INTRODUCTION: Low bone mineral density (BMD), a history of vertebral fractures (VFs), and steroid use are established risk factors for VFs. Additionally, age, nutritional status, muscle mass, and spinal sagittal alignment have been linked to osteoporosis and fractures. This study aims to investigate the risk factors contributing to new occurrences of VFs.
METHODS: We included 597 patients with osteoporosis who visited our outpatient department and were available for follow-up 1 year after the visit. The following data were collected: age at examination, presence of secondary osteoporosis, bod

PURPOSE: Multiple myeloma (MM), a type of B-cell non-Hodgkin lymphoma, frequently causes osteolytic spinal lesions, increasing the risk of fracture and poor sagittal spinopelvic alignment. In relapsed patients, these changes may contribute to increased pain and functional impairment.
METHODS: The clinical data of 86 MM patients (48 males, 38 females) who underwent ≥ 3 whole-body CT scans between 2009 and 2024, with a mean interscan interval of 19.2 months and a total follow-up duration of 37.5 months, were analyzed in this retrospective study. Spinopelvic parameters were measured, and clinica

A young male patient was brought to the Emergency Department after falling from a moving car. It was noted that he had a left scalp hematoma with a deformed right upper limb. Computer tomography of the head, cervical spine, thoracic and lumbar spine, thorax, abdomen, and pelvis was performed. The CT scan showed multiple cervical spine fractures, a fracture of the first rib with localized pneumothorax, and multiple lung contusions. There was also a fracture of the right humerus. CT angiography showed evidence of a complete cutoff of the right axillary artery at its origin in the axilla, which w

A 73-year-old woman with a remote history of successfully treated primary hyperparathyroidism was referred to our office because of multiple skeletal fractures and proximal muscle weakness. Prior laboratory evaluation demonstrated hypocalcemia, vitamin D deficiency, elevated PTH, and elevated alkaline phosphatase. Updated evaluation in our clinic additionally showed hypophosphatemia prompting measurement of serum fibroblast growth factor 23 (FGF23). FGF23 proved to be markedly elevated prompting consideration of tumor-induced osteomalacia. Positron emission tomography with Cu-64 oxodotreotide

BACKGROUND: Percutaneous vertebroplasty (PVP) is a minimally invasive procedure frequently employed to treat osteoporotic vertebral compression fractures in the elderly. However, bone cement leakage is a complication that cannot be completely avoided of PVP, which may lead to disastrous consequences. We present a rare case of small intestinal necrosis caused by embolism of the superior mesenteric artery following PVP. Through this case report, we aim to raise awareness among all medical professionals involved in this procedure about the importance of preventing cement leakage during PVP and to

Bone is the site of metastases for many cancers during tumor pathogenesis. The incidence of new cases of bone metastases is estimated to be about 280,000 annually in the United States. Most notably, breast (65% to 75%), prostate cancer (65% to 90%), lung cancer (17% to 64%), and to a lesser extent, bladder cancer (40%), thyroid cancer (65%), melanoma (14% to 45%), kidney cancer (20% to 25%), and colorectal cancer (10%) favor metastases to the bone. Multiple myeloma accounts for about 70% to 95% of bone lesions. The structural bone damage that ensues leads to significant pain, pathologic fractu

The objective of this study was to compare the kinematics of the head-neck, torso, pelvis, and lower extremities and document injuries and their patterns to small female occupants in frontal impacts with upright and reclined postures using an experimental model. Six postmortem human surrogates (PMHS) with a mean stature of 154 ± 9.0 cm and mass of 49 ± 12 kg were equally divided between upright and reclined groups (seatback: 25 deg and 45 deg), restrained by a three-point integrated belt, positioned on a semirigid seat, and exposed to low and moderate crash velocities (15 km/h and 32 km/h resp

BACKGROUND: Rib fixation for traumatic rib fractures is advocated to decrease morbidity and mortality in select patient populations. We intended to investigate the effect of combination osseous thoracic injuries on mortality with the hypothesis that combination injuries will worsen overall mortality and that SSRF will improve outcomes in combination injuries and in high-risk patients.
METHODS: Patients with rib fractures were identified from the Trauma Quality Improvement Project registry from 2019. Patients were then divided into rib fracture(s) alone or in combination with sternal, thoracic

RATIONALE: Aggressive vertebral hemangiomas (AVHs) destroy continuous vertebral bodies and intervertebral discs and resulting in spinal kyphosis is extremely rare. The very aggressive behavior was attributable to its significant vascular component and contained no adipose tissue.
PATIENT CONCERNS: We report a case of thoracic spine kyphosis of AVHs with multiple vertebral bodies and intervertebral disc destruction in a 45-year-old woman.
DIAGNOSES: Based on the imaging studies, the patient underwent surgical removal of this lesion and spinal reconstruction. Histopathology consistent with ver

Non-traumatic fractures due to seizures are an overlooked diagnostic group. It is well known that patients with generalized tonic-clonic seizures have an increased trauma risk. However, the cause of fracture is rarely due to the violent forces of muscle contractions. Usually, the primary patient examination focuses on the aetiology of the seizure, which sometimes delays the diagnosis of fractures. This is a case report of a 19-year-old woman who sustained three compression fractures of the thoracic spine due to a generalized tonic-clonic seizure, and a discussion of the diagnostic challenges i

BACKGROUND: The timing of major fracture care in polytrauma patients has a relevant impact on outcomes. Yet, standardized treatment strategies with respect to concomitant injuries are rare. This study aims to provide expert recommendations regarding the timing of major fracture care in the presence of concomitant injuries to the brain, thorax, abdomen, spine/spinal cord, and vasculature, as well as multiple fractures.
METHODS: This study used the Delphi method supported by a systematic review. The review was conducted in the Medline and EMBASE databases to identify relevant literature on the

Percutaneous vertebroplasty (PVP) is a surgical procedure that involves injecting polymethylmethacrylate (PMMA) bone cement into the diseased vertebrae to rapidly relieve pain and strengthen the vertebrae. We reported a 73-year-old patient who underwent percutaneous vertebroplasty (PVP) surgery for thoracolumbar vertebral compression fracture. After the surgery, the patient experienced symptoms such as chest tightness and dyspnea. Further examination revealed multiple high-density foreign bodies in the blood vessels/heart and concomitant multi-organ dysfunction. It was considered that the mult

BACKGROUND: Chylothorax is an uncommon condition defined by the escape of lymphatic fluid into the pleural space originating from the thoracic duct.
CASE DESCRIPTION: Our case involves a male patient in his 60s who developed traumatic chylothorax after being involved in a bicycle collision. The total body computed tomography (CT) showed multiple fractures of the ribs and spine, including a fracture of the anterior column of the Th12 vertebra. The patient was placed under observation in the intensive care unit and because of the instability of the Th12 fracture operative stabilization was perf

In recent post-mortem human subjects (PMHS) studies in a high-speed rear-facing frontal impact (HSRFFI), the PMHS sustained multiple rib fractures. The seatback structure and properties of the seats might contribute to these fractures. This study aimed to determine if a homogeneous rear-facing seat with foam-covered seatback would mitigate the risk of thoracic injury during an HSRFFI. Three male PMHS were subjected to the same previous HSRFFI pulse. The seating structure consisted of a homogeneous seatback composed of rigid plates with load cells and covered with both comfort and safety foam.

INTRODUCTION: Pelvic fractures (PF) occur in up to 9 % of trauma cases, primarily from high-impact events, and are associated with increased morbidity and mortality due to frequent concomitant injuries. Thoracolumbar (TL) spinal fractures, particularly at the T10-L2 junction, are also common in high-energy trauma but are less frequently examined in association with PF. Missed TL fractures can lead to serious neurological deficits. Although Advanced Trauma Life Support (ATLS) recommends screening for TL fractures in trauma patients, no standardized guidelines exist. This study aims to explore t

Osteogenesis imperfecta (OI) is an uncommon bone disorder caused by mutations in type I collagen involved in bone matrix leading to increased fracture risk. There are several sub-categories within OI, with OI type I being the most common and mildest form. Women with OI considering pregnancy need to be aware of bone loss and fracture risk, particularly with lactation. We report the first case of a female with twin pregnancy and OI type I who presented with multiple vertebral fractures following delivery and postpartum lactation. Following endocrine review, she weaned breast-feeding but represen
