Operative management of unstable sternal fracture in polytrauma: a case report and literature review.

Sternal fractures are uncommon but clinically relevant injuries, most often resulting from blunt thoracic trauma in motor vehicle collisions. While most isolated fractures can be treated conservatively, surgical stabilisation may be required in cases of displacement, instability, or polytrauma. Modern plating techniques offer biomechanical advantages over traditional wire fixation, but indications remain variably defined. We report the case of a 33-year-old male polytrauma patient involved in a high-impact motor vehicle accident. He sustained a displaced sternal fracture with manubriosternal d

An Overview of Meta-Analyses on the Surgical Stabilization of Rib Fractures in Adults: A Narrative Umbrella Review (2020-2025).

Background: Rib fractures are a common cause of morbidity in trauma patients. The surgical stabilization of rib fractures (SSRF) has gained increasing attention as a therapeutic option; however, evidence from multiple meta-analyses remains heterogeneous. Methods: We performed an overview of 11 meta-analyses, including a total of 1,117,849 adult patients (narrative umbrella review), published between November 2020 and November 2025 to summarize and critically appraise high-level evidence comparing SSRF with non-operative management (NOM) in adults with traumatic rib fractures. Contemporary outcomes of flail chest injuries: An update from ACS-TQIP (2017-2023).

BACKGROUND: Flail chest represents one of the most severe forms of blunt thoracic injury and is associated with significant morbidity and mortality. Much of the understanding of flail chest epidemiology, and outcomes came from Dehghan et al. (2014). Since then, flail chests management has undergone substantial evolution. Given these changes, we sought to examine the outcomes of flail chest that accurately reflects current clinical practice. METHODS: This is a retrospective study analyzing the American College of Surgeons Trauma Quality Improvement Program (ACS-TQIP) 2017 to 2023 data on flail

Surgical Stabilization of Rib Fractures: Current Evidence and Considerations for Posterior Rib Fractures.

PURPOSE: Posterior rib fractures represent severe thoracic trauma and are associated with a higher risk of complications due to their proximity to vital intrathoracic structures, including the intercostal vessels and lungs. Surgical stabilization of rib fractures (SSRF) has emerged as an operative strategy to restore chest wall stability, improve respiratory mechanics, and reduce pain. However, evidence specifically addressing posterior rib fractures remains limited, and current understanding is largely extrapolated from studies involving general rib fracture populations. METHODS: This litera

Rapid sequence versus early surgical stabilization of severe chest wall injuries: a propensity score-matched analysis from a multicenter trauma registry.

BACKGROUND: The optimal timing for post traumatic Chest Wall Reconstruction (CWR) in severely injured / polytraumatized patients with severe chest wall instability remains a subject of debate. While early surgery within 72 h is associated with improved outcomes, the efficacy and safety of an even earlier "rapid sequence" approach on the day of admission are unclear. This study aims to compare outcomes of severely injured patients undergoing rapid sequence surgery (Day 0) versus early surgery (Days 1-3). METHODS: A retrospective analysis was conducted using data from the TraumaRegister DGU® (2

Revised cardiac risk index and adverse outcomes in patients undergoing surgical stabilization of rib fractures.

BACKGROUND: Surgical stabilization of rib fractures (SSRF) is increasingly performed in patients with severe rib fractures or flail chest. It has been shown to reduce ventilator days and pulmonary complications. Nevertheless, SSRF carries perioperative morbidity, particularly pulmonary and cardiac events, and the optimal preoperative cardiac risk stratification tool for this population has not been established. This study evaluated the association between the Revised Cardiac Risk Index (RCRI) and in-hospital mortality and cardiopulmonary complications in SSRF patients. METHODS: This retrospec

Beyond 72 Hours: A Retrospective Evaluation of Factors Associated With Delayed Surgical Stabilization of Rib Fractures.

BACKGROUND: Surgical stabilization of rib fractures (SSRF) is recognized as a key intervention in reducing morbidity among patients with multiple displaced rib fractures or flail chest. Early SSRF, typically within 72 hours of admission, has been associated with improved outcomes. The factors contributing to delayed stabilization, defined as 72 hours or greater following admission, remain unclear. METHODS: A retrospective cohort study was performed, evaluating 492 trauma patients who underwent SSRF at a level II trauma center. Patient demographics, injury characteristics, and associated injur

Outcomes of surgical stabilization of rib fractures in COPD patients: A propensity-matched analysis stratified by injury pattern.

BACKGROUND: Patients with chronic obstructive pulmonary disease (COPD) who sustain rib fractures are at increased risk of respiratory complications due to impaired pulmonary reserve. Surgical stabilization of rib fractures (SSRFs) has demonstrated benefits in selected trauma populations; however, its impact in patients with COPD remains unclear. METHODS: Adult COPD patients aged ≥35 with multiple rib fractures were identified from the National Trauma Data Bank (2017-2022). Patients were stratified according to treatment strategy: SSRF versus nonoperative management. Propensity score matching

Surgical stabilization of flail chest: Patient selection, hospital variation, and outcomes.

BACKGROUND: Surgical stabilization of rib fractures (SSRF) is increasingly used for flail chest, yet practice remains heterogeneous. We aimed to identify predictors of SSRF, quantify hospital-level variation, and evaluate whether hospital SSRF volume and utilization rate are associated with outcomes in flail chest. METHODS: Using ACS-TQIP 2017 to 2020, we conducted a retrospective cohort study of trauma patients older than or equal to 18 years with flail chest. Mixed-effects logistic regression evaluated predictors of SSRF and between-hospital variation. Hospitals that performed ≥1 SSRF were

Timing of surgery and predictors of canal reduction after short-segment fixation for thoracolumbar burst fractures.

PURPOSE: In patients with thoracolumbar burst fractures, surgical delay is often unavoidable due to concomitant injuries. Concerns persist that delayed surgery may compromise indirect reduction of retropulsed fragments via ligamentotaxis. This study aimed to determine whether the timing of surgery affects the quality of intracanal fragment reduction and identify independent predictors of successful reduction following short-segment posterior fixation. METHODS: This multicenter study included 252 patients who sustained a single traumatic thoracolumbar burst fracture and underwent short-segment

A Propensity-Matched Analysis of the Impact of Sternal Fractures on Outcomes in Concurrent Thoracic Spine Fractures: The Fourth Column of the Spine.

BACKGROUND: The thoracic vertebrae, ribs, and sternum, provide structural and protective support for the upper body. The unique framework of the region suggests that fractures involving both the thoracic spine and sternum occur with notable spinal instability. Despite their clinical relevance, the short-term medical and surgical outcomes of combined sternal and thoracic spinal fractures remain poorly understood compared with thoracic fractures alone. This study aims to compare the 90-day complication rates and surgical intervention requirements in patients with concomitant sternal and thoracic

Early surgical stabilization of rib fractures in patients with mechanical ventilation-dependent respiratory failure secondary to severe chest wall injury: a retrospective cohort study.

BACKGROUND: Patients with multiple rib fractures and flail chest often also experience subsequent respiratory failure requiring mechanical ventilation. The benefits of surgical stabilization of rib fractures (SSRF) for this critically patient population remains controversial. This study aimed to evaluate whether early SSRF facilitates ventilator liberation and reduces complications in patients who develop respiratory failure requiring mechanical ventilation within 24 hours of admission. METHODS: This retrospective cohort study analyzed 59 patients with three or more rib fractures or radiograp

Management of Thoracolumbar Spinal Fracture Caused by an Aircraft Accident.

INTRODUCTION: Aircraft crashes are infrequent but often associated with high mortality, morbidity, and only a limited number of survivors. These events are classified as high-energy traumas as aircraft typically maintain significant vertical and horizontal speeds at impact. The forces generated can produce a wide range of injuries, from soft tissue damage to fractures, with particular concern for neurological injury. Due to the low number of survivors, few studies in the literature specifically address spinal injury patterns. METHODS: We retrospectively evaluated thoracolumbar spinal injuries

Video-assisted thoracoscopic surgery surgical stabilization of rib fractures: surgical technique and case report.

Rib fractures are a common consequence of chest trauma. In some instances, the presence of rib fractures may cause chest instability, leading to respiratory failure, dependance from ventilatory support and increased risk of pulmonary infections. Surgical stabilization of rib fractures (SSRF) is a well-established treatment to restore the stability of the rib cage and prevent respiratory complications. While this procedure is commonly performed through an open approach that allows a wide exposure of the ribs surface, in recent years a minimally invasive procedure consisting in thoracoscopic-ass

Establishment and Validation of a Risk Prediction Model for the Superior Cluneal Nerve Entrapment Neuropathy after Thoracic or Lumbar Fracture: A Case-Comparative Study.

BACKGROUND: Superior cluneal nerve entrapment neuropathy (SCNEN) is a known complication following spinal fractures. There is a lack of comprehensive understanding of this condition, and clinical management strategies remain underdeveloped. METHODS: This study analysed the demographic and radiological data of 340 patients to develop a risk prediction model for SCNEN following surgery for thoracic or lumbar vertebral fractures. Patients were randomized into a derivation cohort (70%) and a validation cohort (30%). The least absolute shrinkage and selection operator (LASSO) regression was applie
