Comparable 2-year functional outcomes following vertebroplasty for thoracic and lumbar osteoporotic vertebral compression fractures: a retrospective cohort study.

STUDY DESIGN: Retrospective cohort study. PURPOSE: To compare short- and medium-term patient-reported outcome measures (PROMs) between thoracic and lumbar percutaneous vertebroplasty (PVP) cohorts for osteoporotic vertebral compression fractures (OVCFs). OVERVIEW OF LITERATURE: PVP is well-established for pain relief in OVCF; however, whether clinical outcomes vary by the anatomical location of the treated vertebrae, remains unclear. METHODS: Ninety patients who underwent PVP between January 2010 and August 2022 with complete 2-year PROM follow-up were included and stratified into thoracic

The prevalence of thoracic vertebral fractures identified with the support of Artificial Intelligence-assisted diagnostic methods in patients hospitalized due to internal medicine diseases.

Objectives The number of internal medicine inpatients is increasing, and elderly individuals account for most hospitalizations. Vertebral fractures should be considered in patients hospitalized for internal medicine. This study aimed to investigate thoracic vertebral fractures in patients hospitalized for internal medicine diseases. Methods This study included 2,977 patients who underwent chest computed tomography (CT). Chest CT images were reconstructed to visualize the thoracic spine. The anterior, middle, and posterior vertebral heights were measured using artificial intelligence (AI). A ve

Traumatic posterior mediastinal hematoma with left atrial compression: A case series of a rare, life-threatening entity.

BACKGROUND: Traumatic posterior mediastinal hematoma (TPMH) is a potentially life-threatening entity that, in rare instances, can lead to obstructive shock and sudden death secondary to left atrial compression (LAC). We describe the clinical course and outcomes of seven adult patients with TPMH and LAC. A brief literature review is presented. METHODS: Seven patients (ages spanning from the 20s to 80s) with TPMH and LAC were identified at a Level 1 trauma center from 2016 to 2023. The first 2 patients were identified following standard quality improvement case reviews for unexplained death. Th

Percutaneous fixation as definitive care for AO spine type B thoracolumbar fractures in polytrauma patients.

INTRODUCTION: Percutaneous pedicle screw fixation is considered the standard treatment for thoracolumbar compression fractures. Its role in unstable injuries with anterior or posterior tension band lesion (AO Spine Type B) remains controversial, as open posterior fusion is traditionally recommended. In polytrauma patients, associated injuries and hemodynamic instability may increase the morbidity of open surgery and delay definitive treatment. In this setting, percutaneous stabilization offers a minimally invasive damage-control strategy, allowing restoration of spinal stability and early mobi

The relationship between thoracic kyphosis and Q angle with balance and falls in patients with osteoporosis.

BACKGROUND: Falls drive fragility fractures in postmenopausal osteoporosis, but integrated evidence linking thoracic kyphosis, quadriceps (Q) angle, and ultrasound-derived quadriceps size indexed to body mass index to balance and self-reported fall history remains limited. We hypothesized that greater thoracic kyphosis and higher Q angle would be associated with poorer balance performance and self-reported fall history. METHODS: In this cross-sectional study between May and August 2024, 100 postmenopausal women with osteoporosis were assessed. Thoracic kyphosis was measured on lateral radiogr

Preventing fragility fractures: AI-based opportunistic osteoporosis detection in routine CT .

UNLABELLED: This study shows that an artificial intelligence tool can identify patterns of reduced bone mineral density in routine CT scans that would otherwise remain unreported. Compared to radiologists, the AI system flagged substantially more cases with potential signs of reduced bone mineral density, offering a scalable approach to support earlier identification of individuals who may benefit from further assessment. PURPOSE: Osteoporosis often goes unrecognized in routine computed tomography (CT) scans performed for unrelated indications, leading to diagnosis only after a fragility frac

Traumatic spinal injuries in a rural tourist area: Epidemiology, seasonal trends and implications for healthcare resource allocation.

PURPOSE: Several epidemiological studies have evaluated trends and epidemiology of Traumatic Spinal Injury (TSI) but often they don't take into account the geographical variability of nations. Having identified a lack of literature on the epidemiology of rural and touristic areas we conducted this retrospective study in our hospital near Como Lake to understand how best to manage TSI patients. METHODS: This retrospective epidemiological study included all patients with TSI admitted to Emergency Department (ED) of Gravedona ed Uniti Hospital between April 2018 and March 2024. We focused on ver

Association of preoperative vertebral kyphosis and other factors with cement leakage after percutaneous kyphoplasty: a retrospective cohort study.

BACKGROUND: Percutaneous kyphoplasty (PKP) is an established treatment modality for painful vertebral compression fractures; however, cement leakage remains its most frequent complication. Although factors such as cortical disruption and fracture type have been widely studied, the influence of preoperative sagittal deformity on leakage risk remains unclear. This study aimed to evaluate the association between preoperative vertebral kyphosis and cement leakage after PKP, with particular attention to CT-based bone quality. METHODS: We retrospectively reviewed patients who underwent PKP for thor

Four-level pedicle subtraction osteotomy for severe rigid thoracic hyperkyphosis: a technical case report.

STUDY DESIGN: Technical case report. INTRODUCTION: Severe thoracic hyperkyphosis caused by multiple osteoporotic fractures can lead to significant disability. When deformities are rigid, surgical correction may be required. This report describes a rare case of four-level pedicle subtraction osteotomy (PSO) to address a fixed deformity, with focus on technique, outcomes, and complications. CASE PRESENTATION: A 65-year-old female with Scheuermann's disease developed progressive thoracic hyperkyphosis due to osteoporotic vertebral fractures. Radiographs showed a rigid kyphosis of 130.3°. Is the pelvis "organ protective" in pelvic and spine trauma? A database study of spine and pelvic combination injuries.

BACKGROUND: Traumatic spine fractures often occur with multisystem injuries. Pelvic and transverse process (TP) fractures have been linked to visceral trauma, but whether the pelvis behaves as an "organ protective" structure in spine injury is unclear. OBJECTIVE: To determine how concomitant pelvic fractures and TP fracture burden influence visceral injury patterns in patients with spinal fractures. METHODS: This retrospective cohort included adult trauma patients with CT-confirmed spinal fractures at a single Level I trauma center (2015-2023). Patients were grouped into spine-only or spine-

Emergency radiology of severe acute injury in extreme alpine summer sports: a pictorial essay.

Extreme alpine summer sports are increasingly popular and expose participants to high-energy trauma in remote mountain environments. This pictorial essay reviews the emergency radiology of severe acute injuries associated with down-hill mountain biking, rock climbing, paragliding, BASE jumping, canyoning, and rafting, with emphasis on mechanism-based imaging interpretation. Injury patterns differ across disciplines but are largely determined by velocity, fall height, landing posture, impact surface, axial loading, torsional forces, and water-related hazards. Down-hill mountain biking commonly

Outcomes in Single Versus Two-Stage Surgical Stabilization of Complex Bilateral Chest Wall Injury: A Single-Center Experience.

INTRODUCTION: Surgical stabilization of rib fractures (SSRF) continues to show benefit for severe chest wall injury (CWI) with improvement in morbidity and mortality following stabilization. Complex bilateral injuries pose a clinical and technical challenge. Currently, there is no formal recommendation for an approach to repair such injuries, whether that be bilateral SSRF within the index case or staged repair. We sought to describe our institutional experience with this subset of patients, identify factors that may influence the decision to pursue single- versus two-stage procedures, and ass

CPR-induced vertebral fractures: a case report and systematic review of literature.

Cardiopulmonary resuscitation is a critical intervention for cardiac arrest, but its forceful chest compressions can lead to skeletal injuries, including rare vertebral fractures CASE PRESENTATION: This case report details a 78-year-old woman with a history of cardiac surgery and obesity who suffered a fatal cardiac arrest due to a DeBakey type II aortic dissection. Despite 30 minutes of advanced manual CPR, she succumbed to cardiogenic shock. Autopsy revealed an incidental transvertebral fracture of the 8th thoracic vertebra alongside multiple bilateral rib fractures, with no evidence of pre-

Rib Plating Versus Nonsurgical Treatment of Flail Chest: Comparison of Adverse Events in More than 12,500 Patients.

INTRODUCTION: Guidelines for flail chest, defined as three or more segmental rib fractures, remain largely consensus based with variable supporting evidence. Prior work suggests that surgical management may reduce mortality and pulmonary complications, although existing data are often underpowered and include nonflail fracture patterns. METHODS: This retrospective, matched cohort study leveraged the PearlDiver M170 data set. Flail chest was identified using International Classification of Disease codes, and surgical treatment (rib plating) with Current Procedural Terminology codes. Patients w

Clinical features, diagnostic imaging findings, treatment, and outcome of a cat with C4 vertebral fracture and concomitant tracheal rupture.

BACKGROUND: Vertebral fractures are relatively common in cats, affecting the thoracic or lumbar spine more frequently than the cervical region. Patient history and clinical signs at presentation often guide the initial diagnostic suspicion; however, advanced imaging techniques are usually essential for establishing a definitive diagnosis. The management of these cases is often complex, and not all patients require surgical intervention. When surgery is indicated, the primary goal is to decompress the spinal cord and prevent further secondary injury. CASE DESCRIPTION: A 3-year-old female domes

Strategic four-dimensional reconstruction in cervical exenteration: A preventive reconstructive framework for anticipating and managing defects.

BACKGROUND: Anterior mediastinal tracheostomy (AMTr) following cervical exenteration creates massive, three-dimensional defects with high morbidity, particularly catastrophic great vessel rupture. To mitigate these risks, we developed a hierarchical conceptual framework categorizing reconstructive objectives into primary (immediate form and functional restoration), secondary (intraoperative complication prevention), and tertiary (postoperative functional and structural maintenance) goals. METHODS: We retrospectively reviewed 9 patients (mean age, 66.7 years) who underwent cervical exenteratio
