Floating shoulder associated with multiple rib fractures with cervical dorsal spine fractures and lung injury has not been reported in the literature. Patient underwent single stage elective clavicle and scapula fracture stabilization. Good functional outcome at 6 months postoperative with near normal range of movements. Surgery allows early mobilization and better functional outcome. A CT scan of thorax and spine is highly recommended in such high velocity shoulder injuries to detect associated rib, spinal fractures and lung injury.

PURPOSE: In patients with thoracolumbar burst fractures, surgical delay is often unavoidable due to concomitant injuries. Concerns persist that delayed surgery may compromise indirect reduction of retropulsed fragments via ligamentotaxis. This study aimed to determine whether the timing of surgery affects the quality of intracanal fragment reduction and identify independent predictors of successful reduction following short-segment posterior fixation. METHODS: This multicenter study included 252 patients who sustained a single traumatic thoracolumbar burst fracture and underwent short-segment

BACKGROUND: The thoracic vertebrae, ribs, and sternum, provide structural and protective support for the upper body. The unique framework of the region suggests that fractures involving both the thoracic spine and sternum occur with notable spinal instability. Despite their clinical relevance, the short-term medical and surgical outcomes of combined sternal and thoracic spinal fractures remain poorly understood compared with thoracic fractures alone. This study aims to compare the 90-day complication rates and surgical intervention requirements in patients with concomitant sternal and thoracic

INTRODUCTION: Aircraft crashes are infrequent but often associated with high mortality, morbidity, and only a limited number of survivors. These events are classified as high-energy traumas as aircraft typically maintain significant vertical and horizontal speeds at impact. The forces generated can produce a wide range of injuries, from soft tissue damage to fractures, with particular concern for neurological injury. Due to the low number of survivors, few studies in the literature specifically address spinal injury patterns.
METHODS: We retrospectively evaluated thoracolumbar spinal injuries

BACKGROUND: Superior cluneal nerve entrapment neuropathy (SCNEN) is a known complication following spinal fractures. There is a lack of comprehensive understanding of this condition, and clinical management strategies remain underdeveloped.
METHODS: This study analysed the demographic and radiological data of 340 patients to develop a risk prediction model for SCNEN following surgery for thoracic or lumbar vertebral fractures. Patients were randomized into a derivation cohort (70%) and a validation cohort (30%). The least absolute shrinkage and selection operator (LASSO) regression was applie

ObjectiveThoracolumbar fractures (T11-L2) are common spinal injuries, and accurate classification is essential for treatment planning. However, diagnosis remains challenging because of complex fracture morphology and variability in clinical experience. This study evaluated the feasibility of a deep learning framework for automated classification of thoracolumbar fractures using CT images.MethodsA retrospective study was conducted using CT images from 596 patients with thoracolumbar fractures. A convolutional neural network based on a Multiple Instance Learning (MIL) framework was developed, us

BACKGROUND: Blunt chest trauma (BCT) substantially contributes to morbidity and mortality globally, especially in elderly adults. Most studies on BCT in older adults were conducted in Western countries, not reflecting regional differences. The study aimed to examine the incidence, injury patterns, management, and outcomes of BCT in older adults in the United Arab Emirates (UAE).
PATIENT AND METHODS: Data of patients aged ≥60 years with BCT admitted to a community-based hospital in 2014-2020 were retrospectively analyzed, including age, sex, nationality, mechanism of injury, vital signs, assoc

Instrumented spinal fusion is a common treatment for degenerative spinal disease but may lead to progressive mechanical and biological complications, especially in long multilevel constructs. We report the case of a 62-year-old woman who developed serial adjacent segment degeneration, pseudarthrosis, implant failure, vertebral fractures, and progressive deformity over 11 years following posterior lumbar fusion. Multiple revision surgeries with stepwise cranial extension to the upper thoracic spine were required. Despite advanced fixation techniques-including multi-rod constructs, iliac fixatio

Treatment of severely injured patients is challenging, and timely reading of whole-body computed tomography (WBCT) images therefore crucial. Artificial intelligence is increasingly used to prioritize and detect acute injuries in this context. Algorithms focusing on the cervical spine and compression fractures have been deployed successfully. However, tools for whole spine assessment and the entirety of fracture morphologies are lacking. We aimed to investigate the capabilities of an algorithm to detect spine fractures on WBCTs and factors contributing to the difficulties in its development. A

BACKGROUND: Data on the impact of vertebral fractures on lung function in interstitial lung diseases (ILDs) are limited. This study aimed to evaluate the association between vertebral fractures, quantified by the spinal deformity index (SDI), and pulmonary function parameters, independently of ILD pattern and thoracic morphometric indices. METHODS: This cross-sectional study included adult patients diagnosed with ILD who underwent high-resolution computed tomography (HRCT) and pulmonary function tests (PFTs). PFTs included absolute and percent predicted values of forced vital capacity (ppFVC),

PURPOSE: Treatment of neurologically intact thoracolumbar burst fractures (AO types A3/A4) is controversial. Furthermore, the impact that concomitant posterior ligamentous complex (PLC) injuries have on patient reported outcome measures (PROMs) is not well-established. This analysis compared outcomes between patients with A3/A4 injuries with and without concomitant B1/B2 injuries and between patients treated with operative versus nonoperative management.
METHODS: Data from a prospective multicenter cohort study (ClinicalTrials.gov: NCT02827214) was used. Adults with AO type A3/A4 burst fractu

Vertebral compression fractures, typically caused by trauma or osteoporosis, are generally treated through conservative care, including absolute bed rest and thoracolumbosacral orthosis. However, prolonged bed rest can lead to muscle atrophy, functional impairment, and reduced quality of life. Thus, alternative management approaches for spinal compression fractures are needed. Few reports have examined the effectiveness of integrative Korean medicine treatment in patients with traumatic multiple compression fractures. This case report describes a 51-year-old Korean male patient who presented t

STUDY DESIGN: Retrospective cohort study.
PURPOSE: To compare short- and medium-term patient-reported outcome measures (PROMs) between thoracic and lumbar percutaneous vertebroplasty (PVP) cohorts for osteoporotic vertebral compression fractures (OVCFs).
OVERVIEW OF LITERATURE: PVP is well-established for pain relief in OVCF; however, whether clinical outcomes vary by the anatomical location of the treated vertebrae, remains unclear.
METHODS: Ninety patients who underwent PVP between January 2010 and August 2022 with complete 2-year PROM follow-up were included and stratified into thoracic

Objectives The number of internal medicine inpatients is increasing, and elderly individuals account for most hospitalizations. Vertebral fractures should be considered in patients hospitalized for internal medicine. This study aimed to investigate thoracic vertebral fractures in patients hospitalized for internal medicine diseases. Methods This study included 2,977 patients who underwent chest computed tomography (CT). Chest CT images were reconstructed to visualize the thoracic spine. The anterior, middle, and posterior vertebral heights were measured using artificial intelligence (AI). A ve

BACKGROUND: Traumatic posterior mediastinal hematoma (TPMH) is a potentially life-threatening entity that, in rare instances, can lead to obstructive shock and sudden death secondary to left atrial compression (LAC). We describe the clinical course and outcomes of seven adult patients with TPMH and LAC. A brief literature review is presented.
METHODS: Seven patients (ages spanning from the 20s to 80s) with TPMH and LAC were identified at a Level 1 trauma center from 2016 to 2023. The first 2 patients were identified following standard quality improvement case reviews for unexplained death. Th

INTRODUCTION: Percutaneous pedicle screw fixation is considered the standard treatment for thoracolumbar compression fractures. Its role in unstable injuries with anterior or posterior tension band lesion (AO Spine Type B) remains controversial, as open posterior fusion is traditionally recommended. In polytrauma patients, associated injuries and hemodynamic instability may increase the morbidity of open surgery and delay definitive treatment. In this setting, percutaneous stabilization offers a minimally invasive damage-control strategy, allowing restoration of spinal stability and early mobi

BACKGROUND: Falls drive fragility fractures in postmenopausal osteoporosis, but integrated evidence linking thoracic kyphosis, quadriceps (Q) angle, and ultrasound-derived quadriceps size indexed to body mass index to balance and self-reported fall history remains limited. We hypothesized that greater thoracic kyphosis and higher Q angle would be associated with poorer balance performance and self-reported fall history.
METHODS: In this cross-sectional study between May and August 2024, 100 postmenopausal women with osteoporosis were assessed. Thoracic kyphosis was measured on lateral radiogr

UNLABELLED: This study shows that an artificial intelligence tool can identify patterns of reduced bone mineral density in routine CT scans that would otherwise remain unreported. Compared to radiologists, the AI system flagged substantially more cases with potential signs of reduced bone mineral density, offering a scalable approach to support earlier identification of individuals who may benefit from further assessment.
PURPOSE: Osteoporosis often goes unrecognized in routine computed tomography (CT) scans performed for unrelated indications, leading to diagnosis only after a fragility frac
