxiety who developed extensive MSFN after mastectomy. Intraoperative loss of a suture needle prolonged operative time by approximately one hour, contributing to progressive flap necrosis requiring repeated debridement and negative pressure wound therapy. This case was documented to highlight the importance of recognizing how overlooked comorbidities, psychological stress, and intraoperative incidents can interact to exacerba

Effectiveness of Regional Blocks for Postoperative Pain Control and Recovery in Breast Cancer Surgery in a Resource-Limited Setting: A Prospective Observational Study.

Background and aims Effective postoperative pain management after breast cancer surgery is essential to enhance recovery, minimise opioid use, and prevent chronic post-surgical pain. In low- and middle-income countries (LMICs) like Sri Lanka, limited access to ultrasound and trained personnel often restricts the use of regional anaesthesia (RA). Although regional anaesthesia (RA) is well-established for breast cancer surgery, evidence from low- and middle-income countries remains limited. This study evaluated the effectiveness and feasibility of ultrasound-guided regional nerve blocks in impro

Source-grounded Artificial Intelligence-Driven Transfer of Plastic Surgery Textbooks to Podcasts: Creation of Content and Trainee Satisfaction.

Source-grounded artificial intelligence (AI) models have emerged in the public domain and represent a powerful tool for content creation. These models allow users to "ground" the language model in user-input sources and convert text to audio. Although plastic and reconstructive surgery has traditionally relied on textbook sources for trainee education, we demonstrate the potential to migrate information to audio sources. We created podcasts using source-grounded AI technology based on selected chapters of Grabb and Smith's Plastic Surgery. Generated audio was created using NotebookLM and featu

Medical management of acute partial skin necrosis following nipple-sparing mastectomy using an M101-based oxygenating dressing: Two case reports.

Postmastectomy skin-flap necrosis after nipple-sparing mastectomy (NSM) is common and exceeds rates after skin-sparing mastectomy (SSM) (0-19.5 %), influenced by patient factors and modifiable intraoperative variables. Presentations range from superficial necrosis to full-thickness necrosis requiring surgical debridement. These events drive reoperation or implant loss, may delay adjuvant therapy, and burden patients psychologically. Clinical examination and ancillary tests poorly discriminate depth; thus, management usually entails close observation until demarcation, followed by targeted inte
