Fatal thoracic aortic transection following a motor vehicle collision: Medico-legal significance of the seat belt sign.

Traumatic thoracic aortic transection is one of the most lethal consequences of blunt chest trauma; it is commonly associated with high-speed deceleration injuries in road traffic collisions. Despite advances in trauma management, survival rates remain poor due to rapid internal haemorrhage and the presence of multiple associated injuries. We report a fatal case of thoracic aortic transection in a 50-year-old restrained male driver involved in a high-velocity head-on collision with a truck. Following admission, he presented with hypotension, left-sided flail chest, multiple long bone fractures

Patient centered outcomes after surgical stabilization of rib fractures: A prospective cohort study.

INTRODUCTION: Rib fractures are associated with pain, impaired physical function, and reduced quality of life. Surgical stabilization of rib fractures (SSRF) is established for flail chest and ventilator-dependent patients, but its impact on longer-term patient-reported outcomes in broader injury patterns remains unclear. We evaluated whether SSRF is associated with improved physical functioning and pain outcomes compared to nonoperative management. METHODS: We conducted a prospective observational cohort study of trauma ICU patients with rib fractures admitted between June 2023 and August 20

Intrathoracic rib fixation in flail chest: first South American experience.

Intrathoracic rib fixation represents a technical innovation with advantages over traditional methods through minimally invasive approach. We present the first South American case in a 21-year-old patient with flail chest from polytrauma. Four intrathoracic fixation plates (two 90 mm anterior and two 60 mm posterior) were used through video-assisted anterolateral and posterior approaches. The technique facilitates reduction through guide cables, intrathoracic positioning with minimal muscular disruption and automatic adaptation to costal anatomy. Total surgical time was 155 minutes with 250 mL

Early complications after surgical stabilization of rib fractures: A retrospective cohort study.

BACKGROUND: In recent years, interest in SSRF has increased due to the growing evidence of improved pain control, quality of life, and chest wall function. This study aimed to assess the incidence and the types of early complications (in-hospital and until 3 mo postoperatively) following SSRF and to develop a risk prediction score based on potential risk factors for complications. METHODS: We conducted a retrospective single-center analysis including 554 patients who underwent SSRF between 2008 and 2023. Data were extracted from electronic medical records and included demographic, trauma-rela

A 3D Numerical Model of Ultrasonic Transthoracic Propagation for Cardiac Focused Ultrasound Therapy.

OBJECTIVE: Non-invasive focused ultrasound therapies of abdominal organs, including the heart and the liver, have emerged in the last decades. Transthoracic focusing of ultrasound poses challenges such as pressure loss and aberrations. Numerical models of ultrasonic propagation have been developed to study the focalization in heterogeneous tissues, particularly for transcranial applications. However, ribcage models were less studied than skull models, and no experimental validation of ribcage models has been performed so far. METHODS: Both linear and nonlinear k-space simulations were used to

Parasternal Intercostal Block Catheterization Provides Successful Analgesia and Facilitates Respiratory Recovery in Flail Chest After Cardiopulmonary Resuscitation: A Case Report.

Flail chest resulting from multiple rib fractures after cardiopulmonary resuscitation (CPR) can cause pain and respiratory distress. In this case report, we describe the use of bilateral parasternal intercostal block catheters in a patient who was unable to breathe adequately and could not be extubated due to rib fractures after CPR. The patient received analgesia via the parasternal catheter for 11 days and was discharged on the 17th day. This report highlights the importance of successful pain management through continuous analgesia via a continuous parasternal intercostal block catheter, th

Management of Chest Wall Injuries: An Updated Review.

Injuries to the chest wall are common after blunt thoracic trauma, which can cause injury to the ribcage and intrathoracic structures. Such trauma can result in a spectrum of injuries, ranging from minor injuries such as an isolated rib fracture to extensive injuries such as multiple rib fractures/flail segments with underlying lung or intrathoracic injury. The primary focus of this article will be on the assessment and treatment of injuries of the bony chest wall, with an emphasis on flail chest and multiple rib fractures. Patient evaluation, treatment, and outcomes differ depending on the se

Surgical treatment of traumatic manubrio-sternal dislocation type I associated with chondrosternal joint rupture.

Manubrio-sternal dislocations (MSDs) are a rare entity that requires high-intensity energy trauma. It can be associated with complex fractures that can be challenging to manage. We aim to present our surgical approach to an MSD associated with multiple chondrosternal joint fractures and underline the mechanism of these rare traumatic chest wall damages. A 39-year-old male suffered from a forklift truck mishap. He arrived at our tertiary center with posterior MSD and chondrosternal joint fracture, lung contusion, pneumothorax, and right humerus fracture. He underwent an open reduction of the st

Not All Black and White: Is Routine Chest Radiography Following Rib Fractures Beneficial?

INTRODUCTION: Our institution recommends patients suffering ≥3 traumatic rib fractures, or any number of fractures with hemothorax (HTX), pneumothorax (PTX), or tube thoracostomy placement during admission follow-up for outpatient chest radiography (oCXR). We assessed the utility of oCXR by identifying risk factors for abnormal radiographic findings and need for postdischarge interventions. METHODS: This was a retrospective study of 182 adult patients presenting to a single level 1 trauma center with isolated chest wall injuries and had oCXR within 30 d postdischarge. The primary outcomes wer

Effect of lateral positioning on ventilation in patients with blunt thoracic injury during pressure support ventilation: the VICTORY study.

BACKGROUND: In patients with blunt thoracic injury requiring mechanical ventilation lateral positioning is routinely performed. Whether it modifies ventilation distribution and aeration is unclear. STUDY DESIGN AND METHODS: Patients receiving pressure support ventilation (PSV) were positioned 30 degrees on each side for 30 min. Electrical impedance tomography (EIT) was used to quantify the percentage of right and left ventilation. Secondary outcomes included right and left tidal volume and the modified lung ultrasound score. At baseline, patients were categorized according to ventilation dist

The impact of injury severity and age on short-and long-term mortality and hospital length of stay after surgical stabilisation of rib fractures (SSRF): a German population-based propensity-score matched investigation.

BACKGROUND: Surgical stabilisation of rib fractures (SSRF) is a procedure that has been shown to reduce mortality and complications in patients with thoracic trauma. However, correct patient selection is required, since the immunological hit of a thoracic surgery in the context of trauma is not without risk. In this study we aimed to analyse real-world data on the effect of SSRF mortality and hospital length of stay. METHODS: A retrospective propensity matched analysis of real-world health claims data of a German statutory health insurance company of Management of serial rib fractures and uns

Mechanical Ventilation in Trauma-Induced Respiratory Failure: A Clinical Review.

Trauma is a leading cause of mortality in young adults, with thoracic injuries playing a central role in respiratory failure and ARDS. Pulmonary contusion is the principal risk factor, although extra-thoracic injuries-including traumatic brain injury, abdominal trauma, and long bone fractures-further increase ARDS incidence and complicate management. Early application of damage control strategies, including judicious fluid resuscitation and balanced blood product transfusion, can mitigate these risks. This review examines principles of mechanical ventilation in trauma-induced ARDS, with emphas

A Multicenter Randomized Controlled Trial on the Surgery of Fractured Ribs: The SOFRIB Trial.

OBJECTIVES: To evaluate whether surgical stabilization of rib fractures (SSRF) improves acute and patient-centered outcomes in adults with clinically significant rib fractures. SUMMARY OF BACKGROUND DATA: Rib fractures occur in up to 40% of patients with blunt trauma and contribute substantially to morbidity and health-care cost. SSRF benefits patients with flail chest, but its role in broader injury patterns remains uncertain. METHODS: SOFRIB was a multicenter, parallel-group, randomized controlled trial at nine American College of Surgeons-verified trauma centers (2014-2024). Adults (≥18 y

A Contemporary Review of Operative Indications for Chest Wall Trauma.

Chest wall trauma, including rib and sternal fractures, is prevalent and a source of morbidity and mortality in trauma patients. While many injuries are managed non-operatively, surgical stabilization of flail segment rib fractures has shown benefits in ventilator weaning, pneumonia reduction, and functional recovery. However, evidence for operative management in non-flail injuries remains inconsistent, with outcomes often influenced by patient selection, injury pattern, and institutional expertise. Minimally invasive and thoracoscopic techniques continue to evolve, but their adoption is limit

Timing of Tracheostomy in Critically Ill Geriatric Trauma Patients: A Nationwide Analysis.

INTRODUCTION: Tracheostomy is frequently performed in trauma patients requiring prolonged mechanical ventilation. The optimal timing in older people, with limited physiologic reserve, remains unclear. Our study aims to evaluate whether early tracheostomy (ET) versus late tracheostomy (LT) is associated with differences in in-hospital complications and length of stay. METHODS: We conducted a retrospective cohort study using American College of Surgeons Trauma Quality Improvement Program (2017-2023) on geriatric trauma patients (≥65 years) who underwent ET with an injury severity score (≥9). Assessment of Pendelluft Using Electrical Impedance Tomography in a Patient With Flail Chest.

An 80-year-old man with flail chest exhibiting paradoxical chest wall movement required mechanical ventilation for internal fixation. Despite the apparent paradoxical motion resolution, unstable chest wall segments allowed pendelluft to persist. Thus, EIT-guided management of flail chest should focus on stabilising the visible chest wall and achieving homogenised ventilation dynamics.

Operative management of unstable sternal fracture in polytrauma: a case report and literature review.

Sternal fractures are uncommon but clinically relevant injuries, most often resulting from blunt thoracic trauma in motor vehicle collisions. While most isolated fractures can be treated conservatively, surgical stabilisation may be required in cases of displacement, instability, or polytrauma. Modern plating techniques offer biomechanical advantages over traditional wire fixation, but indications remain variably defined. We report the case of a 33-year-old male polytrauma patient involved in a high-impact motor vehicle accident. He sustained a displaced sternal fracture with manubriosternal d

An Overview of Meta-Analyses on the Surgical Stabilization of Rib Fractures in Adults: A Narrative Umbrella Review (2020-2025).

Background: Rib fractures are a common cause of morbidity in trauma patients. The surgical stabilization of rib fractures (SSRF) has gained increasing attention as a therapeutic option; however, evidence from multiple meta-analyses remains heterogeneous. Methods: We performed an overview of 11 meta-analyses, including a total of 1,117,849 adult patients (narrative umbrella review), published between November 2020 and November 2025 to summarize and critically appraise high-level evidence comparing SSRF with non-operative management (NOM) in adults with traumatic rib fractures.
