Development and validation of a nomogram for predicting tracheostomy risk in traumatic cervical spinal cord injury.

BACKGROUND: Tracheostomy is common in traumatic cervical spinal cord injury (TCSCI) because of respiratory complications, yet objective tools to estimate individual risk remain limited. METHODS: In this single-center retrospective cohort at the Second Affiliated Hospital, Zhejiang University School of Medicine, we enrolled 308 consecutive ICU admissions with TCSCI (January 2018-March 2023) and randomly split the cohort 7:3 (outcome-stratified) into training (n = 215) and validation (n = 93) sets. Candidate admission predictors were screened with Least Absolute Shrinkage and Selection Operator

A new approach to the lower cervical-thoracic spine with dislocation of the sterno-clavicular joint: FAMA (Fast Anterior Medium Approach).

PURPOSE: To evaluate the Fast Anterior Medium Approach (FAMA) as an alternative to traditional anterior cervico-thoracic approaches, enhancing access to the C7-T1-T2 and T2-T4 junctions while minimizing postoperative morbidity. The cervico-thoracic junction is one of the most challenging regions to access surgically due to its deep location and proximity to critical neurovascular structures. Conventional approaches, including postero-lateral thoracotomy and transmanubrial techniques, are associated with high morbidity. The FAMA technique was designed to provide enhanced exposure while reducing

Successful open repair of blunt traumatic thoracoabdominal aortic injury with concomitant L1/L2 fracture-dislocation.

BACKGROUND: Although rare, blunt traumatic aortic injury occurs in the aortic isthmus and rarely in the thoracoabdominal aorta. CASE PRESENTATION: A 56-year-old man became unconscious after falling from the third floor during a suicide attempt. Computed tomography revealed a thoraco-abdominal aortic injury (pseudo-aneurysm) and a posterior dislocation fracture at L1/L2. Thoraco-abdominal aortic graft replacement was performed 3 d after the injury, and posterior spinal fusion was performed 10 d later. CONCLUSIONS: Thoracic endovascular aortic repair is usually performed for blunt traumatic ao

Preoperative C2-tilt as a potential indicator of postoperative trunk shift correction in congenital cervical/cervicothoracic scoliosis.

INTRODUCTION: Shifting of the trunk towards the convex side of the scoliotic deformity (trunk shift, TS), tilting of the head (head tilt, HT), coronal atlantoaxial dislocation (CAAD), and secondary scoliotic curves are clinical and radiographic findings usually seen in children with congenital cervical or cervicothoracic scoliosis. It may be anticipated, that secondary curves, TS, HT and CAAD are compensatory mechanisms to bring the spine into a balanced position and to enable horizontal gaze. Nevertheless, these mechanisms are frequently not sufficient. Surgery for congenital cervical or cerv

High-energy traumatic spondyloptosis at T8-T9 with complete spinal cord injury: a case report.

INTRODUCTION: Spondyloptosis, the most severe form of spondylolisthesis, involves complete (>100%) anterior or posterior displacement of one vertebra over the subjacent segment, resulting in total anatomical dislocation. Typically caused by high-energy trauma, it leads to severe spinal instability, bony fragment intrusion into the canal, and significant neurological deficits. This report presents a representative case of T8-T9 spondyloptosis with complete spinal cord injury [American Spinal Injury Association (ASIA) Impairment Scale Grade A]to analyze its injury features, surgical approach, an

Life-threatening spinal cord injury and occult intercostal artery rupture following low-energy trauma in a patient with ankylosing spondylitis: a case report.

BACKGROUND: Ankylosing spondylitis (AS) significantly increases vulnerability to severe spinal injuries from minor trauma due to rigid "bamboo spine" morphology and secondary osteoporosis. Isolated intercostal artery rupture without rib fractures is extremely rare in general trauma populations. The concurrent occurrence of multilevel spinal fractures with complete spinal cord injury and occult intercostal artery hemorrhage following low-energy blunt trauma in AS patients has rarely been documented, and the underlying pathophysiological mechanisms remain unclear. CASE PRESENTATION: A 43-year-o

Riding the shockwave: Escalating spine injuries from electric micromobility devices in the United States.

BACKGROUND: Electric micromobility vehicle (electric scooter, bicycle, hoverboard, skateboard) usage has risen significantly since the mid-2010s, and is associated with rising orthopaedic trauma. Having a narrow base, upright posture, and allowing rapid changes in velocity, these vehicles cause disproportionately more head and spinal injuries than traditional bicycles. Understanding longitudinal trends in emergency department presentations of electric micromobility vehicle related spine injuries provides clinicians with tools to anticipate cases and develop spine-injury prevention strategies a

Iturin A disrupts Cryptococcus neoformans membrane integrity, attenuates virulence, and synergizes with amphotericin B.

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

Evaluating cervical dislocation methods, without using tension on the tail, for humanely killing adult laboratory mice.

Cervical dislocation (CD) is a widely used method worldwide for humanely killing adult laboratory mice in accordance with national legislation, such as the Animals (Scientific Procedures) Act 1986 in the UK. However, concerns have been raised regarding the limitations and potential risks associated with CD, including a reported failure rate of up to 20% and the risk of injury to the thoracic or lumbar spine region. To address these concerns, we have adopted a CD method that avoids the use of tension on the tail or any additional tools. In this study, we detail our process of validation through

High Risk Predictors for Thoracic Aortic Injury in Pediatric Blunt Chest Trauma.

INTRODUCTION: Incidence of Blunt Thoracic Aortic Injuries (BTAIs) in children is low (0.03%) but has 40%-66% mortality [1, 2]. The gold standard for BTAI diagnosis is chest computed tomography angiogram. However, significant radiation exposure makes routine chest computed tomography angiogram for BTAI screening in children inappropriate. The purpose of this study is to identify predictors for children at high risk for BTAI based on simple history, clinical exam, and x-ray findings. METHODS: In this retrospective cohort study, the Trauma Quality Improvement Program database from 2017 to 2023 w

Early revision rate after posterior spinal fusion surgery increases with every additional BMI point.

PURPOSE: Revision surgery has a profound impact on patient outcomes and is a crucial consideration in the assessment of healthcare burden following spine surgery. In this context, obesity is a significant factor influencing the rate of revision surgery. To elucidate the impact of obesity on the risk of early revision surgery after posterior fusion of the thoracic and lumbar spine, we conducted a prospective single-institution cohort study. METHODS: Over a 24-month period a total of 227 consecutive patients who underwent posterior thoracolumbar spinal fusion surgery involving up to four segmen

Patients Undergoing Multilevel Thoracolumbar Fusions With Prior Total Hip Arthroplasty Are at Higher Risk for Prosthetic Dislocations.

BACKGROUND: As the number of patients undergoing both total hip arthroplasty (THA) and lumbar spinal fusion rises, clinicians must gain a stronger understanding of the biomechanical and clinical associations between these two procedures. This study compared single versus multilevel spinal fusion on spinopelvic parameters, clinical outcomes, and THA dislocation rates in patients with existing THAs. METHODS: Patients with an existing THA undergoing elective spinal fusion were retrospectively identified at a single academic center. Cohorts were stratified by fusion construct length (single or mu

Traumatic Bilateral Perched Facet of the Thoracic Spine With Associated Vertebral Fracture: A Case Report.

Bilateral perched facets of the cervical spine are relatively common in trauma cases; however, similar injuries in the thoracic spine are exceedingly rare due to the inherent stability provided by the rib cage and associated ligamentous structures. We report a rare case of bilateral perched facets at the T10/11 level in a 17-year-old female gymnast who fell from a height of 3 m during training. She presented with severe back pain but no neurological deficits and was ambulatory on arrival. Imaging revealed bilateral perched facets without fracture of facet joints or spinal cord compression, but
